Hermansky-Pudlak syndrome type 3 in Ashkenazi Jews and other non-Puerto Rican patients with hypopigmentation and platelet storage-pool deficiency.

Hermansky-Pudlak syndrome (HPS), consisting of oculocutaneous albinism and a bleeding diathesis due to the absence of platelet dense granules, displays extensive locus heterogeneity. HPS1 mutations cause HPS-1 disease, and ADTB3A mutations cause HPS-2 disease, which is known to involve abnormal intracellular vesicle formation. A third HPS-causing gene, HPS3, was recently identified on the basis of homozygosity mapping of a genetic isolate of HPS in central Puerto Rico. We now describe the clinical and molecular characteristics of eight patients with HPS-3 who are of non-Puerto Rican heritage.

Poisoning due to tricyclic antidepressant overdosage. Clinical presentation and treatment.

Tricyclic antidepressants are among the commonest causes of both non-fatal and fatal drug poisoning in the world. Their toxicity is due to effects on the brain, the heart, the respiratory system and the parasympathetic nervous system. Symptoms usually appear within 4 hours of an overdose and all but the most seriously poisoned patients recover within 24 hours. The most common clinical features are dry mouth, blurred vision, dilated pupils, sinus tachycardia, pyramidal neurological signs, and drowsiness. In severe poisoning, there may be coma, convulsions, respiratory depression, hypotension an

Accommodative intraocular lens versus standard monofocal intraocular lens implantation in cataract surgery.

BACKGROUND: Following cataract surgery and intraocular lens (IOL) implantation, loss of accommodation or postoperative presbyopia occurs and remains a challenge. Standard monofocal IOLs correct only distance vision; patients require spectacles for near vision. Accommodative IOLs have been designed to overcome loss of accommodation after cataract surgery. OBJECTIVES: To define (a) the extent to which accommodative IOLs improve unaided near visual function, in comparison with monofocal IOLs; (b) the extent of compromise to unaided distance visual acuity; c) whether a higher rate of additional c

Revisiting the Molecular Architecture of Photoreceptor Ribbon Synapses Using Ultrastructure Expansion Microscopy.

PURPOSE: Immunofluorescence-based mapping of photoreceptor ribbon synapse nanostructure in extended formaldehyde-fixed specimens is challenging due to diffraction-limited resolution and fixation-associated loss of antigen accessibility that restrict reliable molecular localization and structural interpretation. To circumvent these physicochemical limitations, we tested whether ultrastructure expansion microscopy (U-ExM) could overcome these barriers and enable compartment-resolved structural and molecular analysis of photoreceptor ribbon synapses in archival canine retinal tissues. METHODS: C

Assessment of real-life visual outcomes and treatment efficacy of diabetic macular edema in patients with type 2 diabetes.

BACKGROUND: Diabetic macular edema (DME) is a major cause of vision loss in individuals with diabetes. This study evaluated the long-term visual prognosis of patients with type 2 diabetes (T2D) by assessing real-world DME treatment outcomes. METHODS: Patients with T2D and DME at Oulu University Hospital during 2010-2023 were included. The effect of DME intervention (anti-VEGF-agents, macular laser or both, intravitreal corticosteroids, observation) was evaluated by changes in visual acuity (VA) and residual edema. The dataset comprised additional variables, including age, sex, age at T2D diag

Objective Assessment of Ideomotor Limb Apraxia Using Markerless Computer Vision and Machine Learning: A Proposed Framework and Validation Design.

Ideomotor limb apraxia is a higher-order disorder of learned, skilled movement that cannot be adequately explained by elementary weakness, sensory loss, ataxia, extrapyramidal dysfunction, impaired comprehension, or lack of cooperation. It is common after left hemisphere stroke and occurs in several neurodegenerative diseases. Quantitative kinematic study of apraxic movement is well established in the research literature, and standardized clinical instruments such as the Test of Upper Limb Apraxia and its bedside screen have improved the reliability of the examination. What remains missing is

Postoperative Visual Acuity Outcomes After Phacoemulsification in Patients With Type 2 Diabetes Mellitus: A Systematic Review.

Cataract surgery is commonly performed in people with type 2 diabetes mellitus (T2DM), but concerns persist regarding postoperative visual recovery even before the onset of diabetic retinopathy. Understanding outcomes in this population is essential for evidence-based counselling and perioperative management. Our objective in this systematic review was to evaluate postoperative visual acuity outcomes following phacoemulsification in adults with T2DM without diabetic retinopathy or diabetic macular oedema, compared with non-diabetic individuals. A systematic search of Web of Science, Scopus, an

End-of-life dreams and visions in a patient with delirium: A Brazilian case report and narrative review.

OBJECTIVES: End-of-life dreams and visions (ELDVs) and delirium frequently occur near death but differ in core features. Clinical differentiation becomes challenging when they co-occur. This case report illustrates the interplay between ELDVs and delirium, examines the limits of current diagnostic criteria in mixed cases, and outlines a nuanced approach to distinction. METHODS: We report the case of an elderly Brazilian woman with metastatic cancer who exhibited both ELDVs and delirium. Mental status was serially assessed using the Confusion Assessment Method (CAM). ELDV accounts were prospec

Age-Related Macular Degeneration and Circadian Preference.

PURPOSE: Age-related macular degeneration (AMD) is the leading cause of blindness in developed nations. Within the retina, a subset of cells, called melanopsin-containing intrinsically photosensitive retinal ganglion cells, are implicated in circadian rhythms, prompting a search for a potential connection between circadian behavior and AMD. Our objective was to compare the chronotype (ie, preference for morning or evening activity) of individuals with AMD to that of those without ocular conditions. PATIENTS AND METHODS: The Horne-Östberg Morningness-Eveningness questionnaire was administered

Influence of pseudoxanthoma elasticum on the lipid profile and prognostic implications.

Background: Pseudoxanthoma elasticum (PXE) is a rare, inherited disease characterised by specific skin lesions, progressive loss of vision and early onset atherosclerosis. Atherosclerosis in PXE leads to an increased rate of vascular occlusion and severe intermittent claudication. Although genetically determined, the individual course of PXE is highly variable. Up to now, there is no sufficient parameter to identify individuals at risk of rapid disease progression. This present study focused the lipid profile of patients with PXE and its possible influence on the clinical severity of periphera

Dermatologists' Knowledge and Attitude Toward Isotretinoin Ocular Side Effects in Egypt.

AIM: The FDA approved isotretinoin for moderate and severe nodulo-cystic acne, in which it proved effective. However, this misled the dermatologists into using isotretinoin for mild cases as well. This misuse increased the risk of developing adverse effects, especially on the ocular system. These adverse effects vary from mild reversible eye dryness to severe irreversible loss of night vision. Also, it causes contact lens intolerance and corneal ulcer on top. Both ophthalmologists and dermatologists have different perceptions of the drug. PURPOSE: To evaluate dermatologists' knowledge and att
