Pulmonary function pre- and post-total laryngectomy was assessed in 16 patients without overt inspiratory stridor preoperatively. It could be shown that total laryngectomy is associated with a significant decrease in vital capacity and in an increase in inspiratory flow-volume values 9 days postoperatively. The inspiratory values tended to increase slightly over the next half year. The measured expiratory flow-volume values were preoperatively already significantly lower than the predicted values, and no significant changes could be observed immediately after the operation nor after 6 months. …

A series of 19 patients operated on because of pectus excavatum has been analyzed to assess results of Zuk modification of the Brown method. Vital lung capacity, funnel volume, physical fitness, and cosmetic result have been assessed. An asymmetry and developmental disturbances within insertions of pectoralis major, rectus and oblique abdominal muscles were found as well as developmental disturbances of costal cartilage. In 95% of cases a positive patient opinion along with satisfactory functional and cosmetic result have been achieved.

Our objective was to study the changes in respiratory function of patients with osteoporotic vertebral compression fractures (OVCFs) after vertebroplasty and kyphoplasty. We studied the pulmonary function and chest x-ray films of 20 nonsmokers with idiopathic prolapse of the mitral valve. Forty-five percent of these patients complained of dyspnea. Of the 16 chest films reviewed, 44 percent had radiographic evidence of pectus excavatum and 25 percent thoracic scoliosis. The most common pulmonary function abnormalities were increased residual volume (63 percent) and a reduced steady state diffusing capacity (50 percent of patients). Six subjects with farmer's lung underwent double inhalation challenge tests, each lasting 60 min, using natural antigen exposure. Subjects underwent the tests first without and then with protection by a particle-filtering half mask. Our purpose was to determine whether and to what extent the use of such masks reduced or prevented symptoms in affected patients. Clinical assessment included general and pulmonary symptoms, HR, temperature, WBC count, R, ITGV, VC, TLC, PO2, DCO, and chest x-ray film. The unprotected challenge provoked late responses characteristic of extrinsic allergic alveolitis.

The results of five pulmonary function tests on 52 patients with uncompensated hepatic cirrhosis were presented. Compared with matched healthy adults, the patients had decreased values of VC, TLC and DLCO, and their RV/TLC increased (P < 0.05-0.001). Possible pathogenic factors were investigated. We describe the cases of three women with pulmonary involvement in the eosinophilia-myalgia syndrome. The illness was characterized by elevated peripheral blood eosinophil counts, myalgias, fatigue, and dyspnea. Two of three patients had bilateral infiltrates on chest roentgenograms. All three had markedly decreased carbon monoxide diffusing capacities and pulmonary hypertension. High-dose prednisone therapy provided only partial resolution of the pulmonary symptoms.
