Screening for Perinatal OCD: A Comparison of the DOCS and the EPDS.

Screening for perinatal-occurring obsessive-compulsive disorder (OCD) is rare. We sought to evaluate the Dimensional Obsessive-Compulsive Scale (DOCS) as a screening tool for perinatal OCD and compare the screening accuracy of the DOCS with the commonly recommended Edinburgh Postnatal Depression Scale (EPDS). English-speaking, pregnant individuals aged 19+ (N = 574) completed online questionnaires and diagnostic interviews to assess for OCD prenatally and twice postpartum. The DOCS total score demonstrated the highest level of accuracy. Neither the EPDS-Full nor the three-item Anxiety subscale

Treatise on insanity in pregnant, postpartum, and lactating women (1858) by Louis-Victor Marcé: a commentary.

Louis-Victor Marcé, MD, of Paris compiled an extensive monograph published in 1858 that surveyed knowledge of psychiatric disorders of women during and following pregnancy. This work has largely been ignored for 150 years. We summarize here what is known about Marcé's life and work, and include selected passages from his monograph. Marcé provides extensive clinical descriptions of syndromes, with 79 case examples, and summarizes etiological theories and treatments characteristic of his era and place. This work was based on cases that he personally evaluated and on other reported cases, all dra

Mood states and milk output in lactating mothers of preterm and term infants.

The purpose of this study is to compare psychological distress via both negative and positive mood states between 2 different groups of lactating mothers during the first 6 weeks postpartum with a large sample. Mood states were measured using the Multiple Affect Adjective Check List-Revised by a convenience sample of newly delivered mothers from 4 tertiary care units in Illinois. Preterm mothers' negative mood states of anxiety, depression, hostility, and dysphoria were significantly greater than those reported for term mothers. For the positive mood states of positive affect and PASS (positiv

Trauma History Questionnaire: validation with novel samples of incarcerated women and perinatal women.

Trauma exposure is associated with numerous negative outcomes, many of which are amplified within at-risk populations. Two under-researched and at-risk populations, incarcerated women and perinatal women, both report high rates of trauma, psychopathology, and PTSD compared to the general population. One common measure of trauma exposure in various populations, including incarcerated women and perinatal women, is the Trauma History Questionnaire (THQ). However, no known studies have validated the THQ within these two unique, understudied populations. Using data from two studies of incarcerated

Trajectories of perinatal sleep quality from early pregnancy to six weeks postpartum and their risk factors: A multicenter longitudinal study in China.

BACKGROUND: Poor sleep quality during pregnancy is common and linked to adverse maternal and fetal outcomes. This study examined different sleep trajectories during pregnancy and risk factors associated with sleep quality. METHOD: Data came from a prospective cohort study of 1210 Chinese pregnant women who were within 13 weeks of gestation. Participants completed surveys at seven time points: five during pregnancy (gestational week 10-13 (T1), week 15-18 (T2), week 23-25 (T3), week 30-32 (T4) and week 36-37 (T5)) and two postpartum (7 days (T6) and 6 weeks (T7)). Factors influencing receipt of mental health services for Black women living in Los Angeles County during the perinatal period.

BACKGROUND: Although California is regarded as a leader in reproductive freedoms and healthcare within the United States, Black women experience persistent inequities in perinatal mental healthcare. Despite healthcare system involvement, a significant proportion of Black women experiencing symptoms of perinatal mood and anxiety disorders (PMADs) go untreated. The purpose of this study was to 1) characterize clinician screening practices for PMADs; 2) describe the extent of perinatal mental health needs; and 3) identify factors within health systems that influence receipt of mental health servi

Joint effects of recent stressful life events and adverse childhood experiences on perinatal comorbid anxiety and depression.

BACKGROUND: Stressful life events (SLEs) and adverse childhood experiences (ACEs) have been reported to be associated with perinatal depression (PND) or perinatal anxiety (PNA) alone; however, in most cases, majority of PND and PNA coexist and could lead to more serious health consequences. The independent effect of recent SLEs and their joint effects with ACEs on perinatal comorbid anxiety and depression (CAD) remain inadequately explored. METHODS: Based on a longitudinal study, 1082 participants receiving prenatal care in Ma'anshan, China were included. Women were recruited in the first tri

Peripartum depression and infant care, sleep and growth.

Peripartum depression is a common, serious complication in mothers. To assess the influence of infant care, sleep and growth on the risk of peripartum depression, 1,271 mothers of healthy one-month-old infants completed comprehensive questionnaires including the Edinburgh Postnatal Depression Scale. Relationships between high depression scores and variables related to infants' care, sleep and growth were assessed adjusting for other variables. High depression scores were found in 233 mothers, which were associated with variables related to infants' care (poor satisfaction with infant care supp

Perceptions of predisposing and protective factors for perinatal depression in same-sex parents.

