Single versus combination intravenous antibiotic therapy for people with cystic fibrosis.

BACKGROUND: Choice of antibiotic, and the use of single or combined therapy are controversial areas in the treatment of respiratory infection in cystic fibrosis (CF). Advantages of combination therapy include wider range of modes of action, possible synergy and reduction of resistant organisms; advantages of monotherapy include lower cost, ease of administration and reduction of drug-related toxicity. Current evidence does not provide a clear answer and the use of intravenous antibiotic therapy in CF requires further evaluation. OBJECTIVES: To assess the effectiveness of single compared to co

Melatonin versus midazolam in the premedication of anxious children attending for elective surgery under general anaesthesia: the MAGIC non-inferiority RCT.

BACKGROUND: Anxiety in children prior to general anaesthesia is common, with up to half displaying distress. Anxiety and distress may lead to unsuccessful anaesthesia, together with greater postoperative pain, agitation and behavioural changes after surgery including sleep disturbances. Midazolam is the current standard premedication; however, it has adverse effects such as the potential for respiratory suppression and unpredictable effects which may result in agitation rather than anxiolysis. Melatonin is an alternative preoperative anxiolytic; however, previous trials have delivered conflict

Participation and yield of colonoscopy versus non-cathartic CT colonography in population-based screening for colorectal cancer: a randomised controlled trial.

BACKGROUND: Screening for colorectal cancer is widely recommended, but the preferred strategy remains unidentified. We aimed to compare participation and diagnostic yield between screening with colonoscopy and with non-cathartic CT colonography. METHODS: Members of the general population, aged 50-75 years, and living in the regions of Amsterdam or Rotterdam, identified via the registries of the regional municipal administration, were randomly allocated (2:1) to be invited for primary screening for colorectal cancer by colonoscopy or by CT colonography. Randomisation was done per household wit

Role of Topical Nitroglycerin in Prevention and Management of Post-mastectomy Skin Flap Necrosis: A Prospective Randomized Controlled Study.

INTRODUCTION: Mastectomy skin flap necrosis is a common postoperative complication following breast cancer surgery and is associated with delayed wound healing, increased morbidity, prolonged hospital stay, additional surgical procedures, and delayed initiation of adjuvant therapy. Impaired flap perfusion is considered the primary underlying mechanism. Topical nitroglycerin, through nitric oxide-mediated vasodilation, has been proposed as a simple and cost-effective method to improve flap vascularity and reduce ischemic complications. This study aimed to evaluate the efficacy and safety of top

Transcranial direct current stimulation of the dorsolateral prefrontal cortex improves dual-task performance in people with stroke.

BACKGROUND: Gait and cognitive dysfunction in people with stroke are often exacerbated during dual tasks, limiting their ability to perform daily tasks. This study aimed to investigate the effects of anodal transcranial direct current stimulation (tDCS) over the left dorsolateral prefrontal cortex (DLPFC) on dual-task performance in individuals with chronic stroke. Unlike previous studies that primarily targeted motor cortical regions (e.g., M1), our innovative approach focuses on stimulating the DLPFC, a key cognitive region, to enhance cognitive-motor resource allocation, reduce dual-task co

The fracture orthopedic risk of non-home discharge (FORD) score: A novel bedside predictive tool for non-home discharge in orthopedic trauma patients.

INTRODUCTION: Non-home discharge after orthopedic trauma is associated with worse outcomes, increased costs, and greater resource utilization. Existing prediction tools often rely on hospital course variables unavailable at presentation or are limited to specific fracture populations. This study aimed to develop and validate the Fracture Orthopedic Risk of Non-Home Discharge (FORD) Score, a bedside tool using only emergency department-available variables to predict non-home discharge in adult fracture patients. METHODS: A retrospective cohort study was conducted of adult fracture patients tre

Avoiding routine gastric residual volume measurement in neonatal critical care (the neoGASTRIC trial): study protocol for a multi-centre, unblinded, randomised, controlled trial.

BACKGROUND: Routine measurement of gastric residual volumes involves regularly aspirating the entire stomach contents to assess the volume and colour of the aspirate to inform feeding. This is an established practice in many United Kingdom and Australian neonatal units for preterm infants receiving gastric tube feeds. The rationale is to assess feed tolerance and to predict and potentially prevent necrotising enterocolitis, a serious gut condition. Routine measurement of gastric residual volumes may also be associated with adverse outcomes and harm, including delayed achievement of full entera

Effectiveness of Nurse-Led atrial Fibrillation Detection for Patients With Embolic Stroke of Unknown Source in Stroke Units: A Retrospective Study.

BACKGROUND: Atrial fibrillation is a primary cause of ischemic stroke, necessitating early detection, particularly in patients with embolic stroke of undetermined source. While nurseled continuous electrocardiogram monitoring allows real-time rhythm assessment in acute stroke units, its specific clinical impact remains insufficiently clarified. AIM: To evaluate the impact of nurse-led notification of suspected atrial fibrillation in patients with cryptogenic stroke. STUDY DESIGN: A retrospective medical record review was conducted on patients with cryptogenic stroke admitted to a stroke unit

The short-term effects of dry cupping the lumbar paraspinal muscles on pain and function in individuals with non-specific low back pain: A single-blind randomized trial.

