Imaging features of central nervous system fungal infections.

Fungal infections of the central nervous system (CNS) are rare in the general population and are invariably secondary to primary focus elsewhere, usually in the lung or intestine. Except for people with longstanding diabetes, they are most frequently encountered in immunocompromised patients such as those with acquired immunodeficiency syndrome or after organ transplantation. Due to the lack of inflammatory response, neuroradiological findings are often nonspecific and are frequently mistaken for tuberculous meningitis, pyogenic abscess or brain tumor. Intracranial fungal infections are being

Prevalence of "dural tail sign" in patients with different intracranial pathologies.

The purpose of this study is to clarify the prevalence, spectrum of associated tumors and the diagnostic value of the linear meningeal thick … e "dural tail sign" (18 meningiomas, 2 pituitary adenomas, 1 primary cerebral lymphoma and 1 fungal brain abscess). Fifty-eight percent of the patients with biopsy-proven meningioma were observed to show "dural tail sign". In conclusion, we found the "dural tail sign" to have a sensitivity of 58.6% and specificity of 94.02% in diagnosis of meningioma.

[A case of solitary neurocysticercosis of unknown transmission route].

We report a case of solitary neurocysticercosis of unknown transmission route. A 26-year-old male was taken to our hospital with a history of general convulsions. On admission, physical and neurological findings were normal. On the basis of neuroimaging (computed tomography scan and magnetic resonance imaging), initial diagnosis was brain abscess and the patient was treated with antibiotics. Two months later, the patient, at times, presented a loss of consciousness. The follow-up MRI revealed that the enhanced lesion became enlarged and perifocal edema became evident, so the patient was surgic…

[Benefits of 0.5T MR diffusion-weighted images for super-acute-phase cerebral infarction].

Though diffusion-weighted images (DWI) have been increasingly used to detect super-acute-phase cerebral infarction in recent years, they have primarily been obtained through the use of high magnetic machines of more than 1.5T. In this study, we discussed the usefulness of DWI obtained using 0.5T MRI in comparison with CT, MRI (FLAIR and T2 weighted image) and SPECT (99mTc-HMPAO). DWI were able to detect ischemic lesions earlier than FLAIR or T2-weighted images. Scanning time was short at four seconds for eight slices, and the quality of image was sufficient for clinical usage. The most availab

AIDS-related visceral aspergillosis: an underdiagnosed disease during life?

Five out of nine consecutive patients with HIV-related visceral aspergillosis observed by us since 1984 were diagnosed only at necropsy examination. The histopathological features of these five patients [two with isolated pneumonia, one with central nervous system (CNS) involvement, one with brain abscess and respiratory disease and one with pulmonary, pleural and kidney infection] have been evaluated according to epidemiological, clinical and radiological features. On the basis of our experience, life-threatening aspergillosis, which is often misdiagnosed or missed in the setting of HIV infec…

Optic neuritis and orbital lesions: lipid-suppressed chemical shift MR imaging.

A derivative of the Dixon method, chopper fat suppression, was used in the magnetic resonance imaging evaluation of 40 patients: 37 with suspected optic nerve lesions, predominantly optic neuritis, and three with orbital lesions. In patients with optic neuritis, the technique was sensitive, allowing detection of 29 of 34 visual-evoked-response-confirmed lesions on T2-weighted lipid-suppressed images. There were no false-positive studies. Sensitivity for detection of optic neuritis was 89%; specificity, 100%; and accuracy, 86%. The technique was also useful for depicting inflammatory changes in

Neurological manifestations of infective endocarditis. Review of clinical and therapeutic challenges.

The nervous system is frequently involved in patients with infective endocarditis. When a careful review of presenting complaints is undertaken, neurological symptoms have been found in as high as 29% of patients. Because these manifestations may be so protean in nature, for example, stroke or transient ischaemic attack (the most common), toxic encephalopathy, meningitis, brain abscess, visual loss, seizures, headache, backache, or acute mononeuropathy, the neurologist needs to consider infective endocarditis as a possible diagnosis in many patients. During the past two decades, infective endo…

Operative intracranial infection following craniotomy.

OBJECT: Postoperative infection after cranial surgery is a serious complication that requires immediate recognition and treatment. In certain cases such as postoperative meningitis, the patient can be treated with antibiotics only. In cases that involve a bone flap infection, subdural empyema, or cerebral abscess, however, reoperation is often needed. There has been significant disagreement regarding the incidence of postoperative central nervous system (CNS) infections following cranial surgery. In this paper the authors' goal was to perform a retrospective review of the incidence of CNS infe

Late diagnosis and removal of a large wooden foreign body in the cranio-orbital region.

The rare case of a large wooden foreign body impaled in the cranio-orbital region, and its late diagnosis and successful removal, is presented. A 26-year-old man was admitted to a regional hospital after suffering a severe penetrating craniocerebral injury from a motorcycle accident. Two months after the accident, computed tomographic examination at Burdenko Neurosurgical Institute revealed a large foreign body located in the cranio-orbital region and penetrating the right temporal lobe, with surrounding abscess development. Radiological examination, including three-dimensional computed tomogr

Neurologic complications in a group of 86 bacterial endocarditis.

Criteria defined by von Reyn were applied to 86 cases of bacterial endocarditis. Neurologic complications (NC) were categorized according to Pruitt definitions. Neurologic accidents were observed in 48 cases. They were the first clinical manifestation in 20 patients. Neurologic events were of poor prognosis in BE, mortality increasing from 26% in patients without NC to 83% in patients with NC (P less than 0.01). Two factors affect the incidence of NC: first, the location of endocarditis with 76% of NC in mitral valve endocarditis compared with 37% in other cases (P less than 0.005); and second

Acquired immunodeficiency with intestinal cryptosporidiosis: possible transmission by Haitian whole blood.

A 31-year-old Frenchman had an acquired immunodeficiency syndrome (AIDS) with profound depression of cellular immunity and relative sparing of humoral immunity. The clinical picture included intractable secretory diarrhoea, vomiting, abdominal pain, and weight loss. Gastrointestinal cryptosporidiosis was present and a perfusion technique showed profuse secretion of fluid in the proximal small bowel. The patient also had recurrent Salmonella typhimurium septicaemia, cytomegalovirus infection, and cerebral toxoplasmosis and he died within 13 months. This patient did not belong to any of the grou

[Role of chloramphenicol in current clinical practice].

INTRODUCTION: Chloramphenicol is an antibiotic with a broad spectrum of action and excellent tissue penetration. It had been widely used in … hospitals, with a frequency of up to 10 patients treated per year. The most common indications are brain abscesses, purulent meningitis, intra-abdominal, pelvic and lung abscesses, and polymicrobial infections with anaerobes. Chloramphenicol is almost always administered as an alternative antibiotic because of polyvalent allergy, bacterial resistance, and fail …

Nocardiosis in Srinagarind Hospital, Thailand: review of 70 cases from 1996-2001.

OBJECTIVE: Nocardiosis is a common opportunistic infection found in both immunocompromised and immunocompetent patients. The clinical manife … SMX was commonly used. In-hospital mortality was 20%. Most of the cases who died had dissemination, brain abscesses or infection with TMP-SMX-resistant strains. The long-term prognosis was good, with a treatment success rate of 93.75%. CONCLUSION: Nocardiosis is a common opportunistic infection in many immunocompromised conditions. It can present with various …
