The hidden battlefield: platelet function in the wake of severe trauma.

Trauma contributes significantly to the global burden of suffering and death, and interrupts the complex balance between hemostasis and the immune system; platelets are key in integrating hemostatic and immune functions. This review will provide an overview of how different types of traumatic injury including traumatic brain injury, hemorrhagic shock, and burns impact platelet function through both separate and interconnected mechanistic pathways. Moreover, this review will examine in detail the multiple manifestations of trauma-induced platelet dysfunction, including but not limited to impair

Nocturia: when the heart wakes you up at night.

BACKGROUND: Nocturia is a common and burdensome symptom that may reflect underlying cardiovascular dysfunction. While previous studies have linked nocturia to heart failure and fluid overload, direct hemodynamic evidence supporting this association remains limited. OBJECTIVE: To evaluate the relationship between left- and right-sided filling pressures, particularly pulmonary artery wedge pressure (PAWP) and right atrial (RA) pressure, and nocturia severity in a cohort of patients undergoing right heart catheterization. METHODS: We conducted a prospective observational study of 74 adult patie

Gradients in signal complexity of sleep-wake intracerebral EEG.

Spatial variation in the morphology of the electroencephalogram (EEG) over the head is classically described. Ultimately, location-dependent variation in EEG must arise from the cytoarchitectural and network structure of the portion of cortex sensed. In previous work, we demonstrated that over the lateral frontal lobe, sample entropy (SE) of intracerebral EEG (iEEG) over a subdural recording contact was predictive of that contact's connectivity to other contacts. In this work, we used a publicly available repository (the Montreal Neurological Institute Atlas; MNIA) of whole-brain normative iEE

Wake-up intracerebral hemorrhage: hematoma expansion and outcomes.

INTRODUCTION: While understudied, wake-up intracerebral hemorrhage (WU-ICH) is not uncommon (8.8-20.3% of ICH patients). Since the risk of hematoma expansion (HE) decreases as time passes, an uncertain onset time in WU-ICH may influence the risk of in-hospital HE and the potential effects of HE-preventive treatments. We aimed to evaluate HE and outcomes in WU-ICH compared to known-onset ICH. METHODS: We included ICH patients admitted to the Karolinska University Hospital from 2016 to 2022, comparing WU-ICH vs. known-onset ICH regarding baseline characteristics, HE, and outcomes. RESULTS: Of

The burden of acute pain in the U.S. in the wake of the opioid crisis.

The prevalence of acute pain has grown substantially over the past two decades, due primarily to more surgeries, an aging population, and the rapid growth in the prevalence of metabolic disease. Although opioids are often the only effective treatment for many types of acute pain, especially severe acute pain, their use, even over a short period of time, comes with substantial risks of dependence, misuse, and diversion. Moreover, a large fraction of the patients currently suffering from opioid use disorder and those dying from opioid overdoses had their first exposure as pain patients. Converse

Automated Classification of Sleep-Wake States and Seizures in Mice.

Sleep-wake states bi-directionally interact with epilepsy and seizures, but the mechanisms are unknown. A barrier to comprehensive characterization and the study of mechanisms has been the difficulty of annotating large chronic recording datasets. To overcome this barrier, we sought to develop an automated method of classifying sleep-wake states, seizures, and the post-ictal state in mice ranging from controls to mice with severe epilepsy with accompanying background EEG abnormalities. We utilized a large dataset of recordings, including EMG, EEG, and hippocampal local field potentials, from c

A role of prefrontal cortico-hypothalamic projections in wake promotion.

Ventromedial prefrontal cortex (vmPFC) processes many critical brain functions, such as decision-making, value-coding, thinking, and emotional arousal/recognition, but whether vmPFC plays a role in sleep-wake promotion circuitry is still unclear. Here, we find that photoactivation of dorsomedial hypothalamus (DMH)-projecting vmPFC neurons, their terminals, or their postsynaptic DMH neurons rapidly switches non-rapid eye movement (NREM) but not rapid eye movement sleep to wakefulness, which is blocked by photoinhibition of DMH outputs in lateral hypothalamus (LHs). Chemoactivation of DMH glutam…

Complexity of cortical wave patterns of the wake mouse cortex.

