Neonatal mouse protection against infection with multiple group B streptococcal (GBS) serotypes by maternal immunization with a tetravalent GBS polysaccharide-tetanus toxoid conjugate vaccine.

Most cases of neonatal sepsis and meningitis caused by group B streptococci (GBS) are attributable to one of four major capsular serotypes: Ia, Ib, II, or III. Because resistance to infection with GBS has been correlated with the presence of serum antibodies to the type-specific capsular polysaccharides in both experimental animals and human neonates, efforts have been made to elicit protective immunity with GBS capsular polysaccharide vaccines. However, the GBS capsular polysaccharides alone are not highly immunogenic in either animals or human volunteers. Therefore, we and other investigator

Hyperimmune human IgG or recombinant human granulocyte-macrophage colony-stimulating factor as adjunctive therapy for group B streptococcal sepsis in newborn rats.

Group B streptococcus (GBS) continues to cause considerable morbidity and death in newborn infants despite the use of antibiotics. We investigated the use of adjunctive therapies to be used with antibiotics in the treatment of neonatal sepsis, using a neonatal rat model of established GBS disease. After the development of GBS bacteremia, a human IgG preparation hyperimmune for GBS, administered with penicillin, decreased the mortality rate compared with the use of penicillin alone (14% vs 57%; p = 0.02). Similarly, recombinant human granulocyte-macrophage colony-stimulating factor, administere

Delayed thromboxane synthesis inhibition, but not cholinergic blockade, reverses group B streptococcus-induced pulmonary hypertension.

Anisodamine, an anticholinergic drug, is widely used in China for treatment of infants with septic shock and has been reported to inhibit thromboxane synthesis in cultured cells. Thromboxane A2 plays an important role in the early pulmonary hypertension in sepsis; however, the role of thromboxane A2 later in sepsis is unclear. We tested the hypothesis that thromboxane A2 synthesis inhibition with dazmegrel, and cholinergic blockade with anisodamine, would attenuate the later phase of pulmonary hypertension induced by 4 h of group B streptococcus (GBS) infusion. 1 mg/kg of dazmegrel reversed th

Pathophysiology and histopathology of group B streptococcal sepsis in Macaca nemestrina primates induced after intraamniotic inoculation: evidence for bacterial cellular invasion.

Four pregnant Macaca nemestrina dams at 140-145 days of gestation received an intraamniotic inoculation of group B streptococci (GBS). All four premature infants were born by cesarean delivery, were bacteremic at birth, and showed symptoms of GBS sepsis similar to infected human infants with early-onset disease. Three infants did not receive antibiotics and died of GBS sepsis by 10 h of age despite mechanical ventilation and fluids for blood pressure support. Penicillin treatment of the fourth infant prolonged survival and decreased the requirement for supportive therapy. Quantitative cultures

Human immunoglobulins for intravenous use: comparison of available preparations for group B streptococcal antibody levels, opsonic activity, and efficacy in animal models.

Currently available human immunoglobulin preparations for intravenous use (IVIGs) are being used (with antibiotics) by some physicians for therapy of sepsis in newborns. Most neonatal sepsis and/or meningitis in this country is caused by group B Streptococcus (GBS), and most of these cases are due to type III GBS (III-GBS). The killing of III-GBS in vitro is dependent on specific IgG antibody. Adequate serum levels of specific III-GBS antibody protect the exposed newborn from the development of invasive disease. Therefore, III-GBS was used as a model to evaluate the activity of three IVIG prep

Neonatal septicemia in Stockholm.

In a prospective study to determine the incidence and etiology of neonatal septicemia at Karolinska Hospital, Stockholm, all cases of neonatal septicemia in infants born at the hospital from 1981 to 1985 were reviewed. There were 4.0 cases of neonatal septicemia per 1,000 live births during the study period. During the five year period the incidence of septicemia tended to increase, mainly due to the increasing incidence of septicemia in infants with a birth weight below 1,500 grams. The single most common causative organism was group B streptococci with an incidence of 1.4 per 1,000 live birt

Correlation between low levels of maternal IgG antibodies to R protein and neonatal septicemia with group B streptococci carrying R protein.

A method was designed for quantitation of serum antibodies to R protein in group B streptococci (GBS). Four sera from mothers of infants with neonatal septicemia caused by type II, R+ GBS showed a binding range from 400 to 740 cpm in contrast to a range of 690-1,220 cpm in 5 parturients carrying type II, R+ strains in the urogenital tract giving birth to healthy infants (p less than 0.05). The range of antibodies in sera from 8 mothers to infants infected with GBS type III,R+ was 370-750 cpm, whereas the range in sera from 10 carriers of type III,R+ giving birth to healthy infants varied betwe

Functional analysis of neutrophil granulocytes from healthy, infected, and stressed neonates.

Neutrophil granulocyte function was assessed in 17 well term infants, 14 stressed infants, and eight infants with group B streptococcal infection. Chemiluminescence production elicited by opsonized zymosan or by a wild strain of type III group B streptococci, as well as phagocytosis and killing of streptococci, were measured. Chemiluminescence production by PMNs of term neonates in response to opsonized zymosan or group B streptococci was equal to that of adult controls. In contrast, six of nine stressed or infected neonates had depressed CL responses upon zymosan challenge. When opsonized typ

Escherichia coli and group B streptococcal infections in experimental animals.

