Predicting Mastoid Extension and Complications such as Labyrinthine Fistula and Dural Exposure in Middle Ear Cholesteatoma Using Lesion Size and Detectability on Non-echo-planar Diffusion-weighted MR Imaging.

PURPOSE: This study aimed to quantitatively evaluate whether non-echoplanar diffusion-weighted MRI (non-EP DWI) lesion size and detectability can predict mastoid extension and complications such as labyrinthine fistula and dural exposure in middle ear cholesteatoma. METHODS: This retrospective study included 120 lesions with surgically confirmed middle ear cholesteatoma. Non-EP DWI was performed within 6 months preoperatively and evaluated for lesion detectability and size measurements, including maximum axial diameter, maximum axial area, and volume. Surgical findings were used to assess mas

European Position Statement on Diagnosis, and Treatment of Meniere's Disease.

Meniere Disease keeps challenges in its diagnosis and treatment since was defined by Prosper Meniere at the beginning of 19th Century. Several classifications and definition were made until now and speculations still exist on its etiology. As the etiology remains speculative the treatment models remain in discussion also. The European Academy of Otology and Neurotology Vertigo Guidelines Study Group intended to work on the diagnosis and treatment of Meniere's disease and created the European Positional Statement Document also by resuming the consensus studies on it. The new techniques on diagn

Caloric stimulation and video-head impulse testing in Ménière's disease and vestibular migraine.

CONCLUSION: Both the bithermal caloric test and the video-head impulse test (vHIT) were more often abnormal in Ménière's disease (MD) than in vestibular migraine (VM). Horizontal vestibulo-ocular reflex (hVOR) evaluation with caloric test (low-frequency test) was significantly more often abnormal than vHIT (high-frequency test). Therefore, both tests can be used in a complementary way for frequency-selective testing of peripheral vestibular function. OBJECTIVES: To compare the results of caloric testing and vHIT in MD and VM and to determine which test is more sensitive to uncover peripheral

Blockade of interleukin-6 signaling suppressed cochlear inflammatory response and improved hearing impairment in noise-damaged mice cochlea.

Hearing impairment can be the cause of serious socio-economic disadvantages. Recent studies have shown inflammatory responses in the inner ear co-occur with various damaging conditions including noise-induced hearing loss. We reported pro-inflammatory cytokine interleukin-6 (IL-6) was induced in the cochlea 6h after noise exposure, but the pathophysiological implications of this are still obscure. To address this issue, we investigated the effects of IL-6 inhibition using the anti-IL-6 receptor antibody (MR16-1). Noise-exposed mice were treated with MR16-1 and evaluated. Improved hearing at 4k

[A case of herpetic facial paralysis in which cochleovestibular symptoms outweigh facial nerve symptoms].

A 42-year-old man presented with sensorineural hearing loss of acute onset, tinnitus, and vertigo. Physical examination revealed slight asymmetry in facial nerve functions and spontaneous nystagmus. Magnetic resonance imaging of the internal acoustic canal showed contrast enhancement consistent with edema-inflammation, being notable and diffuse in the seventh and eighth cranial nerve complex, and minimal in the cochlea. Non-hydropic cochleovestibular syndrome was considered and the patient was treated with antiviral and corticosteroid medications. A week later, facial paralysis improved and th

Panic disorder in otoneurological experience.

The evaluation of cochleovestibular dysfunction in psychiatric patients often causes a difficult problem in neurootological experience. The authors discuss here the neurobiological basis of panic disorders and cochleovestibular dysfunction. In this multicenter study, we examined 63 patients with vertigo and panic disease. Twenty patients with primary panic disease and consecutive vertigo composed group 1, whereas group 2 comprised 43 vertiginous patients with secondary panic disease. The most interesting question is whether the patients have an organic vestibular lesion, which would explain wh

Cortical representation of the vestibular system as evidenced by brain electrical activity mapping of vestibular late evoked potentials.

We examined the space and temporal distributions of the rotatory evoked brain electrical activity patterns (brain electrical activity mapping of vestibular evoked potentials [VestEP]) in humans. We performed a longitudinal scalp line analysis, transversal line analysis, and clockwise/counterclockwise rotation analysis of the VestEP principal components in 75 healthy persons aged 22 to 30 years (mean: 25.8). We found that the shortest VestEP latencies and the highest amplitudes were registered in a relatively distinct cortical area that is covered by the transversal electrode line T3-C3-Cz-C4-T

Differential induction of brain-derived neurotrophic factor mRNA in rat inferior olive subregions following unilateral labyrinthectomy.

Vestibular compensation, the neuronal process underlying behavioral recovery from vestibular dysfunction produced by unilateral labyrinthectomy, is attributed to functional reorganization of neuronal circuits in the brainstem and cerebellum. Climbing fibers originating from the inferior olive are suggested to play a crucial role in this compensatory process. To assess the possible contribution of brain-derived neurotrophic factor (BDNF) to the function of climbing fibers during vestibular compensation, we investigated the BDNF mRNA expression in the rat inferior olive after unilateral labyrint

Vestibular Function in Judo Athletes: Evidence of Subclinical Vestibular Dysfunction.

BACKGROUND: Judo is a dynamic combat sport demanding high levels of balance, agility, postural control, and kinesthetic awareness. The vestibular system plays a vital role in maintaining balance and spatial orientation, integrating sensory inputs essential for judo performance. However, few studies have directly examined vestibular function in judo athletes, despite their frequent exposure to head impacts and rapid rotational movements that may affect vestibular integrity. PURPOSE: This study aimed to compare the vestibular functions of active judo athletes with those of healthy controls. RE

Artificial intelligence for understanding concussion: Retrospective cluster analysis on the balance and vestibular diagnostic data of concussion patients.

OBJECTIVES: We propose a bottom-up, machine-learning approach, for the objective vestibular and balance diagnostic data of concussion patients, to provide insight into the differences in patients' phenotypes, independent of existing diagnoses (unsupervised learning). METHODS: Diagnostic data from a battery of validated balance and vestibular assessments were extracted from the database of the Swiss Concussion Center. The desired number of clusters within the patient database was estimated using Calinski-Harabasz criteria. Complex (self-organizing map, SOM) and standard (k-means) clustering to

Acute and chronic effects of single dose memantine after controlled cortical impact injury in adult rats.

BACKGROUND: Altered glutamatergic neurotransmission after traumatic brain injury (TBI) contributes to excitotoxic cell damage and death. Prevention or suppression of such changes is a desirable goal for treatment of TBI. Memantine (3,5-dimethyl-1-adamantanamine), an uncompetitive NMDA receptor antagonist with voltage-dependent open channel blocking kinetics, was reported to be neuroprotective in preclinical models of excitotoxicity, brain ischemia, and in TBI when administered prophylactically, immediately, or within minutes after injury. METHODS: The current study examined effects of memanti

The basis for using bone-conducted vibration or air-conducted sound to test otolithic function.

Extracellular single neuron recordings of primary vestibular neurons in Scarpa's ganglion in guinea pigs show that low-intensity 500 Hz bone-conducted vibration (BCV) or 500 Hz air-conducted sound (ACS) activate a high proportion of otolith irregular neurons from the utricular and saccular maculae but few semicircular canal neurons. In alert guinea pigs, and humans, 500 Hz BCV elicits otolith-evoked eye movements. In humans, it also elicits a myogenic potential on tensed sternocleidomastoid muscles. Although BCV and ACS activate both utricular and saccular maculae, it is possible to probe the