Background: Non-specific low back pain (NSLBP) is a major global cause of disability, contributing to substantial healthcare costs and reduced quality of life. Current treatments often provide inconsistent relief. Dry cupping has shown potential for improving pain and function, but high-quality evidence supporting its efficacy in NSLBP remains limited. Objective: This study assessed the short-term effects of dry cupping on pain and function in individuals with NSLBP. Methods: A single-blind randomized clinical trial enrolled 52 adults (31 females, 21 males, aged 18-55 years) with clinician-con

Ultrasound-guided stellate ganglion acupuncture for chronic insomnia disorder using fNIRS: study protocol for a randomized controlled trial.

BACKGROUND: Chronic insomnia disorder (CID) is common and often impairs daytime functioning and quality of life. Although cognitive behavioral therapy for insomnia and hypnotic medication are widely used, their clinical use may be limited by accessibility, cost, adherence, and concerns about long-term adverse effects. Acupuncture is commonly used for CID, but conventional acupuncture alone may require a relatively long treatment course. Because sympathetic hyperarousal plays an important role in insomnia, the stellate ganglion (SG) may represent a useful therapeutic target. This trial aims to

Effect of Implementing a Nursing-Led SKINCARE Bundle for Preventing Medical Device-Related Pressure Injury in Critical Care Units.

BACKGROUND: Medical device-related pressure injuries (MDRPIs) are generally preventable yet increasingly recognised complications in critical care units, contributing to patient harm, prolonged hospitalisation and higher costs. A nursing-led skincare bundle may provide a structured, evidence-based approach to standardising practice for the prevention of MDRPIs. AIM: This study aimed to evaluate the effect of implementing the SKINCARE bundle on the prevention of MDRPIs in critically ill patients. STUDY DESIGN: A quasi-experimental study in critical care units evaluating the effect of a SKINCA

End-to-side anterior interosseous nerve transfer augmentation of ulnar nerve function in moderate and severe cubital tunnel syndrome; a randomised controlled trial protocol.

BACKGROUND: The anterior interosseous nerve (AIN) end-to-side (ETS) nerve transfer is a relatively recent treatment innovation in managing cubital tunnel syndrome (CuTS). Potential benefits must be balanced against the risks associated with the adoption of an innovative technique, evaluating procedure-specific risks, and considering additional costs in the safe delivery of care. The ETS technique has been proposed as a method of improving intrinsic muscle function in the hand in the setting of ulnar nerve compression at the elbow. The technique is controversial with emerging evidence from retr

A protocol for the rehabilitation of patients undergoing mechanical ventilation from the ICU to the post-hospital phase: the MOVE-ICUtoHOME, a multicentre randomised controlled trial.

INTRODUCTION: Intensive care unit-acquired weakness (ICU-AW) in patients with acute respiratory failure requiring invasive mechanical ventilation (IMV) is associated with reduced functional exercise capacity and increased mortality. Early mobilisation in the ICU may improve functional outcomes and reduce length of stay; however, evidence remains limited, protocols are heterogeneous and post-discharge rehabilitation is frequently lacking. This study aims to evaluate the effects of a continuous care programme combining early intensive ICU mobilisation with post-hospitalisation rehabilitation on

Adjuvant Use of Intranasal Fluticasone Furoate to Improve Graft Uptake in Type 1 Tympanoplasty Cases: A Comparative Study.

BACKGROUND: Chronic otitis media (COM) with tympanic membrane perforation remains a significant cause of hearing impairment worldwide. While tympanoplasty is the standard surgical treatment, graft failure rates of 10%-30% persist despite technical refinements. Topical corticosteroids may improve outcomes by modulating postsurgical inflammation. OBJECTIVE: The objective of the study was to compare graft uptake rates in type 1 tympanoplasty between patients receiving adjuvant topical intranasal fluticasone furoate (FF) versus standard postoperative care alone. METHODOLOGY: This prospective cro

Spatiotemporal patterns and climate-induced macroeconomic burden of malaria in sub-Saharan Africa.

BACKGROUND: The global malaria burden is characterized by economic, geographical, and climatic disparities, especially in sub-Saharan Africa (SSA). Moreover, meteorological factors have become increasingly important to understand the malaria burden in SSA in the context of climate change. METHODS: Value of Statistical Life Year (VSL) and machine learning are proposed to jointly assess the weather-induced economic burden of malaria in SSA from 2000 to 2022. Geographic information systems (GIS) are employed to examine the spatial autocorrelation of malaria burden and address the issues of spati

Rapid, season-specific PCR testing versus traditional diagnostics for pneumonia in the emergency department.

BACKGROUND: Traditional culture-based diagnostics for emergency-department (ED) pneumonia are slow and season-agnostic, delaying targeted therapy. We evaluated whether season-tailored multiplex PCR panels accelerate pathogen identification and improve antibiotic stewardship. METHODS: In a single-center, prospective study, adults with radiographically confirmed pneumonia were enrolled consecutively and allocated by a rotating week-on/week-off schedule to either a seasonal PCR panel or conventional diagnostics. Primary outcomes were (i) time to final pathogen report and (ii) diagnostic yield (≥
