Bilateral keratoconus in Crouzon's syndrome.

Crouzon's syndrome is an autosomal dominant disorder characterized by premature craniosynostosis leading to multiple ocular abnormalities and vision loss. Strabismus, optic atrophy, exposure keratitis, hypertelorism, nystagmus, and unexplained visual loss have been reported. Keratoconus has been described only once previously in a patient with other ocular abnormalities. This article reports bilateral keratoconus in a patient with Crouzon's syndrome without other ocular pathology. We emphasize the importance of early recognition and therapy of this treatable cause of decreased vision in Crouzo

Retrospective analysis of refractive errors in children with vision impairment.

PURPOSE: Emmetropization is the reduction in neonatal refractive errors that occurs after birth. Ocular disease may affect this process. We aimed to determine the relative frequency of ocular conditions causing vision impairment in the pediatric population and characterize the refractive anomalies present. We also compared the causes of vision impairment in children today to those between 1974 and 1981. METHODS: Causes of vision impairment and refractive data of 872 children attending a pediatric low-vision clinic from 1985 to 2002 were retrospectively collated. As a result of associated impa

Does order matter? Evaluating the impact of binocular vision test sequence.

PURPOSE: To assess the repeatability of common clinical tests of accommodative and vergence function and evaluate the impact of sequence of testing on outcomes. METHODS: Participants underwent a series of accommodative and vergence function tests under associated and dissociated conditions over 3 study visits. At the first study visit, participants were randomized to undergo associated or dissociated tests first. The testing order was reversed at the second visit (to evaluate sequence of testing), and the third visit replicated the order of testing of the first visit (to evaluate intersession

Acute onset incomitant image disparity modifies saccadic and vergence eye movements.

New-onset impairment of ocular motility will cause incomitant strabismus, i.e., a gaze-dependent ocular misalignment. This ocular misalignment will cause retinal disparity, that is, a deviation of the spatial position of an image on the retina of both eyes, which is a trigger for a vergence eye movement that results in ocular realignment. If the vergence movement fails, the eyes remain misaligned, resulting in double vision. Adaptive processes to such incomitant vergence stimuli are poorly understood. In this study, we have investigated the physiological oculomotor response of saccadic and ver

Evaluation of vestibular and visual oculomotor function.

The visual system interacts synergistically with the vestibular system. A normally functioning vestibulo-ocular reflex is necessary but not sufficient for optimum visual acuity during head motion. Studies of dynamic visual acuity, the acuity achieved during relative motion of visual targets or of the observer, indicate that motion of images on the retina markedly compromises vision. The vestibulo-ocular reflex normally provides a substantial measure of stabilization of the retina during head movements, but purely vestibular compensatory eye movements are not sufficiently precise for optimal vi

Acute Central Vision Loss in an IV Drug User.

This report describes the case of a 21-year-old heroin user who presented with a 6-day history of decreased vision in her right eye, preceded by 1 week of headache and tender scalp nodules, neck stiffness, and photophobia. A broad infectious workup for acute vision loss was completed, and she was ultimately presumed to have acquired toxoplasmic chorioretinitis (ocular toxoplasmosis). We review the initial workup for chorioretinitis, and the epidemiology, diagnosis, and treatment of ocular toxoplasmosis. Intravenous drug users may be at increased risk of acquired ocular toxoplasmosis.

Ophthalmic Emergencies for the Clinician.

Primary care physicians are at the forefront of patient care and often are the first clinicians to triage and diagnose any eye-related complaints. They must be able to quickly identify vision-threatening pathologies, as delay in treatment of an ocular emergency can result in permanent vision loss. This concise review describes the definition, presentation, examination, and management of various ophthalmic emergencies including blunt ocular trauma, chemical ocular injury, orbital cellulitis, endophthalmitis, acute angle closure glaucoma, optic neuritis, giant cell arteritis, central retinal art

Tuberculous retinal vasculitis.

A patient who was referred to the eye department for routine ocular assessment prior to commencement of antituberculous therapy was found to have periphlebitis in both eyes despite being visually asymptomatic. Fluorescein angiography confirms the presence of vasculitis without any retinal oedema or areas of non-perfusion, which may sometimes accompany the condition. Within 2 months of systemic treatment, the ocular signs regressed without any permanent effect on vision. This case highlights a rare ocular complication associated with systemic tuberculosis which fortunately did not result in los

PEST-containing nuclear protein in eye health and disease: A review.

The human visual system plays a crucial role in how we perceive the world. However, it is susceptible to ocular conditions that can result in visual impairments. Globally, over 2.2 billion people suffer from vision impairments, including macular degeneration, refractive errors, cataracts, and glaucoma. In the field of iridology and ocular biology, essential proteins governing ocular homeostasis are frequently mutated or dysregulated. Clear vision is essential for people, and mutations related to these proteins can significantly impact the prevalence and progression of eye disorders. This revie

Diagnostic Validity of Accommodative Tests for Screening Suspected Accommodative Excess in Adolescents.

PURPOSE: Accommodative excess (AE) is characterised by an excessive accommodative response (lead) and impaired ability to relax accommodation, typically accompanied by asthenopic symptoms and reduced visual comfort during near tasks. Although AE is frequently observed in clinical practice, its definition and diagnostic criteria remain inconsistent, and the diagnostic performance of commonly used clinical tests is not well established. This study examined the diagnostic validity of tests evaluating the accommodative response and relaxation to detect suspected AE in a non-clinical adolescent pop

The symptomatic patient with normal phorias at distance and near: what tests detect a binocular vision problem?

PURPOSE: Patients often manifest symptoms that appear to be related to binocular distress. Many of these patients have normal heterophoria at distance and near, making the etiology of such symptoms perplexing. We performed a visual examination of patients having normal heterophoria at distance and near in order to investigate which visual analysis tests differentiate symptomatic from asymptomatic patients. METHODS: Eighty subjects (30 symptomatic, 50 asymptomatic) with ages between 18 to 35 years of either sex and any race were pooled based on vision-symptom level determined by a 9-question s

Keeping the world in focus: how might this be achieved?

To see clearly, the eye needs to have a sharply focused image on its retina. For distance vision, the eye should ideally develop toward emmetropia, and for near vision, the accommodation system should produce an appropriate increase in ocular power. The characteristics of the emmetropization and accommodation mechanisms are compared. Following Howland (paper presented at the 13th International Myopia Conference, July 26-29, 2010, Tubingen, Germany), it is suggested that emmetropization may be guided by imagery in the peripheral retina, perhaps making use of oblique astigmatism; however, such a

The effect of incipient presbyopia on the correspondence between accommodation and vergence.

PURPOSE: To investigate the accommodation-convergence relationship during the incipient phase of presbyopia. The study aimed to differentiate between the current theories of presbyopia and to explore the mechanisms by which the oculomotor system compensates for the change in the accommodation-convergence relationship contingent on a declining amplitude of accommodation. METHODS: Using a Canon R-1 open-view autorefractor and a haploscope device, measurements were made of the stimulus and response accommodative convergence/accommodation ratios and the convergence accommodation/convergence ratio

Clinical features and treatment of ocular toxoplasmosis.

Ocular toxoplasmosis is a disease caused by the infection with Toxoplasma gondii through congenital or acquired routes. Once the parasite reaches the retina, it proliferates within host cells followed by rupture of the host cells and invasion into neighboring cells to make primary lesions. Sometimes the restricted parasite by the host immunity in the first scar is activated to infect another lesion nearby the scar. Blurred vision is the main complaint of ocular toxoplasmic patients and can be diagnosed by detection of antibodies or parasite DNA. Ocular toxoplasmosis needs therapy with several
