Vestibular evoked myogenic potentials in patients with idiopathic bilateral vestibulopathy. Report of three cases.

Idiopathic bilateral vestibulopathy (IBV) is an acquired bilateral peripheral vestibular disorder of unknown cause. Three patients diagnosed as IBV by neuro-otological examination were reported. They underwent vestibular evoked myogenic potential (VEMP) testing which reflects the functionality of the sacculo-collic pathway. As a result, 2 of the 3 patients showed bilateral absence of VEMPs and one showed unilateral absence. The VEMPs of the 3 patients revealed that IBV affects not only the superior but also the inferior vestibular nerve systems. As previously reported in the cases of vestibula

A quantitative analysis of head-shaking nystagmus of peripheral vestibular origin.

A quantitative analysis of horizontal head-shaking nystagmus (HSN) was made on 48 patients with unilateral peripheral vestibular lesions in conjunction with stimulus intensity. Each patient underwent three head-shaking tests with 10, 30 and 50 horizontal head-excursions at a frequency of approximately 2 Hz, and HSN was recorded on ENG with eyes open in total darkness. i) HSN appeared in a biphasic or monophasic pattern. ii) The maximal slow-phase eye velocity (MSV) of the 1st phase (PI) of biphasic HSN increased significantly in proportion to stimulus intensity, and was significantly greater t

Clinical Assessment of Otolith-Ocular Function: A Practical Guide and Review of the Video-Ocular Counter-Roll.

Otolith organs play a central role in gaze stabilization, spatial orientation, and postural control. Advances in video-oculography have created new opportunities to assess this key aspect of vestibular function directly at the bedside. Using this technology, the video ocular counter-roll (vOCR) test extends vestibular evaluation to the otolith-ocular system by measuring ocular counter-roll (OCR), a torsional vestibulo-ocular response during lateral head tilt. This review provides a practical guide to the clinical application of vOCR, including setup, analysis, and interpretation, and discusses

Physiological assesment of vestibular function and toxicity in humans and animals.

Physiological methods that can be similarly recorded in humans and animals have a major role in sensory toxicology, as they provide a bridge between human sensory perception data and the molecular and cellular data obtained in animal studies. Vestibular toxicity research lags well behind other sensory systems in many aspects, including the availability of methods for functional assessment in animals that could be robustly translated to human significance. Here we review the methods available for the assessment of vestibular function in both humans and laboratory animals, with an emphasis on th

Nonspecific vertigo with normal otoneurological examination. The role of vestibular laboratory tests.

Vestibular laboratory tests are not generally necessary in the diagnosis of patients with a clear description of vertigo accompanied by positive otoneurological examination findings. The purpose of the study was to investigate the role of conventional vestibular laboratory tests in the diagnosis of patients complaining of nonspecific vertigo, despite their having a documented normal otoneurological examination. The results of the standard electronystagmography (ENG) and sinusoidal harmonic acceleration (SHA) tests of 52 patients referred for ambulatory vestibular laboratory tests due to a nons

[Therapy of vestibular vertigo].

The non-surgical treatment of vestibular disorders must be based on current knowledge of vestibular pathophysiology. It is generally accepted that after vestibular lesions a self-repair mechanism exists that allows a more or less complete recovery. In cases of persisting vestibular complaints the physician's duty consists in stimulation of these pre-existing mechanisms. This can be done by physical exercises, as has been recommended since the work of Cawthorne and Cooksey in 1946. This concept is meanwhile supported by modern neurophysiological research. This article describes a short training

Positional test in acute vestibular disorders.

With the aid of th positiogram, a quantitative recording of th positional test, it was possible to study the intensity of nystagmus in three acute vestibular diseases (vestibular neuronitis, Meniere's disease and sudden hearing loss with vestibular involvement). Patients with vestibular neuronitis in the acute stage showed a higher intensity of nystagmus during the positional test than patients passing through the irritation stage of Menière's disease. Especially patients suffering from vestibular neuronitis complained also subjectively about such an intense vertigo that they were seized by a

Vestibulo-Ocular Reflex Function in Adolescents With Sport-Related Concussion: Preliminary Results.

BACKGROUND: Oculomotor impairments, dizziness, and imbalance are common after sports-related concussion (SRC) in adolescents and suggest a relationship between SRC and vestibular system dysfunction. However, it is not clear whether the source of these problems is attributable to the peripheral or central vestibular system. HYPOTHESIS: The video Head Impulse Test (vHIT), which assesses peripheral vestibular function, will show differences in gain between adolescents with and without SRC. Furthermore, there will be an association between vHIT and clinical balance and vestibular/oculomotor testi

Correlating vestibular schwannoma size with vestibular-evoked myogenic potential results.

OBJECTIVES: The maximum size of the vestibular schwannoma (VS) that is compatible with preservation of the function of the vestibular nerve in performing stereotactic radiosurgery remains unclear. This study utilized ocular vestibular-evoked myogenic potential (oVEMP) and cervical VEMP (cVEMP) test results to correlate with the size of VS. DESIGN: Fifty patients with unilateral VS underwent audiometry, and caloric, oVEMP and cVEMP tests. Tumor size from magnetic resonance imaging was measured on the axial plane, and the relationships between tumor size and each test result were analyzed. RES

Vestibular functions in motion sickness susceptible individuals.

There exists no functional guide that can serve as a diagnostic tool for individual susceptibility to motion sickness (MS). We evaluated vestibular system functioning via a caloric test (which assesses functioning of the superior vestibular nerve) and the vestibular-evoked myogenic potentials (VEMP) test (which assesses inferior vestibular nerve functioning) in 20 MS susceptible and 20 nonsusceptible individuals. Susceptibility to MS was determined by self-declaration and with MS susceptibility questionnaire and Hamilton Anxiety Scale (HAS). We found statistically significant differences for s

Evaluation of vestibular and visual oculomotor function.

The visual system interacts synergistically with the vestibular system. A normally functioning vestibulo-ocular reflex is necessary but not sufficient for optimum visual acuity during head motion. Studies of dynamic visual acuity, the acuity achieved during relative motion of visual targets or of the observer, indicate that motion of images on the retina markedly compromises vision. The vestibulo-ocular reflex normally provides a substantial measure of stabilization of the retina during head movements, but purely vestibular compensatory eye movements are not sufficiently precise for optimal vi
