Mid-term clinical and functional outcomes after reverse shoulder arthroplasty with latissimus dorsi transfer.

BACKGROUND: Although reverse total shoulder arthroplasty (rTSA) with concomitant latissimus dorsi transfer (LDT) has been shown to effectively treat external rotation (ER) deficits, there are limited data regarding its outcomes with modern implants and its impact on activities of daily living (ADLs) requiring ER. The purpose of this study was to assess the mid-term clinical and radiographic outcomes of rTSA with concomitant isolated LDT in patients with an ER lag sign and posterior rotator cuff deficiency. METHODS: This retrospective cohort study with prospective follow-up included consecutiv

Factors affecting functional outcome of surgical correction of claw hand in leprosy.

The objective of this paper is to study the results and the factors that impact the results of claw hand surgery in leprosy. 110 patients who underwent claw hand correction between 2002 and 2006 were followed up and studied. Brand's criteria for objective assessment and a visual analog scale for subjective assessment were used. The factors studied were age, sex, clinical, duration of paralysis, long flexor tightness, degree of contracture and type of surgery. Objective assessment showed Excellent or Good results in 76.6%, Fair in 20% and Poor in 3.4%. Subjective assessment showed that 80.8% we

Calcaneal osteotomy in the treatment of adult acquired flatfoot deformity.

Calcaneal osteotomies are an essential part of our current armamentarium in the treatment of AAFD. Soft tissue correction or bony realignment alone have failed to adequately correct the deformity; therefore, both procedures are used simultaneously to achieve long-term correction. Medial displacement and lateral column lengthening osteotomies in isolation or in combination and the Malerba osteotomy have been employed along with soft tissue balancing to good effect by various authors. The goal is to create a stable bony configuration with adequate soft tissue balance to maintain dynamic equilibr

Suppression of experimental osteoarthritis by adenovirus-mediated double gene transfer.

BACKGROUND: Osteoarthritis (OA) is a chronic and incurable disease, lacking effective treatment. Gene therapy offers a radical different approach to the treatment of arthritis. Even though the etiology of OA remains unclear, there is now considerable evidence to suggest that interleukin-1 (IL-1) and tumor necrosis factor-alpha (TNF-alpha) are the main mediators in the pathogenesis of OA. The goal of this study was to determine the efficacy of local expression of interleukin-1 receptor antagonist (IL-1Ra) and soluble tumor necrosis factor-alpha receptor type I (sTNF-RI) by direct adenoviral-med

Intra-articular biomechanical environment following modified Bristow and Latarjet procedures in shoulders with large glenoid defects: relationship with postoperative complications.

BACKGROUND: Although coracoid transfers including the modified Bristow and Latarjet procedures are widely used to treat anterior shoulder instability, the influence of the choice of procedure on the biomechanical outcomes is not well characterized. We aimed to clarify the intra-articular stress distribution following these 2 procedures using 3-dimensional finite-element analysis and to investigate the role of stress distribution in the pathophysiology of postoperative complications. METHODS: Overall, 6 male patients aged 17-47 years with unilateral anterior shoulder instability were recruited

Arthroscopic, Implant-Free Bone-Grafting for Shoulder Instability with Glenoid Bone Loss.

UNLABELLED: Anterior shoulder instability is commonly associated with injuries to the labrocapsular structures of the glenohumeral joint and with osseous glenoid defects of varying degree. If the bone defect exceeds a clinically relevant threshold, mere soft-tissue repairs result in high rates of recurrent instability and the glenoid needs to be reconstructed with osseous augmentation. The most popular techniques for glenoid reconstruction include variations of coracoid transfer or bone-block procedures; however, these procedures are not free from risks and complications, which are often relat

Accuracy of Palpation-Guided Versus First Attempt at Ultrasound-Guided Electromyography Needle Placement for the Extensor Carpi Radialis Longus and Brevis by Trainees.

INTRODUCTION/AIMS: Ultrasound (US) guidance may be essential for accurate electromyography (EMG) needle placement in less-commonly evaluated muscles, such as the extensor carpi radialis longus (ECRL) and brevis (ECRB), and particularly for identifying suitable donor muscles prior to tendon transfers to restore upper extremity function. Our primary aim was to compare the accuracy of trainee palpation-guided versus first-attempt US-guided needle placement. Our secondary aim was to describe the locations of inaccurate needle placement. METHODS: Seven subjects each performed four needle placement…

Surgical management of leprosy-associated neuropathy in Pakistan: Restoring function and reducing disability in the post-elimination era: A narrative review.

