Cliniconeuropathologic correlates of human immunodeficiency virus in the era of antiretroviral therapy.

The objective of this study was to examine the spectrum of human immunodeficiency virus (HIV) brain pathology and its clinical correlates in the antiretroviral era. We carried out a cross-sectional survey, analyzing prospective clinical and neuropathological data collected by the National NeuroAIDS Tissue Consortium (NNTC), comprising 589 brain samples from individuals with advanced HIV disease collected from 1999 onwards. We assessed gender, ethnicity/race, mode of transmission, age, year of death, nadir CD4, plasma viral load, last antiretroviral regimen, presence of parenchymal HIV brain pa

Factors determining serologic response to treatment in patients with syphilis.

BACKGROUND: The goal of this study was to describe the clinical and epidemiologic manifestations of a syphilis outbreak in downtown Madrid, Spain. Because human immunodeficiency virus (HIV)-positive patients may be at increased risk of serologic failure during syphilis treatment, analysis of factors determining the response to treatment was performed in a cohort of HIV-positive and HIV-negative patients with syphilis. METHODS: We performed a longitudinal, retrospective study of patients with syphilis who received the diagnosis at a university-affiliated hospital in Madrid from 2003 through 20

Characterization of sexually transmitted disease clinic patients with recent human immunodeficiency virus infection.

The serologic testing algorithm for recent human immunodeficiency virus (HIV) seroconversion (STARHS) distinguishes between recent acquisition of HIV infection (seroconversion, on average, in the past 129 days) or long-standing infection. STARHS was offered to sexually transmitted disease clinic patients to estimate HIV incidence and determine correlates of recent infection from October 1998 through December 1999. Of the 5227 patients tested, 116 (2.1%) were HIV infected, and 28 had recent infections. The incidence was highest among homosexual men (5.3%/year; 95% confidence interval [CI], 2.6%

Anal and cervical intraepithelial neoplasia: possible parallel.

This study evaluated prospectively the use of an endoscope in examination of the anal canal for the detection of premalignant lesions. All patients underwent endoscopy and anal epithelial biopsy; the biopsy samples were examined histologically and human papillomavirus (HPV) DNA hybridisation was done. No evidence of anal intraepithelial neoplasia (AIN) was found in 20 control patients with anal fissure or fistula. Of 82 patients with anal HPV infection, 23 had evidence of AIN. The prevalence of AIN was significantly higher among homosexual than among heterosexual men (17 of 28 vs 1 of 26) with

Alternative methods of reproduction. Effects on the child.

Artificial donor insemination (DI), surrogate parenting, in vitro fertilization, and embryo transfer are alternative methods of reproduction being utilized with increasing frequency. Technological developments are being studied in detail. Because of the secrecy inherent in the families involved, there has been little written describing the emotional and psychological well-being of the participants. Significant factors, such as unlimited pregnancies per donor, lack of recordkeeping of the genetic background of each insemination, need for psychological counseling of the parents, the impact of th

Victimization, smoking, and chronic physical health problems among sexual minority women.

BACKGROUND: Sexual minority women (SMW) have been shown to be at increased risk for abuse, smoking, and chronic physical health problems compared with heterosexual women. In the general population, abuse and smoking are associated with physical health problems. However, there has been little research on their associations among SMW. PURPOSE: The current study examined a mediational model of abuse, smoking, and self-reported physical health conditions in a national sample of SMW. METHODS: Participants (N = 1,224) were recruited via the Internet and completed measures of childhood trauma, adul

Changes in women's sexual risk behaviors after therapeutic community treatment.

This study examines sexual risk behaviors among 197 women mandated to substance abuse treatment in therapeutic communities. The women's risk behaviors after treatment are compared with their behaviors prior to treatment, and risk behaviors among those who completed treatment are compared with those who did not. The women had a high prevalence of sexual risk behaviors (i.e., multiple partners, unprotected sex, sex trade, not asking partners to use condoms, sex with injection drug users [IDUs]) and relationship violence and conflict before treatment and a significant decrease in unprotected sex,

Management of unprotected sexual encounters.

After "unprotected" sexual encounters, sexual history guides risk assessment and testing for sexually transmissible infections (STIs). Chlamydia trachomatis infection is the most prevalent bacterial STI. Sexually active young people (aged < 25 years) should have annual chlamydia testing. Opportunistic STI testing is indicated for population groups at increased risk of STI, including young people, gay and other homosexually active men, and Indigenous people. Gay and other homosexually active men should be regularly tested for HIV, syphilis, chlamydia and gonorrhoea. Indigenous people should be

HIV Drug Resistance Among Key Populations in Nigeria: Insights From the 2020 Integrated Biological and Behavioral Surveillance Survey.

OBJECTIVES: HIV drug resistance mutations (DRMs) undermine the effectiveness of antiretroviral therapy (ART) and can lead to treatment failure. This study aimed to characterize HIV drug resistance among key populations in Nigeria. DESIGN: A cross-sectional integrated biological and behavioral surveillance survey was conducted across 6 Nigerian geopolitical zones among female sex workers, men who have sex with men, people who inject drugs, and transgender individuals (August-December 2020). METHODS: Dried blood spot specimens were collected from 2309 participants, of whom 719 (31.1%) were HIV

HIV Risk Behaviors and the Use of Geosocial Networking Dating Apps among Men Who have Sex with Men and Transgender Women in Lima, Peru: a Cross-Sectional Study.

Geosocial networking dating apps (GSN apps) are an increasingly widespread technology used by populations throughout the world to facilitate sexual encounters. Studies from a variety of settings suggest a possible association between GSN app use and HIV risk behaviors, including among sexual and gender minority populations such as men who have sex with men (MSM) and transgender women (TW). However, it remains unclear to what extent GSN apps play a causal role. We explored the relationship between GSN app use and sexual risk behaviors among MSM and TW in Lima, Peru by analyzing data from a mult

Needs and Opportunities for Anal Cancer Prevention in Patients Engaged With PrEP Care: Development of Conceptual Model.

INTRODUCTION: Populations at risk for HIV infection-including gay, bisexual, and other men who have sex with men (GBM) and transgender/gender diverse people (TGD)-are at disproportionate risk for anal cancer. Most anal cancers are caused by human papillomavirus (HPV) and are preventable with HPV vaccination and screening. Engaging at-risk populations who are already receiving HIV preventive care (eg, pre-exposure prophylaxis [PrEP]) may be an effective implementation strategy. The purpose of this study was to (1) identify the information, motivation, and behavioral skills that influence decisi
