The Uni Angler Knot: A Novel Self-lock Sliding Surgical Knot.

Achieving secure knotting in narrow surgical fields or in high-tension suturing situations remains a technical challenge. Conventional square and surgeon's knots often require an assistant to maintain tension during tying and multiple hitches to ensure security, resulting in bulky knots. To address these limitations, we introduced the uni angler knot (UAK), a novel single-strand, self-locking sliding knot based on the original fishing knot described by angler and fishing writer Norman Duncan as the "universal knot." The UAK is designed to preserve suture tension during tying and to ensure secu

Bibliometric and visualization analysis of non-intubated thoracic anaesthesia [2014-2025].

BACKGROUND: Non-intubated thoracic anaesthesia (NITA) is an emerging anaesthetic approach that integrates spontaneous breathing general anaesthesia with regional blockade. By avoiding tracheal intubation, NITA effectively reduces anaesthesia-related complications and significantly curtails anaesthetic consumption. Evidence indicates that this technique lowers the incidence of postoperative pulmonary complications, facilitates immediate airway clearance, permits early ambulation, improves early postoperative comfort, and shortens hospital stay, thereby accelerating patient recovery. The growing

Early and long-term complaints following video-assisted thoracoscopic surgery: evaluation in 173 patients.

OBJECTIVE: Minimal invasive surgical techniques have gained high acceptance in thoracic surgery during the last 10 years. However, up to now, only scant information exists on chronic postoperative pain and discomfort in patients who underwent video-assisted thoracoscopy. Therefore, a retrospective study was performed with the aid of a self-reported questionnaire. METHODS: Two hundred and thirteen patients (of whom 79 females) with a mean age of 48 (range 15-88) years were operated on for a total of 225 procedures. Thoracoscopy was performed for pneumothorax (n=70), pulmonary nodules (n=44), i

Robotic NICE Procedure Using Handsewn Technique.

INTRODUCTION: In 2018, we described a robotic natural orifice-assisted left-sided colorectal resection with intracorporeal anastomosis and transrectal extraction of the specimen and termed it the natural orifice intracorporeal anastomosis with transrectal extraction procedure. More recently, we have explored the feasibility, safety, and utility of performing total handsewn intracorporeal anastomosis. We present a technical video and initial experience depicting the unique steps to accomplish this procedure with colorectal end-to-end handsewn anastomosis. TECHNIQUE: Twenty natural orifice intr

Video-assisted thoracoscopic ligation of a patent ductus arteriosus in a very low-birth-weight infant using a novel retractor.

Patent ductus arteriosus (PDA) ligation is a potentially lifesaving procedure that is frequently performed in very low-birth-weight (VLBW) infants. Video-assisted thoracoscopic surgery (VATS) PDA ligation has many advantages; however, this approach has not been widely used in these extremely small patients. We present a technique using a novel retractor that allows safe VATS PDA ligation in the VLBW neonate. A 740-g male infant with necrotizing enterocolitis underwent general anesthesia. After placement in the right lateral decubitus position, he underwent a left VATS PDA ligation using a new

Epicardial electrode insertion by means of video-assisted thoracic surgery technique.

BACKGROUND: The number of patients who depend on pacemaker (PM) insertion is steadily increasing. Slovenia has two centers for PM insertion, in which 250 such procedures are performed every year. Thus, with regard to the number of procedures per year per million inhabitants, Slovenia holds a middle position in the list of countries. METHOD: We report a patient with cardiomyopathy, complete A-V block (A-V block III) and permanent transvenous pacing lead replacements. The resulting thrombosis of the superior vena cava compelled us to insert an epicardial permanent pacing lead. Video-assisted th

Surgeon-modified fenestrated endovascular grafts and thoracoscope-assisted fixation for treatment of thoraco-abdominal aortic aneurysms.

