Obstruction of right ventricular outflow tract caused by intracavitary metastatic disease: analysis of 14 cases.

Obstruction of the right ventricular outflow tract from metastatic disease is rare. Eleven previous case reports and three new cases are presented. Two tumor types (pancreas and breast), not previously associated with right ventricular outflow tract obstruction, are included. Congestive symptoms, systolic murmur and right axis deviation or right bundle branch conduction abnormality were universal features. Echocardiography is valuable in the delineation of metastatic cardiac involvement and the detection of intracardiac gradients. Adverse hemodynamic consequences developed in 3 of 10 patients

Recognition of developing left ventricular outflow tract obstruction in complete transposition of the great arteries.

In the months following balloon atrial septostomy for complete transposition of the great arteries (TGA), clinical deterioration is often ascribed to closure of the septostomy. Of 16 neonates with TGA, 5 showed early clinical deterioration. Each had an adequate septostomy at operation. Deterioration was characterized clinically by increasing cyanosis and radiographically by progressive diminution in pulmonary vascularity. Subsequent catheterization and axial cineangiography demonstrated that pulmonary flow had decreased, probably secondary to a progressive form of left ventricular outflow trac

[Transposition of the great arteries: anatomic types of left ventricular outflow tract obstruction (author's transl)].

A postmortem investigation has been carried out in 48 heart specimens with transposition of the great arteries (TGA) to evaluate incidence and type of left ventricular outflow tract obstruction. The frequence of pulmonary stenosis in our series (23%) is similar to those reported from previous studies. Cases have been divided in two groups according to presence or absence of ventricular septal defect. In cases with intact ventricular septum the obstruction was determined by asymmetric hypertrophy of the septum (1 observation) and by abnormal attachement of clefted anterior mitral leaflet to the

Left ventricle to aorta valved conduit for relief of diffuse left ventricular outflow tract obstruction.

Operative relief of congenital tunnel subaortic stenosis by means of local incision or excision, or both, has generally been unsatisfactory. The use of a valve-bearing conduit between the left ventricular apex and thoracic aorta offers a predictable means of bypassing the left ventricular outflow obstruction. The procedure was used in a 17 year old girl with an excellent hemodynamic result. The history of operative management with diverting plantation of valved conduits in this position have not been defined, but use of these prostheses appears advisable in severe subvalvular, valvular and sup…

Cardiac Ventricular Contractile Responses to Chronically Increased Afterload Secondary to Right Ventricular Outflow Obstruction in Patients With Tetralogy of Fallot.

We examined the adaptive mechanism of the pulmonary ventricle (PV) in response to increased afterload secondary to pulmonary stenosis in tetralogy of Fallot (TOF, n = 47) and congenitally corrected transposition of the great arteries (cCTGA, n = 18), where the PV is morphologically different. We also elucidated the effects of such adaptation on systemic ventricular (SV) function. PV contractility, assessed by dp/dtmax, showed significant positive correlations with PV pressure (r = 0.82, p <0.01 for TOF and r = 0.78, p <0.01 for cCTGA) and pulmonary-to-systemic ventricular pressure ratio (r = 0

Aortic atresia or severe left ventricular outflow tract obstruction with ventricular septal defect: results of primary biventricular repair in neonates.

BACKGROUND: Aortic atresia or severe aortic stenosis and left ventricular outflow tract obstruction is a frequent component of complex congenital heart disease. Aortic atresia or severe aortic stenosis and left ventricular outflow tract obstruction with two adequate ventricles is sometimes treated by Norwood palliation followed by late biventricular repair. We reviewed our experience with primary biventricular repair in this group of neonates. METHODS: Retrospective review identified 17 neonates (10 males) with aortic atresia or severe left ventricular outflow tract obstruction with ventricul

Usefulness of color-flow Doppler in diagnosing and in differentiating supracristal ventricular septal defect from right ventricular outflow tract obstruction.

Color-flow Doppler is used for special localization of cardiac flows. The transeptal jet in supracristal ventricular septal defect (VSD) is directed toward the pulmonary valve and is often difficult to distinguish from the right ventricular (RV) outflow tract flow. Pulsed- and continuous-wave Doppler can misread the VSD jet as a stenotic lesion in the RV outflow tract. This study describes the color-flow Doppler characteristics of supracristal VSD and determines if color-flow Doppler can differentiate supracristal VSD jets from RV outflow tract flow. The study group comprised 28 patients rangi

Prevalence of exercise-induced left ventricular outflow tract obstruction in symptomatic patients with non-obstructive hypertrophic cardiomyopathy.

BACKGROUND: Resting left ventricular outflow tract obstruction (LVOTO) occurs in 25% of patients with hypertrophic cardiomyopathy (HCM) and is an important cause of symptoms and disease progression. The prevalence and clinical significance of exercise induced LVOTO in patients with symptomatic non-obstructive HCM is uncertain. METHODS AND RESULTS: 87 symptomatic patients (43.3 (13.7) years, 67.8% males) with HCM and no previously documented LVOTO (defined as a gradient >or=30 mm Hg) underwent echocardiography during upright cardiopulmonary exercise testing: 54 patients (62.1%; 95% CI 51.5 to

Obstruction of the right ventricular outflow tract caused by a tuberculoma in a patient with ventricular septal defect and aneurysm of the membranous septum.

