Renal manifestations of primary mitochondrial disorders.

The aim of the present review was to summarize and discuss previous findings concerning renal manifestations of primary mitochondrial disord … , growth retardation, aminoaciduria, cholestasis, iron overload, lactacidosis, and early death, and hyperuricemia, pulmonary hypertension, renal failure in infancy and alkalosis syndrome. The present study proposes that the frequency of renal involvement in MIDs is probably underestimated. Diagnosis of renal involvement follows general guidelines and treatment …

Alcohol-related diseases and alcohol dependence syndrome is associated with increased gout risk: A nationwide population-based cohort study.

OBJECTIVE: Alcohol intake is strongly associated with hyperuricemia, which may cause gout. This study evaluated the risk of gout in patients with alcohol-related diseases and alcohol dependence syndrome. METHODS: We used the Taiwan National Health Insurance Research Database (NHIRD) to conduct a nationwide population-based cohort study to assess the risk of gout and gout incidence in patients with alcohol-related diseases and alcohol dependence syndrome (as defined by the International Classification of Diseases, Ninth Revision). In the NHIRD records from 1998 to 2008, we identified 11,675 ca…

Rare cause of abdominal pain and fever in a pregnant woman.

CLINICAL PRESENTATION: A pregnant woman (third trimester) presented with intense abdominal pain, nausea and myalgia. The patient was obese (body mass index 38) and was being treated for high blood pressure, hyperuricemia and hypothyroidism. She had chronic renal deficiency related to focal segmental glomerular sclerosis requiring dialysis. The physical examination at admission revealed a fever of 39°C and an acute abdomen with abdominal guarding in the right upper quadrant without hepatomegaly or splenomegaly. There were no clinical signs of pre-eclampsia. Fetal ultrasound and a Doppler of the…

Novel UMOD mutations in familial juvenile hyperuricemic nephropathy lead to abnormal uromodulin intracellular trafficking.

BACKGROUND: Familial juvenile hyperuricemic nephropathy (FJHN) is an autosomal dominant disorder characterized by hyperuricemia and progressive chronic kidney disease. Uromodulin gene (UMOD) mutations, leading to abnormalities of uromodulin intracellular trafficking contribute to the progress of the disease. METHODS: We did UMOD screening in three Chinese FJHN families. We thus constructed mutant uromodulin express plasmids by site-mutagenesis from wild type uromodulin vector and transfected them into HEK293 (human embryonic kidney) cells. And then we detected uromodulin expression by western…

Factors associated with recurrent hospital admissions for gout: a case-control study.

OBJECTIVE: Although gout has recognized prevention and treatment options, some patients have recurrent unplanned hospital admissions with go … e on lower doses (median dose 200 mg/d vs. 300 mg/d, P = 0.0019). CONCLUSION: Medical comorbidity, hyperuricemia, and inadequate allopurinol use are associated with repeated unplanned hospitalization for gout. These data suggest that the use of intensive urate-lowering therapy may prevent such admissions.

Immature renal structures associated with a novel UMOD sequence variant.

Mutations of the UMOD gene, encoding uromodulin, have been associated with medullary cystic kidney disease 2, familial juvenile hyperuricemic nephropathy, and glomerulocystic kidney disease. We report on a 13-year-old boy presenting with chronic reduced kidney function, hyperuricemia, and impairment in urine-concentrating ability. His father was affected by an undefined nephropathy that required transplantation. The boy's renal ultrasonography showed reduced bilateral kidney volumes and cortical hyperechogenicity, with 2 tiny cysts in the left kidney. Renal biopsy showed up to 60% of glomeruli…

[Alcohol intake and gamma -GTP observed from the viewpoint of an occupational physician].

Alcohol is an important basic factor in health management at the workplace. The fact is, however, when alcohol is pervasive in a worker's daily life, effective measures are very difficult to carry out. We examined an intervention program based on serum y -GTP (IU/l) measurements at physical examination. Subjects were clients of the Keio Counseling Center in2005 (male, 5568: female, 1725). Among nondrinkers, gamma-GTP values were under 50 in 83% of men and 93% of women. Relative risk of lifestyle-related diseases (obesity, hypertension, hyperlipidemia, hyperuricemia, hyperglycemia and fatty liv…

Use of propofol anesthesia during outpatient radiographic imaging studies in patients with Lesch-Nyhan syndrome.

Lesch-Nyhan syndrome is a rare, x-linked, recessive disorder of purine metabolism resulting in hyperuricemia, spasticity, choreoathetosis, dystonia, self-injurious behavior, and aggression, without significant cognitive impairment. Anesthetic management of inpatients who demonstrate classic manifestations of Lesch-Nyhan syndrome and require surgical interventions have been described. There are no guidelines in the literature addressing the anesthetic management of the outpatient with Lesch-Nyhan syndrome. Specifically, sudden, unexplained death, abnormalities in respiration, apnea, severe brad…

Protein hydrolysates and xanthine oxidase inhibitory peptides from sunflower capitulum exert antihyperuricemic effects: a comprehensive study based on serum metabolomics and gut microbiota analysis.