BACKGROUND: Total endovascular technique with fenestrated endovascular graft might be hampered for the late dilatation of proximal landing zone, which may cause endografts migration. We describe a successful urgent hybrid procedure for extent III thoracoabdominal aortic aneurysm with aortic intramural hematoma. CASE PRESENTATION: A 55-year-old female with thoracoabdominal aortic aneurysm was considered at high surgical risk and unfit for open repair due to multiple comorbidities. Therefore, a hybrid procedure of surgeon-modified fenestrated endovascular graft combined with thoracoscope-assist

Unanticipated troubles in video-assisted thoracic surgery: a proposal for the classification of troubleshooting.

INTRODUCTION: Most thoracic surgeons encounter atypical cases or unexpected situations that usually lead them to convert minimally invasive surgery to open thoracotomy. But are there other options besides open surgery? The purpose of this study was to suggest a video-assisted thoracic surgery (VATS) classification system and present tips for the application of VATS to atypical cases or unexpected situations. We have categorized VATS procedures for atypical cases or unexpected situations into two groups: the modification of techniques/instruments and the creation of additional access incisions.

Robot-assisted pyeloplasty: How to deal with anatomic variants? A step-by-step video presentation.

INTRODUCTION: Pyeloplasty (open or Robot-assisted) is the gold standard of a symptomatic UPJ stenosis. Sometimes anatomic variants make the procedure challenging. This video describes a step-by-step approach in three settings: a crossing blood vessel and two different presentations of incomplete duplicated system. MATERIALS AND METHODS: Under general anesthesia, patient positioned in lateral decubitus, three trocars are placed. After mobilization of the colon, the Gerota's fascia is opened, and the renal pelvis is dissected off the surrounding structures. Ureter and obstructed pyelum were sub

[Flexible endoscope in thoracic surgery: CITES or cVATS?].

Early pain and persistent parietal disorders remains a major unresolved problem in thoracic surgery. Thoracotomy and the use of multiple ports in most Video Assisted Thoracic Surgery (VATS) procedures are the major cause of this persistent pain. For the last decade, a few publications describing the use of either single incision VATS and cervical thoracic approaches have been reported without significant results in comparison with current used techniques. Intercostals compression during surgery and early after by intercostals chest tube placement, are probably the major cause of postoperative

Mastering the maze: navigating prolonged air leak in thoracic surgery.

Despite technological advancements, prolonged air leak (PAL) remains a significant complication in thoracic surgery. PAL, indicative of an alveolar-pleural fistula (APF), impacts patient recovery, quality of life, and healthcare costs. Risk factors such as smoking and surgical techniques predispose patients to PAL, highlighting the importance of preoperative risk assessment. Segmentectomies, while beneficial for early-stage lung neoplasms, pose challenges due to increased PAL risk. Intraoperative techniques like the submersion test aid in PAL detection and categorisation. Standardising intraop

Relevance of robotic surgery for thymoma: a narrative review.

BACKGROUND AND OBJECTIVE: Thymectomy with median sternotomy is the gold standard for thymoma and myasthenia gravis, although minimally invasive procedures such as robot-assisted surgery have recently become more common. However, the superiority of these approaches has not been established, and they are infrequently recommended for localized lesions. The International Thymic Malignancies Interest Group warned that despite the perceived reduction in length of hospital stay and pain, the benefits of these approaches compared to the open approach have not been fully substantiated and that prospect

Surgery of Simple and Complex Anal Fistulae in Adults: A Review of the Literature for Optimal Surgical Outcomes.

Anal fistulas are common anorectal conditions, and surgery is the primary treatment option. In the last 20 years of literature, there exist a large number of surgical procedures, especially for the treatment of complex anal fistulas, as they present more recurrences and continence problems than simple anal fistulas. To date, there are no guidelines for choosing the best technique. We conducted a recent literature review, mainly the last 20 years, based on the PubMed and Google Scholar medical databases, with the goal of identifying the surgical procedures with the highest success rates, lowest
