The association between New Zealand Blackcurrant anthocyanins and airway inflammation, including PAI-1, in asthma: A placebo-controlled crossover feasibility study.

Asthma is a common chronic inflammatory airway disease. Plasminogen activator inhibitor 1 (PAI-1) contributes to airway inflammation, remodelling, disease severity, and exacerbations. As anthocyanins (ACN) possess antioxidant and anti-inflammatory properties, and oxidative stress is a key feature of asthma, investigating the efficacy of anthocyanin supplementation in this population is warranted. Although the study was originally designed to assess variability to plan a larger study in the event only five participants were recruited. The study design was a cross-over design. Participants recei

The Use of Montelukast in Pediatric Asthma and Anti-Inflammatory Effects - Easy Choice or a Balancing Act?

Montelukast is a selective cysteinyl leukotriene receptor 1 antagonist with an established, though nuanced, role in the management of asthma and allergic rhinitis. By inhibiting leukotriene-mediated signaling, montelukast targets a pathway involved in bronchoconstriction, mucus secretion, vascular permeability, eosinophil recruitment, cytokine release, and airway remodeling. Available evidence indicates that montelukast may reduce several markers of allergic inflammation, including fractional exhaled nitric oxide, peripheral and tissue eosinophilia, eosinophil-derived mediators, adhesion molec

Microfluidic and MEMS-Based Biosensing Platforms for Fungal Respiratory Infections in Immunocompromised Patients: Toward Rapid, Specific, and Minimally Invasive Diagnosis.

Invasive fungal respiratory infections (IFRIs) remain a major cause of morbidity and mortality among immunocompromised patients, yet diagnosis continues to be hindered by nonspecific clinical features, limited sample accessibility, and the poor sensitivity or specificity of conventional tests. Microfluidic and microelectromechanical systems (MEMS)-based biosensing platforms have emerged as promising alternatives, enabling rapid, minimally invasive, and highly specific detection of fungal pathogens and host responses. Microfluidic nucleic acid and antigen assays allow on-chip amplification and

Noninvasive malaria detection beyond blood sampling.

Malaria remains a major global health challenge, particularly in sub-Saharan Africa, despite being one of the oldest documented human diseases. Although blood-based detection remains the mainstream diagnostic modality, its use is constrained by the need for trained personnel, sterile procedures, patient discomfort, and reduced sensitivity for low-parasitemia and asymptomatic infections. Recent advances in sensing and diagnostics, increasingly integrated with machine learning and artificial intelligence, have created new opportunities for noninvasive malaria detection using accessible biologica

Effects of Heart Rate-Synchronized Breathing on Anaerobic Threshold During Incremental Exercise Testing: A Preliminary Case Series.

Heart rate-synchronized breathing (HRSB) -- a technique in which the respiratory cycle is entrained to the cardiac cycle -- may optimize autonomic nervous function and ventilatory efficiency, potentially improving exercise tolerance. However, its effect on the anaerobic threshold (AT) remains unclear. This study examined the influence of HRSB on AT and related physiological indices during incremental cycle ergometry in healthy young adults. Three healthy male adults (mean age: 22.3 ± 1.2 years) underwent two cardiopulmonary exercise tests on separate days: Day 1 (normal breathing, control) and

Development of a clinical prediction model using volatile organic compounds as breath biomarkers in squamous oesophageal neoplasms: ViSON, protocol for a multicentre case-control study in UK NHS hospitals.

INTRODUCTION: Oesophageal squamous cell carcinoma (OSCC) accounts for more than 85% of the global burden of oesophageal cancer, resulting in 500 000 deaths annually. In the UK, over two-thirds of OSCC cases are diagnosed at an advanced stage, with a 5-year overall survival of less than 20%. Breath-based triage tests are feasible, acceptable and show proof of concept for earlier cancer detection. METHODS AND ANALYSIS: The Volatile Organic Compounds as Breath Biomarkers in Squamous Oesophageal Neoplasms (ViSON) study is a multicentre case-control study powered to estimate breath test performanc

Biomarkers for selecting biologic therapies in asthma.

Asthma is a heterogeneous chronic respiratory disease driven by diverse inflammatory pathways that vary across patients. The recognition of distinct molecular endotypes has led to the development of targeted biologic therapies that have transformed the management of moderate-to-severe asthma. However, selecting the most appropriate biologic therapy for each patient remains challenging. Health providers often rely on trial-and-error approaches that may delay disease control, increase costs, and expose patients to unnecessary side effects. Biomarkers are key to precision asthma care, as they pro

B cell activating factor (BAFF) levels are associated with failed remission after 12 months of treatment with benralizumab and mepolizumab in severe asthma.

The introduction of biologicals interfering with the interleukin-5 (IL-5) pathway has revolutionized the treatment of severe eosinophilic asthma (SEA), rendering remission an achievable goal even in patients with previously uncontrolled disease. This study investigated whether specific plasma biomarkers or clinical features could predict clinical remission in patients treated with an IL-5- (mepolizumab) or IL-5 receptor alpha-antibody (benralizumab) for severe eosinophilic asthma. Clinical remission, defined as sustained symptom control, absence of exacerbations, stable lung function, and no n

Quitting Smoking and Two-Year Survival After a Diagnosis of Small-Cell Lung Cancer.

BACKGROUND: Smoking at diagnosis is associated with worse survival in non-small-cell lung cancer, and quitting is independently associated with better 2-year survival. The effects of smoking status at baseline and smoking cessation on small-cell lung cancer (SCLC) outcomes are unclear. METHODS: In a subgroup of a large UK multicenter lung cancer study, we report outcomes at 2 years for 720 patients newly diagnosed with SCLC. All were offered smoking cessation advice and cancer treatments according to national guidelines. Smoking status was verified by exhaled carbon monoxide at all visits. Ka

Effects of Moderate-Intensity Aerobic Exercise on Clinical Symptoms and Physiological Outcomes in Young Adults with Persistent Allergic Rhinitis: A Randomized Controlled Trial.

Allergic Rhinitis (AR) is an IgE-mediated inflammatory disorder that impairs quality of life and systemic function. Following the 'one airway, one disease' paradigm, AR-related inflammation often extends to the lower respiratory tract. This randomized controlled trial investigated the effects of an 8-week moderate-intensity aerobic exercise (MOA) program on clinical symptoms, nasal airflow, airway inflammation, pulmonary function, and cardiorespiratory parameters in young adults with physician-confirmed persistent AR. To isolate the exercise effects, all participants discontinued antihistamine

Changes in systemic carbon dioxide levels during simulated avalanche burial using a one-way valve device: A randomized, double-blind, crossover trial.

INTRODUCTION: Mortality following complete avalanche burial remains high, primarily due to asphyxiation. Rebreathing-related CO₂ accumulation and oxygen depletion in the air pocket and surrounding snow are considered key mechanisms contributing to asphyxia. Artificial air pocket devices (AAPD) may mitigate this effect by separating exhaled from inhaled air. The present trial aimed to assess this effect in a simulated avalanche burial setting using continuous transcutaneous CO2 monitoring. METHODS: In this randomized, double-blind, crossover trial, 11 healthy participants (3 female) underwent
