[Exposure testing with powdered gloves in 60 health care workers with latex allergy].

BACKGROUND AND OBJECTIVE: Type 1 sensitization to natural rubber latex occurs in up to 22% of health care workers. Most sensitizations are due to the use of powdered latex gloves. Work place-associated exposure tests were performed to ascertain how frequently persons who developed breathing difficulties, rhinitis/conjunctivitis or obstructive respiratory tract illness when using powdered latex gloves are allergic to latex. PATIENTS AND METHODS: 60 persons (48 women and 12 men; mean age 29 +/- 7 years) with type 1 sensitization to natural rubber latex were studied. The past history was obtaine

Respiratory countermaneuvers in autonomic failure.

BACKGROUND: Selective increase of inspiratory impedance augments blood pressure in healthy subjects through activation of the respiratory pump. We studied the efficacy of respiratory maneuvers to reduce orthostatic hypotension in autonomic failure. METHODS: Mean arterial pressure (MAP) after standing up was recorded in 10 patients in five conditions: normal standing, leg muscle tensing, inspiratory pursed lips breathing, inspiratory sniffing, and a device causing inspiratory obstruction. RESULTS: The maneuvers caused significant differences in standing MAP. Inspiratory obstruction and leg mu

Feasibility of Rib Kinematics and Intercostal-Space Biomechanical Characterization by Ultrasound in Adolescent Idiopathic Scoliosis.

The aim of this work was to determine the feasibility of combined ultrasonography and elastography measurement to characterize the mechanical properties of the intercostal space during breathing. Eighteen asymptomatic participants (ages 13 ± 2 y) and six participants with adolescent idiopathic scoliosis (AIS) were included (Cobb angle 60° ± 12°). Ultrasonographic and elastographic clips were acquired of T8-T9 ribs and the intercostal space. The two adjacent ribs were tracked to infer the breathing cycle. Shear-wave speed (SWS) was measured in the intercostal space at different stages of the br

Deep breathing couples CSF and venous flow dynamics.

Venous system pathologies have increasingly been linked to clinically relevant disorders of CSF circulation whereas the exact coupling mechanisms still remain unknown. In this work, flow dynamics of both systems were studied using real-time phase-contrast flow MRI in 16 healthy subjects during normal and forced breathing. Flow evaluations in the aqueduct, at cervical level C3 and lumbar level L3 for both the CSF and venous fluid systems reveal temporal modulations by forced respiration. During normal breathing cardiac-related flow modulations prevailed, while forced breathing shifted the domin

Awareness of issues related to sleep disordered breathing amongst practicing physicians.

BACKGROUND: Most of the sleep related breathing disorders currently in India are both under diagnosed and under treated. The exact prevalence of sleep disordered breathing (SDB) in our country is not clearly known and this is partly due to lack of awareness amongst physicians and lack of formal epidemiological data from Indian subcontinent. Several international agencies have emphasised the urgent need for medical training at all levels in sleep medicine, so as to include an exposure to the broader aspects of sleep medicine. OBJECTIVE: To assess the knowledge of practicing chest physicians on

Moisturizing and mechanical characteristics of a new counter-flow type heated humidifier.

BACKGROUND: During mechanical ventilation effective conditioning of inspired air is important. In this respect, conventional humidifiers do not perform optimally. By design, a counter-flow-type humidifier should improve humidification and heating, but may increase resistance. METHODS: We investigated mechanical impedance and work of breathing (using pressure-flow characteristics and additional pressure-time product) of a new counter-flow-type humidifier, a conventional heated humidifier, and a passive heat and moisture exchanger (HME) in physical models of the respiratory system. We investiga

Asthma-related work disability in Sweden. The impact of workplace exposures.

Work disability due to respiratory disease, especially asthma, is common and costly among working age adults. The goal of this analysis was to characterize the risk factors for such disability. We analyzed data from the Swedish part of the European Community Respiratory Health Survey (ECRHS), a random population-based sample of adults age 20 to 44, enriched with symptomatic subjects at increased likelihood of having asthma. We analyzed structured interview data available for 2,065 subjects and further analyzed methacholine challenge and skin prick test data for 1,562 of these. We defined respi

A protocol for validation of novel artificial intelligence-based framework for dyspnoea investigation with cardiopulmonary exercise testing.

BACKGROUND: Exertional dyspnoea largely represents the sensory translation of an ever-growing dynamic mismatch between ventilatory demand and capacity as exercise intensifies. This fundamental tenet, however, has not been formally incorporated into data display and clinical interpretation of incremental cardiopulmonary exercise testing (CPET). The objectives of the present study were to validate a novel framework (Dynamic Assessment of Dyspnoea and Ventilation on Exercise (DyVe-X)) to quantify the severity of exertional dyspnoea while exposing its mechanical-ventilatory underpinnings across pr

Developmental patterns of laryngeal and respiratory function for speech production.

Estimates of subglottal air pressure, laryngeal airflow, and laryngeal airway resistance from syllable repetitions of children and adults were used in describing developmental changes in these variables and in hypothesizing corresponding changes in respiratory function. A trend was found for pressure and resistance to decrease with increases in flow from preschool age through adulthood. These patterns could be explained by the smaller size of laryngeal airway structures and increased expiratory muscle forces during speech in the younger subjects. When the subglottal air pressures were combined

Emergency presentation of iatrogenic airway stenosis following intubation in a patient with COVID-19 and its management.