Hyperuricemia (HUA), which results from an imbalance between uric acid production and excretion, is a major risk for gout and other metabolic disorders. Food-derived bioactive peptides have attracted increasing attention as dietary candidates for HUA management. However, the relative in vivo efficacy of protein hydrolysates and their derived xanthine oxidase inhibitory peptides remains unclear. In this study, sunflower capitulum protein hydrolysate (SCPH) and its derived peptide, Leu-Gly-Asp-Phe (LGDF) were systematically compared by combining in vitro assays, animal experiments, serum metabol…

Pollen of Brassica campestris L. ameliorates hyperuricemic and obstructive nephropathy by targeting the β-catenin/TCF4-ferroptosis pathway.

ETHNOPHARMACOLOGICAL RELEVANCE: The pollen of Brassica campestris L. (BCP, syn. Brassica rapa L. subsp. oleifera (DC.) Metzg.), included in the Chinese Pharmacopoeia as a licensed patent medicine (Pule'an tablets), is used clinically for kidney-qi deficiency-related lower urinary tract symptoms in chronic prostatitis and benign prostatic hyperplasia. Prostatic obstruction is a common cause of obstructive uropathy that may progress to renal fibrosis. However, the renoprotective effects and the underlying mechanism remain to be fully elucidated. AIM OF THE STUDY: To study the renoprotective eff

Is there a relationship between serum uric acid and prostate-specific antigen in middle-aged and elderly Chinese men?

Hyperuricemia may be one of the risk factors for the development of prostate cancer. We evaluated the association between serum uric acid (SUA) and elevated prostate-specific antigen (PSA) in the middle-aged and elderly Chinese men, and the adjustment effects of age, glycolipid metabolism and renal function. From January 2019 to December 2024, 967 participants from the middle-aged and elderly Chinese men who attended the urology outpatient clinic of Shuguang Hospital (Shanghai, China) were recruited in this cross-sectional study. Blood samples from participants were collected for the determina…

Prolonged Fructose Overconsumption Activates Original Biosynthetic and Metabolic Pathways of Endogenous Purine in Rats.

Fructose is a nutrient used widely and indiscriminately as a sweetener in various types of foods and beverages. Accumulating evidence suggests that dietary fructose has a close relationship with metabolic diseases like hyperuricemia (HUA). However, the role and metabolic fate of fructose intake remain elusively obscure. The present work made a pioneering endeavor to unravel the potential mechanistic link between prolonged fructose overconsumption and the incidence of HUA. High-fructose (30%) free drinking was provided to rats for 17 weeks. Results indicated that long-term high-fructose intake …

Conducting genetic epidemiology research on hyperuricaemia and gout in an indigenous community in Guam - a feasibility study.

BACKGROUND: Gout, the most common inflammatory arthritis disease, and hyperuricaemia onset are influenced by environmental and genetic factors. We sought to investigate these factors in an Indigenous community in Guam. METHODS: In this feasibility study, the University of Guam led the qualitative inquiry with the native community, training (pre-screening of participants, data collection methods, and biospecimen handling), study implementation (outreach and recruitment, data collection, and DNA extraction and quantification), and qualitative and epidemiologic data analyses. Recruitment targets

[Gout, beyond the joint: How should we treat it?].

Gout is a disease caused by the chronic deposition of monosodium urate crystals. Its clinical presentation as an acute, self-limiting arthritis and the belief that it is a banal, self-inflicted disease have led to its poor management. Despite advances in the knowledge of the disease and the simplicity of its management, no more than 30% of patients are well treated. In Spain, the prevalence of gout is 2.5% and its incidence is increasing. In the following article we will review the pathogenesis of gout and hyperuricaemia, highlighting the greater weight of genetics and renal function over diet

Two still unanswered questions about uric acid and cardiovascular prevention: Is a specific uric acid cut-off needed? Is hypouricemic treatment able to reduce cardiovascular risk?

AIMS: The most frequent consequence of elevated uric acid (UA) levels is the development of gout and urate kidney disease. Besides these effects, several studies have investigated the association between hyperuricemia and cardiovascular (CV) disease. High serum UA has been identified as an important determinant of all-cause and CV mortality and CV events (acute and chronic coronary syndrome, stroke and peripheral artery disease). Despite the high number of publications on this topic, there are two questions that are still unanswered: do we need a specific CV cut-off of serum UA to better refin…

The effect of metformin on urate metabolism: Findings from observational and Mendelian randomization analyses.

AIM: To evaluate the effect of metformin on urate metabolism. MATERIALS AND METHODS: Using the UK Biobank, we first performed association analyses of metformin use with urate levels, risk of hyperuricaemia and incident gout in patients with diabetes. To explore the causal effect of metformin on urate and gout, we identified genetic variants proxying the glycated haemoglobin (HbA1c)-lowering effect of metformin targets and conducted a two-sample Mendelian randomization (MR) utilizing the urate and gout genetic summary-level data from the CKDGen (n = 288 649) and the FinnGen cohort. Poorly controlled gout: who is doing poorly?

Gout, an inflammatory arthritis caused by the deposition of monosodium urate crystals, is commonly seen in primary care and specialist clinics. In recent years, there has been a resurgence of interest in gout due to advances in therapies and the understanding of pathophysiology, with new guidelines being published by international bodies. However, there is still a gap between the goals of treatment and actual day-to-day practice. Barriers that result in poorly controlled gout include patient factors such as lack of understanding of the disease, stigma and nonadherence to treatment, as well as