Reported here is an obstruction of the right ventricular outflow tract caused by a tuberculoma in a 15-year-old boy who presented with a ventricular septal defect. The obstruction was discovered at surgery and the tuberculous aetiology was only demonstrated histologically. To the authors' knowledge, this is the first report of a tuberculoma of the heart associated with congenital heart disease.

Diagnostic pitfalls in constrictive pericarditis coexisting with right ventricular outflow tract obstruction and coronary artery anomaly.

Constrictive pericarditis associated with an anomalous left anterior descending artery is a very rare and complex condition. The aim of this report is to highlight the diagnostic challenges and surgical management in this disease. A 40-year-old man with right heart failure, dyspnea, and ascites, underwent comprehensive cardiac evaluation and surgery. The pulmonary valve was dysplastic, causing severe right ventricular outflow tract (RVOT) obstruction. During resection, the left anterior descending artery was inadvertently transected and bypassed with a saphenous vein graft. RVOT reconstruction

Severe pulmonary disease as a driver of dynamic left ventricular outflow tract obstruction in stress cardiomyopathy.

BACKGROUND: Dynamic left ventricular outflow tract obstruction (LVOTO) is a recognized complication of stress cardiomyopathy and can mimic hypertrophic obstructive cardiomyopathy (HOCM) even in the absence of structural hypertrophy. In critically ill patients with severe pulmonary disease, preload, afterload, and contractility can be altered which may exacerbate obstruction. CASE PRESENTATION: A 76-year-old woman with interstitial lung disease and microscopic polyangiitis receiving immunosuppressive therapy presented with progressive dyspnea and shock. Transthoracic echocardiography demonstra

Primary Cardiac Lymphoma Presenting as a Right Ventricular Mass With Outflow Obstruction: A Diagnostic-Therapeutic Challenge.

BACKGROUND: Primary cardiac lymphoma (PCL) is an exceedingly rare malignant non-Hodgkin lymphoma. It represents <2% of primary cardiac tumors often with nonspecific symptoms that may delay diagnosis. CASE SUMMARY: An 83-year-old woman with hypertension and hypothyroidism presented with progressive dyspnea and fatigue. The initial differential diagnosis was concerning for pulmonary embolism or right-heart failure. Echocardiography revealed a 6 × 7-cm right ventricular mass causing right ventricular outflow tract obstruction and right-sided heart failure. Computed tomography confirmed the prese

Anterior Mediastinal Mass and Pericardial Effusion With Right Ventricular Outflow Tract Obstruction Secondary to Dual Compression.

BACKGROUND: Anterior mediastinal masses frequently manifest with pericardial effusion and compressive symptoms, creating management challenges due to the risk of cardiovascular collapse during intervention. CASE SUMMARY: Two patients with primary mediastinal B-cell lymphoma presented with large pericardial effusions and right ventricular outflow tract obstruction. The first patient was successfully managed with tumor-directed therapy alone. The second patient underwent emergency surgery following failed pericardiocentesis, resulting in cardiac arrest requiring mechanical circulatory support.

Half-turned truncal switch for transposition of the great arteries with left ventricular outflow tract obstruction.

OBJECTIVE: The Rastelli operation is accepted as 1 of the standard techniques for complete transposition of the great arteries (TGA) and TGA-type double-outlet right ventricle with left ventricular outflow tract (LVOT) obstruction. The half-turned truncal switch (HTTS) operation has been reported as an alternative to the Rastelli approach. The aim of this study was to compare these 2 operations for children with TGA and LVOT obstruction (TGA/LVOTO). METHODS: Between 2012 and 2024, 11 patients underwent TGA/LVOTO repair. The median age at repair was 12 months and median weight was 8.4 kg. Preo

Left ventricular outflow tract obstruction/hypertrophic cardiomyopathy/takotsubo syndrome: A new hypothesis of takotsubo syndrome pathophysiology.

The pathophysiology of TTS is still elusive. This viewpoint proposes that TTS is an acute coronary syndrome, engendered by an ASNS/catecholamine-induced LVOTO, which results in an enhanced wall stress and afterload-based supply/demand mismatch, culminating in a segmental myocardial ischemic injury state, in susceptible individuals. Such individuals are felt to be particularly women with chronic hypertension, known or latent HCM, or non-HCM segmental myocardial hypertrophy, and certain structural abnormalities involving the LV and the MV apparatus. Recommendations are provided to explore furthe

Haemodynamic effects of acute intravenous landiolol in Takotsubo cardiomyopathy with dynamic left ventricular outflow tract obstruction.

Takotsubo cardiomyopathy (TCM) leads to serious left ventricular outflow tract (LVOT) obstruction with cardiogenic shock in 6%-20% of cases. The onset of LVOT obstruction, coupled with mitral regurgitation resulting from systolic anterior motion of mitral valve leaflets, can lead to haemodynamic instability in addition to severely impaired systolic function. We describe three patients who experienced chest discomfort following emotional stress. These patients displayed pronounced abnormalities on ECGs, insignificant obstructive coronary disease and haemodynamic instability due to LVOT obstruct