Small doses of Escherichia coli endotoxin or of viable E. coli caused only a mild illness in pregnant mice but severely impaired fetal development. After intravenous injection and after experimental renal infection E. coli multiplied in placental tissue, causing resorption of the conceptuses and sometimes fetal infection. It is suggested that in humans coliform bacteraemia sometimes results in abortion and premature delivery because of the placental damage brought about by E. coli and its abortifacient endotoxin. Isolates of group B streptococci from various sites in 175 patients varied marked

Neonatal early-onset infections: Comparing the sensitivity of the neonatal early-onset sepsis calculator to the Dutch and the updated NICE guidelines in an observational cohort of culture-positive cases.

BACKGROUND: The early-onset sepsis calculator (EOSC) reduces unnecessary antibiotic treatment in newborns. However, its performance in identifying cases with early-onset disease (EOD) is unclear. We compared the sensitivity of the EOSC to the current Dutch and National Institute for Health and Care Excellence (NICE) guidelines when applied to a cohort of newborns with culture-positive early-onset sepsis and meningitis. METHODS: Culture-positive Streptococcus agalactiae (GBS) and Escherichia coli (E. coli) sepsis and meningitis patients ≤3 days old with a gestational age ≥34 weeks, identified …

The impact of Vaginal Flora Screening in the third trimester on pregnancy outcomes in primiparas undergoing vaginal delivery: a retrospective cohort study.

BACKGROUND: The necessity of third-trimester vaginal flora screening remains debated. This study aims to evaluate the association between third-trimester vaginal flora screening and pregnancy outcomes among primiparous women undergoing vaginal delivery, as well as neonatal health outcomes. METHODS: This study enrolled 1,380 primiparous women with singleton pregnancies who underwent GBS screening alone or combined GBS plus vaginal flora screening at 35-37 weeks of gestation. Participants were categorized into two cohorts: Group A (GBS screening only; n = 880) and Group B (GBS plus vaginal flor

Predictors, aetiology and outcome of early-onset neonatal bloodstream infection: a case-control study in a tertiary hospital in South Africa.

Difficulty identifying neonates at highest risk of early-onset bloodstream infection (EO-BSI) leads to high empiric antibiotic use. This retrospective case-control study analysed maternal and neonatal factors, pathogen profile, and outcomes of culture-confirmed EO-BSI (<72 hr of life) at a large neonatal unit in Cape Town, South Africa (1 January 2019-31 December 2021). Cases (neonates with culture-confirmed BSI) were matched 1:3 with randomly selected controls ('at risk' neonates with negative blood cultures, C-reactive protein < 10 mg/l and ≤4 days of antibiotics). Factors associated with EO

Maternal and neonatal outcomes of Group B Streptococcus colonization: a retrospective study.

BACKGROUND: Group B Streptococcus (GBS) colonization is one of the major causes of severe neonatal infections. The study was intended to identify GBS colonization in pregnant women, explore its potential risk factors, and analyze the impact of GBS on outcomes for both mothers and newborns. MATERIAL AND METHODS: A retrospective research was carried out on pregnant women who had undergone GBS screening and delivered from June 2020 to December 2022. Pregnant women between 35 and 37 weeks of gestation had GBS screening using real-time polymerase chain reaction (RT-PCR). The clinical characteristi

Group B Streptococcus CAMP Factor Does Not Contribute to Interactions with the Vaginal Epithelium and Is Dispensable for Vaginal Colonization in Mice.

The Gram-positive pathogen group B Streptococcus (GBS) is a leading cause of neonatal bacterial infections, preterm birth, and stillbirth. Although maternal GBS vaginal colonization is a risk factor for GBS-associated adverse birth outcomes, mechanisms promoting GBS vaginal persistence are not fully defined. GBS possesses a broadly conserved small molecule, CAMP factor, that is co-hemolytic in the presence of Staphylococcus aureus sphingomyelinase C. While this co-hemolytic reaction is commonly used by clinical laboratories to identify GBS, the contribution of CAMP factor to GBS vaginal persis

Ultrasound of acquired posterior fossa abnormalities in the newborn.

Neonatal brain sonography is part of routine clinical practice in neonatal intensive care units, but ultrasound imaging of the posterior fossa has gained increasing attention since the burden of perinatal acquired posterior fossa abnormalities and their impact on motor and cognitive neurodevelopmental outcome have been recognized. Although magnetic resonance imaging (MRI) is often superior, posterior fossa abnormalities can be suspected or detected by optimized cranial ultrasound (CUS) scans, which allow an early and bed-side diagnosis and monitoring through sequential scans over a long period

Serial clinical observation for management of newborns at risk of early-onset sepsis.

PURPOSE OF REVIEW: Current management approaches for asymptomatic neonates at risk of early onset sepsis remain controversial. Strategies based entirely on clinical observation (SCO, serial clinical observation) have gained consensus. RECENT FINDINGS: We briefly compare different strategies for managing asymptomatic newborns suggested in four high-income countries. Then this review details the existing differences in carrying out the SCO in the United Kingdom, the USA, and Italy; the experiences from the studies performed using the SCO; and open questions regarding this strategy.
