INTRODUCTION: We performed a retrospective study of 145 consecutive patients referred on suspicion of dementia. We were particularly interested in the occurrence of reversible dementia. Dementia was defined with reference to the ICD-10 criteria.
MATERIAL AND METHODS: All patients underwent a clinical neurological examination, a battery of blood tests, and a CT scan of the brain.
RESULTS: Dementia was found in 63% and in only 4% was it potentially reversible: Three patients had subnormal cobalamin and one patient had normal pressure hydrocephalus. However, treatment did not influence the stat

The pattern of injury of the specific cell structures of the central nervous system (CNS) is different in the various types of the dementia syndrome. We challenged the hypothesis that this could be reflected in specific patterns of brain-specific proteins in the cerebrospinal fluid (CSF). The neuron-specific enolase (NSE), S-100, myelin basic protein (MBP) and lactate levels were retrospectively analyzed in the CSF of 159 patients with various types of dementia. A previous study from our department demonstrated age-related reference values for the brain-specific proteins in the CSF. The presen

Freezing is a common symptom in parkinsonian syndromes, but its association with different causes of parkinsonism as well as with other symptoms has never been investigated. We conducted a database survey of the occurrence of freezing in parkinsonism. Of 347 patients with a clinical diagnosis of parkinsonism other than idiopathic Parkinson's disease and with specific data regarding freezing, 158 patients had freezing (46%). Freezing was significantly associated with progression of the disease as rated on the Hoehn & Yahr scale [odds ratio (OR), 1.69; p < 0.004]. Gender was not a risk factor fo

We report Japanese siblings with the missense mutation 717Val --> Ile in the amyloid precursor protein. The maternal grandmother died of an unknown dementing disorder. The proband's mother had gradually increasing amnesia beginning at age 64, which was diagnosed as Alzheimer's disease (AD). She died in a psychiatric hospital (duration of illness: 16 years). Both the proband and her elder sister were affected at about age 55 years. Disturbances of memory, judgment, and emotion, as well as personality changes, occurred first, with dementia eventually predominating. The elder sister died of pneum

INTRODUCTION: Akinetic crisis is a severe deterioration of motor performance occurring in syndromes with pre- or postsynaptic dopaminergic deficit, necessitating effective dopamine replacement therapy. The subcutaneously applicable levodopa derivative foslevodopa represents a new therapeutic option for patients with advanced Parkinson's disease as a continuous therapy. However, its potential role as a parenteral treatment option for akinetic crisis has not been investigated, yet.
THE CASE: A 78 year-old patient who had developed akinetic-rigid symptomatology in the context of normal pressure

Spinal cavernous hemangioma is rare, and it is extremely rare for cavernous hemangioma to develop in the cauda equina. There has been only one report of hydrocephalus associated with cavernous hemangioma in the cauda equina. We report a case of cavernous hemangioma in the cauda equina diagnosed on the basis of the headaches due to hydrocephalus. A 67-year-old man was being treated for Parkinson's disease because of tremor of both upper extremities for several years. In December 1991 he complained of occasional headaches. On February 15, 1992 the headaches became severe and frequent, with nause

The present study introduces an adaptive calculation of approximate entropy (ApEn) by exploiting sample-by-sample construction and update of nearest neighborhoods in an n-dimensional space. The algorithm is first validated with a standard numerical test set. It is then applied to electrocardiogram R wave interval (RR) and beat-to-beat intracranial pressure signals recorded from 12 patients undergoing normal pressure hydrocephalus diagnosis. The ApEn time series are further processed using the causal coherence analysis to study the interaction between ICP and RR interval. Numerical validation d

Since the development of phase-contrast magnetic resonance imaging (PC-MRI), quantification of cerebrospinal fluid (CSF) flow across the cerebral aqueduct has been utilized for diagnosis of conditions such as normal pressure hydrocephalus (NPH). This study aims to develop an automated method of aqueduct CSF flow analysis using convolution neural networks (CNNs), which can replace the current standard involving manual segmentation of aqueduct region of interest (ROI). Retrospective analysis was performed on 333 patients who underwent PC-MRI, totaling 353 imaging studies. Aqueduct flow measureme

PURPOSE: Brain abnormalities in patients with syndromic craniosynostosis can either be a direct result of the genetic defect or develop secondary to compression due to craniosynostosis, raised ICP or hydrocephalus. Today it is unknown whether children with syndromic craniosynostosis have normal brain volumes. The purpose of this study was to evaluate brain and ventricular volume measurements in patients with syndromic and complex craniosynostosis. This knowledge will improve our understanding of brain development and the origin of raised intracranial pressure in syndromic craniosynostosis.