OBJECTIVES: To narratively review the role of surgical interventions in the management of leprosy-associated neuropathy in Pakistan, focusin … in Pakistan are largely limited to specialized centres such as MALC and AKUH. Nerve decompression, tendon transfers, and reconstructive procedures have the potential to restore function and prevent ulcers, with functional recovery rates in comparable low-resource settings. Early surgical intervention combined with rehabilitation yields the best outcomes in most …

A Uniquely Detailed Foot and Ankle Chart for Accurate Recording of Injuries in Dancers.

BACKGROUND: Foot and ankle injuries are common in dance and well documented in ballet. Commonly used injury surveillance tools do not depict the foot and ankle in much detail and are often considered as a single anatomical structure. The foot and ankle houses some of the body's most complex anatomy, and because of the incidence of injuries to these areas in dancers, and different management, a more detailed approach should be taken to reflect the complexity of the area for more accurate research and clinical application. This article proposes and demonstrates that the foot and ankle be broken

Children with Rare Nager Syndrome-Literature Review, Clinical and Physiotherapeutic Management.

Nager syndrome is a rare human developmental disorder characterized by craniofacial defects including the downward slanting of the palpebral fissures, cleft palate, limb deformities, mandibular hypoplasia, hypoplasia or absence of thumbs, microretrognathia, and ankylosis of the temporomandibular joint. The prevalence is very rare and the literature describes only about a hundred cases of Nager syndrome. There is evidence of autosomal dominant and autosomal recessive inheritance for Nager syndrome, suggesting genetic heterogeneity. The majority of the described causes of Nager syndrome include

Sick sinus syndrome as the initial manifestation of neuromyelitis optica spectrum disorder: a case report.

BACKGROUND: Sick sinus syndrome (SSS) is known to occur due to lesions in the medulla oblongata. Although medullary lesions have occurred in patients with neuromyelitis optica spectrum disorder (NMOSD), there are few reports of SSS associated with NMOSD. We report a patient with NMOSD who developed refractory nausea, vomiting and SSS as the initial manifestation. CASE PRESENTATION: A 77-year-old female developed refractory nausea and frequent episodes of syncope. The patient was diagnosed with SSS because sinus pauses lasting five to six seconds were observed, and pacemaker implantation was p

Comparative analysis of clinical outcome and quality of life between amputations and combined bone and flap reconstructions at the lower leg.

PURPOSE: At the lower leg, soft tissue defects with exposed bones, tendons, or hardware require flap coverage. In this retrospective study, we analyzed combined bone and soft tissue reconstructions compared to amputations of the lower leg in a civilian setting. MATERIALS AND METHODS: Patients who underwent combined bone and flap reconstruction (LR) or amputation (LA) of the lower leg were eligible for the study. Bone conditions included fractures and bony defects due to posttraumatic osteomyelitis and non-union. Besides the analysis of the medical history, the study included clinical examinat

Biomechanical Evaluation of a Cadaveric Flatfoot Model and Lateral Column Lengthening Technique.

Patients with adult acquired flatfoot have progressive worsening of bony alignment with many being unable to perform a heel rise. Following reconstruction, pathologic skeletal alignment is corrected and the ability to perform a heel rise is often restored. The purpose of this study was to evaluate the relationship between forefoot liftoff forces and skeletal alignment in a cadaveric flatfoot model by assessing the effect of sequential lengthening of the lateral column using an Evans-type calcaneal osteotomy. Bony alignment was measured in 8 cadaveric specimens with the use of a 3-dimensional d

Biomechanical evaluation of suture buttons versus cortical screws in the Latarjet-Bristow procedure: a fresh-frozen cadavers study.

INTRODUCTION: A commonly used method of fixation of the transferred coracoid in the traditional Latarjet-Bristow procedure (open or arthroscopic) is by two bicortical screws. Although mechanically effective, screw fixation is also a major source of hardware and neurologic complications. This study aimed to compare the biomechanical performances of traditional metal screws and endobuttons as fixators of the Latarjet-Bristow procedure. MATERIALS AND METHODS: Nine fresh-frozen cadaveric human scapulae with the conjoined tendon attached to the coracoid process were used for the Latarjet-Bristow p