Increasing numbers of women are choosing to have children in the context of same-sex relationships or as "out" lesbian or bisexual individuals. This study used qualitative methods to assess perceived predisposing and protective factors for perinatal depression in lesbian, gay, bisexual, and queer (LGBQ) women. Two focus groups with LGBQ women were conducted: 1) biological parents of young children and 2) nonbiological parents of young children or whose partners were currently pregnant. Three major themes emerged. Issues related to social support were primary, particularly related to disappoint

Rumination time and reticuloruminal temperature as possible predictors of dystocia in dairy cows.

The objectives of this study were to explore changes of rumination time and reticuloruminal pH and temperature of dairy cows and heifers (means ± standard deviation; age = 5.8 ± 1.9; parity = 2.7 ± 1.4; body condition score = 3.2 ± 0.2) with eutocic (EUT, n = 10) and dystocic calving (DYS, n = 8). The recording period lasted from 3 d before calving until 7 d in milk. For the comparison of rumination time and reticuloruminal characteristics between groups, time to return to baseline (the time interval required to return to baseline from the delivery of the calf) and area under the curve (AUC; b

Latent profiles analysis of maternal perinatal stress and their association with NR3C1 expression in the perinatal period.

Maternal psychosocial stress during the perinatal period is highly prevalent and a major risk factor for maternal and child health. However, the operationalization of perinatal stress remains fragmented, and its biological embedding is poorly understood. This study aimed to (1) identify latent profiles of maternal perinatal stress and (2) examine their association with maternal NR3C1 expression, a molecular marker of hypothalamic-pituitary-adrenal (HPA) axis regulation reflecting glucocorticoid receptor availability and feedback sensitivity. A total of 241 mothers were recruited during pregnan

Resilience and Stress during Pregnancy: A Comprehensive Multidimensional Approach in Maternal and Perinatal Health.

This narrative review addresses resilience and stress during pregnancy, which is part of a broader concept of maternal health. Pregnancy and postpartum are opportune periods for health promotion interventions, especially because the close contact of the women with health professionals. In this way, it can be considered a useful window of opportunity to identify women at higher risk for adverse outcomes. Integrated health is a concept that aims at providing comprehensive care related to the promotion of individuals' physical, mental, and social well-being. In this context, stress during pregnan

Association of perinatal factors with neurodevelopmental referrals in a population-based cohort study in Japan.

Although the causes of neurodevelopmental disorders remain unknown, several environmental risk factors have attracted considerable attention. We conducted a retrospective, longitudinal, population-based cohort study using data from infant health examinations of children born to mothers with pregnancies between April 1, 2014 and March 31, 2016 in Kobe City to identify the perinatal factors associated with neurodevelopmental referrals in 3-year-old children. There were 15,223 and 1283 children in the normal and referral groups, respectively. Neurodevelopmental referrals at the health checkup for

Caring for the mother-child dyad as a family physician.

Infant social, emotional and neurological development is shaped by the mother-child dyad. Dysfunction in this bond, as well as maternal mental health problems, can negatively impact child development. The family physician is well-placed to spot dysfunction in the mother-child dyad and screen for postnatal depression during well-child visits. If any issues are identified, the family physician can provide support and help the mother-child dyad to access community resources and specialist psychiatric services.

Antipsychotic drugs and breastfeeding.

OBJECTIVE: The incidence of psychotic disorders during the postpartum period is higher than at any other time during a women's life and coincides with the time when breastfeeding is most recommended. As a result, safety data on use of antipsychotic drugs during lactation is essential. Our aim was to analyze the medical literature for information on antipsychotic drug use during breastfeeding and to determine the safety of their use for the exposed infant. DATA SOURCES: Medline (U.S. National Library of Medicine), LactMed (U.S. National Library of Medicine) and Reprotox (Reproductive Toxicolog

Psychological/verbal abuse and utilization of mental health care in perinatal women seeking treatment for depression.

Research on psychological violence has suggested it is common among perinatal women and is predictive of later physical violence. Psychological violence is also a strong correlate of negative mental and physical health outcomes and may influence engagement in health services. Both physical and mental health care are of critical importance for perinatal women who may be especially vulnerable to psychological violence and its deleterious effects. This study examined the clinical records of 299 perinatal patients who received treatment in a psychiatric partial hospital program to determine whethe