Rich spatiotemporal dynamics of cortical activity, including complex and diverse wave patterns, have been identified during unconscious and conscious brain states. Yet, how these activity patterns emerge across different levels of wakefulness remain unclear. Here we study the evolution of wave patterns utilizing data from high spatiotemporal resolution optical voltage imaging of mice transitioning from barbiturate-induced anesthesia to wakefulness (N = 5) and awake mice (N = 4). We find that, as the brain transitions into wakefulness, there is a reduction in hemisphere-scale voltage waves, and…

Molecular mechanism of the wake-promoting agent TAK-925.

The OX2 orexin receptor (OX2R) is a highly expressed G protein-coupled receptor (GPCR) in the brain that regulates wakefulness and circadian rhythms in humans. Antagonism of OX2R is a proven therapeutic strategy for insomnia drugs, and agonism of OX2R is a potentially powerful approach for narcolepsy type 1, which is characterized by the death of orexinergic neurons. Until recently, agonism of OX2R had been considered 'undruggable.' We harness cryo-electron microscopy of OX2R-G protein complexes to determine how the first clinically tested OX2R agonist TAK-925 can activate OX2R in a highly sel…

Identification of ultrastructural signatures of sleep and wake in the fly brain.

The cellular consequences of sleep loss are poorly characterized. In the pyramidal neurons of mouse frontal cortex, we found that mitochondria and secondary lysosomes occupy a larger proportion of the cytoplasm after chronic sleep restriction compared to sleep, consistent with increased cellular burden due to extended wake. For each morphological parameter, the within-animal variance was high, suggesting that the effects of sleep and sleep loss vary greatly among neurons. However, the analysis was based on 4-5 mice/group and a single section/cell. Here, we applied serial block-face scanning el

Validity of In-Home Teleneuropsychological Testing in the Wake of COVID-19.

OBJECTIVE: Teleneuropsychology (TNP) has been shown to be a valid assessment method compared with in-person neuropsychological evaluations. Interest in delivering TNP directly to patients' homes has arisen in response to the coronavirus disease 2019 (COVID-19) pandemic. However, prior research has typically involved patients tested in clinical settings, and the validity of in-home TNP testing has not yet been established. The present study aims to explore the validity and clinical utility of in-home TNP testing in a mixed clinical sample in the wake of COVID-19. METHODS: Test profiles for 111

Wake-up stroke is not associated with obstructive sleep apnea.

OBJECTIVE/BACKGROUND: Obstructive sleep apnea is a risk factor for stroke. This study sought to assess the relationship between obstructive sleep apnea (OSA) and wake-up strokes (WUS), that is, stroke symptoms that are first noted upon awakening from sleep. PATIENTS/METHODS: In this analysis, 837 Brain Attack Surveillance in Corpus Christi (BASIC) project participants completed an interview to ascertain stroke onset during sleep (WUS) versus wakefulness (non-wake-up stroke, non-WUS). A subset of 316 participants underwent a home sleep apnea test (HSAT) shortly after ischemic stroke to assess …

Sleep-wake disturbances after traumatic brain injury.

Sleep-wake disturbances are extremely common after a traumatic brain injury (TBI). The most common disturbances are insomnia (difficulties falling or staying asleep), increased sleep need, and excessive daytime sleepiness that can be due to the TBI or other sleep disorders associated with TBI, such as sleep-related breathing disorder or post-traumatic hypersomnia. Sleep-wake disturbances can have a major effect on functional outcomes and on the recovery process after TBI. These negative effects can exacerbate other common sequelae of TBI-such as fatigue, pain, cognitive impairments, and psycho

In the wake of Angelina - managing a family history of breast cancer.

BACKGROUND: Accurate risk assessment by general practitioners (GPs) of breast and ovarian cancer for unaffected women with a family history is important to ensure appropriate referral. Family cancer clinics can provide significant benefits for those at high risk, but genetic testing is unlikely to benefit those who are unaffected, or have little or no family history. The overwhelming increase in referrals following celebrity Angelina Jolie's decision to have a risk-reducing mastectomy because she carries a gene mutation has put pressure on services. OBJECTIVE: To provide information for GPs a

Automatic interpretation and writing report of the adult waking electroencephalogram.

Automatic interpretation of the EEG has so far been faced with significant difficulties because of a large amount of spatial as well as temporal information contained in the EEG, continuous fluctuation of the background activity depending on changes in the subject's vigilance and attention level, the occurrence of paroxysmal activities such as spikes and spike-and-slow-waves, contamination of the EEG with a variety of artefacts and the use of different recording electrodes and montages. Therefore, previous attempts of automatic EEG interpretation have been focussed only on a specific EEG featu