BACKGROUND AND PURPOSE: Quantitative MR imaging allows segmentation of different tissue types and automatic calculation of intracranial volume, CSF volume, and brain parenchymal fraction. Brain parenchymal fraction is calculated as (intracranial volume - CSF volume) / intracranial volume. The purpose of this study was to evaluate whether the automatic calculation of intracranial CSF volume or brain parenchymal fraction could be used as an objective method to monitor volume changes in the ventricles.
MATERIALS AND METHODS: A lumbar puncture with drainage of 40 mL of CSF was performed in 23 pat

The choroid plexus (ChP), a highly vascularized brain structure responsible for cerebrospinal fluid (CSF) production, undergoes significant age-related changes that may contribute to neurodegenerative diseases involving disrupted immune regulation, fluid homeostasis and waste clearance. Compared to other brain regions, vascular research on the ChP remains limited despite its critical role as a central interface between the blood and CSF. This review focuses on age-related vascular and structural alterations in the ChP from both histopathological and neuroimaging perspectives, and explores thei

The cervical carotid Doppler velocity is determined by the cardiovascular function, hemodynamic characteristics of proximal vascular beds, the carotid artery, and the distal vascular beds, i.e., cerebral circulation. If the former three factors remain constant, cervical carotid Doppler velocimetry reflects the cerebral circulation. The purpose of this report is to study whether the cervical carotid Doppler velocimetry reflects cerebral circulation or not. Parameters measured by the common carotid Doppler velocimetry were compared with cerebral blood flow (CBF) in the internal carotid arterial

Diagnosing Alzheimer's disease (AD) through pathological markers is typically costly and invasive. This study aims to find a noninvasive, cost-effective method using portable electroencephalography (EEG) to detect changes in AD-related biomarkers in cerebrospinal fluid (CSF). A total of 102 patients, both with and without AD-related biomarker changes (amyloid beta and phosphorylated tau), were recorded using a 2-minute resting-state portable EEG. A machine-learning algorithm then analyzed the EEG data to identify these biomarker changes. The results showed that the machine learning model could

Pneumocephalus is a rare but potentially life-threatening condition caused by the presence of air within the intracranial cavity. It commonly arises from trauma, neurosurgical procedures, infections, malignancies, or spontaneous causes. In patients with ventriculoperitoneal (VP) shunts, pneumocephalus may result from complications such as altered cerebrospinal fluid dynamics or skull base defects leading to a vacuum-like effect. We report a case of an 83-year-old male with a history of VP shunt placement for normal pressure hydrocephalus who presented with acute neurological deterioration, inc

We present a case of cervical myelopathy caused by epidural hematoma formation due to chronic cerebrospinal fluid overdrainage. A 55-year-old man who underwent ventriculoperitoneal (V-P) shunt surgery for normal pressure hydrocephalus presented with progressive weakness of both the upper and lower extremities. Magnetic resonance imaging (MRI) revealed compressive myelopathy at the cervicomedullary junction at the C1-C2 level caused by epidural hematoma formation due to intracranial hypotension (IH) caused by a complication of V-P shunt. He underwent decompressive laminectomy and hematoma remov

The afferent visual system is one of the most common structures involved in patients with craniopharyngioma, and the manifestations include deficits in visual acuity, color vision, and visual fields. Here, we report a case of craniopharyngioma that presented with acute blindness in an elderly man. A healthy 54-year-old man presented with an acute progressive blurring of vision and became blind in six weeks. He developed symptoms of increased intracranial pressure only a week after becoming blind. On examination, visual acuity in both eyes was no perception of light (NPL). He also had left esot

A 78-year-old man, who had undergone lumboperitoneal shunt (LPS) placement for idiopathic normal-pressure hydrocephalus eight years prior, presented with intermittent claudication, lower back pain, and radicular pain on the inside of the right thigh. Magnetic resonance imaging (MRI) revealed an extradural arachnoid cyst (EDAC) above the lumbar catheter of the LPS. The EDAC compressed the spinal dural sac and cauda equina toward the anterior side at level L3/4, triggering his clinical manifestations. The LPS was removed and simultaneously converted into a ventriculoperitoneal shunt (VPS), which