A 59-year-old man presented to the emergency department with recent onset biphasic stridor, dyspnoea and increased work of breathing on the background of prolonged intubation for the novel COVID-19 2 months previously. Flexible laryngoscopy revealed bilateral vocal fold immobility with a soft tissue mass in the interarytenoid region. The patient's symptoms improved with oxygen therapy, nebulised epinephrine (5 mL; 1:10 000) and intravenous dexamethasone (3.3 mg). The following morning, the patient was taken to theatre, underwent suspension microlaryngoscopy and found to have bilateral fixation

Therapeutic benefits of helium-oxygen delivery to infants via nasal cannula.

OBJECTIVE: The benefits of helium-oxygen (heliox) administration for pediatric upper and lower respiratory disorders have been well described. However, while most studies advocate delivery via a sealed or semisealed facemask system, such systems may not be tolerated in the young child. This report describes the successful and efficacious delivery of heliox via nasal cannula to 5 infants. METHODS: A mixture of 80% helium/20% oxygen was blended with 100% oxygen from a wall source and delivered via nasal cannula to 5 spontaneously breathing infants with respiratory distress at flow rates of 2 to

Thoracic weighting of restrained subjects during exhaustion recovery causes loss of lung reserve volume in a model of police arrest.

Restraint asphyxia has been proposed as a mechanism for some arrest-related deaths that occur during or shortly after a suspect is taken into custody. Our analysis of the literature found that prone positioning, weight applied to the back, recovery after simulated pursuit, and restraint position have led to restrictive, but non life-threatening respiratory changes when tested in subsets. However, the combined effects of all four parameters have not been tested together in a single study. We hypothesized that a complete protocol with high-sensitivity instrumentation could improve our understand

[Sleep and breathing at altitude of 3800 m--the acclimatization effect].

UNLABELLED: Periodic breathing (PB) is a very common phenomenon occurring during sleep at high altitude. It consists of repetitive apneas or hypopneas of central origin and clusters of hyperpneic breaths. The aim of this study was to analyze sleep structure and periodic breathing in shift workers of a gold mine situated in Tien-Schan Mountains at 3800 m. In 12 subjects aged 36.1 +/- 9.3 yrs, polysomnography (PSG) was performed twice, on the night after the first working shift or during the day following first night shift, and the second PSG at the end of the first week of work. After one week,

Ventilatory response kinetics and breathing pattern during exercise at different chemoreceptive drives.

For studying the effects of different chemoreceptive drives on the ventilatory responses to muscular work, experiments were carried out with 16 healthy men. They accomplished a 5-min exercise while breathing with different gas mixtures or after hyperventilation. As was shown, the rapid component of the exercise hyperpnea increased with intensifying the hypercapnic and/or hypoxic stimuli and was reduced or abolished with attenuation of these stimuli. At this, hypoxic drive exerted primary influence on the response kinetics. In the steady-state exercise effects of the chemoreceptive stimuli were

Compartmental chest wall volume changes during volitional normocapnic hyperpnoea.

During increased ventilation, inspiratory rib cage muscles have been suggested to take over part of diaphragmatic work after the diaphragm fatigues. We investigated the extent to which this proposed change in muscle recruitment is associated with changes in the relative contribution of chest wall compartments to tidal volume (V(T)). Thirteen healthy subjects performed 1 h of fatiguing normocapnic hyperpnoea. Chest wall volumes were assessed by optoelectronic plethysmography. While breathing frequency increased (43±3 to 56±5 breaths min(-1), p=0.006) and V(T) decreased during normocapnic hyperp

Density-dependent airflow and ventilatory control during exercise.

The influence of respired gas density on ventilatory control during cycle-ergometer exercise was investigated in six healthy subjects. They underwent constant-load exercise for 10 min both at 50% and 90% of the anaerobic threshold, inhaling air for the first 5 min followed abruptly by 80% helium-20% oxygen (He-O2) for the remaining 5 min (and vice versa). The He-O2 breathing elicited no discernible effect on ventilation (VI) or mean alveolar PCO2 (PACO2) at rest or at the lower work rate. However, at the higher work rate, He-O2 breathing resulted in a clear and sustained hyperventilation in al

Bi-level positive airway pressure ventilation maintains adequate ventilation in post-polio patients with respiratory failure.

BACKGROUND: Patients suffering from post-polio syndrome still contribute significantly to the number of patients with chronic respiratory failure requiring home mechanical ventilation (HMV). Many of these patients are treated either with invasive (tracheostomy) or non-invasive (nasal mask) controlled mechanical ventilation i.e. volume-controlled ventilation (VCV). In this group of patients, we have previously shown that bi-level pressure support ventilation (bi-level PSV) decreases the oxygen cost of breathing. The aim of this study was to compare the effect of bi-level PSV, with special regar

Comparison of breath and in-mouth collection for the measurement of oral malodorous compounds by gas chromatography using sulfur chemiluminescence detection.

Volatile sulfur compounds (VSCs), specifically hydrogen sulfide, methyl mercaptan and dimethyl sulfide, are generally considered to be the primary volatiles responsible for 'morning' malodors in breath. To date, the 'gold standard' for detecting VSC concentrations in breath is the use of gas chromatography coupled with sulfur chemiluminescence detection. Breath gas is often collected in a polypropylene syringe and then aliquots are injected into a gas chromatograph for analysis. The objective of this work was to compare the Twister™ bar in-mouth extraction methodology for measurement of VSCs w
