Metabolic complications associated with use of diuretics.

Diuretics are commonly used therapeutic agents that act to inhibit sodium transport systems along the length of the renal tubule. The most effective diuretics are inhibitors of sodium chloride transport in the thick ascending limb of Henle. Loop diuretics mobilize large amounts of sodium chloride and water and produce a copious diuresis with a sharp reduction of extracellular fluid volume. As the site of action of diuretics moves downstream (thiazide and potassium-sparing diuretics), their effectiveness declines because the transport systems they inhibit have low transport capacity. Depending

Renal function in gout: long-term treatment effects of febuxostat.

BACKGROUND: The association between hyperuricemia, gout, and impaired renal function has long been recognized. Recent data provide evidence for the causal relationship between elevated serum urate (sUA) and renal changes, leading to declines in glomerular filtration rates. In healthy adults, glomerular filtration rate wanes with age. Urate-lowering therapy (ULT) with allopurinol has been shown to stabilize or reverse this. OBJECTIVE: Here we examine the long-term effects of ULT with febuxostat on estimated glomerular filtration rate (eGFR). METHODS: This is a post hoc analysis of the Febuxos…

Association of serum uric acid with cardiovascular disease in rheumatoid arthritis.

OBJECTIVES: Elevated serum uric acid (SUA) levels have been associated with cardiovascular disease (CVD) in the general population. Rheumatoid arthritis (RA) is not thought to associate with high SUA but is characterized by increased CVD morbidity and mortality. We aimed to explore a potential association of SUA with CVD in RA patients and to evaluate whether such an association is present when the traditional CVD risk factors are taken into account. METHODS: . 400 consecutive RA patients were recruited in this cross-sectional study and had all traditional CVD risk factors and SUA assessed. T

Crystal-associated arthritis in the elderly.

Since the original recognition of these conditions in 1961, a great deal has been learned about the pathogenesis, clinical manifestations, and appropriate treatment of gout and pseudogout, and the role of crystals in osteoarthritis has been further defined. The variable manifestations of crystal-induced arthritis in elderly populations has led to a greater need for proper diagnosis and treatment strategies for these increasingly common forms of arthritis.

[Allopurinol-induced chronic granulomatous interstitial nephritis].

Although drug induced interstitial nephritis is a relatively common cause of renal failure, granulomatous forms remain a rare condition. The development of a chronic granulomatous interstitial nephritis due to allopurinol is exceptional, only three cases have been described previously. We report on a patient who presented a granulomatous interstitial nephritis after 10 years of allopurinol administration (300 mg/day). At diagnosis, he had end stage renal disease and dialysis treatment was needed. Two months after drug withdrawal and on corticoid treatment a slow recovery of renal function was o

The elevated prevalence of apolipoprotein E2 in patients with gout is associated with reduced renal excretion of urates.

OBJECTIVE: Previous studies have demonstrated that the lower renal excretion of urates in patients with hyperuricaemia is inversely related to plasma very low-density lipoprotein (VLDL) levels, and the different genotypes of the apolipoprotein E gene are related to the plasma levels of lipids. The aim of this study was to determine the prevalence of apolipoprotein E in hyperuricaemic patients and to investigate whether the renal excretion of urates is conditioned by the apoliprotein E genotype. METHOD: The plasma levels of lipoproteins, cholesterol, triglycerides and uric acid, and the renal

Novel frameshift variant in the β subunit of epithelial sodium channels uncovers Liddle syndrome in a young patient with metabolic syndrome: a case report with review of literature.

Liddle syndrome is a rare monogenic form of hypertension that can be misdiagnosed as essential hypertension or, in the context of obesity, be mistaken for metabolic syndrome. This case report describes a 19-year-old Chinese male with obesity, hypertension, insulin resistance, impaired glucose tolerance, dyslipidemia, and hyperuricemia-a classic presentation of metabolic syndrome. However, the early onset of severe hypertension and a strong family history prompted further investigation. Laboratory evaluation revealed mild hypokalemia, suppressed plasma renin activity and normal aldosterone leve…

Functional validation of a non-canonical HNF1B splice-site variant in MODY5.

BACKGROUND: Mutations in the HNF1B gene can cause developmental and functional abnormalities in multiple organs such as the pancreas, kidneys, and liver. The patient is a 19-year-old male with normal growth and development, presenting with a 9-year history of diabetes that remains poorly controlled despite intensive subcutaneous insulin therapy. His clinical features include surgically managed bilateral cataracts, hyperuricemia, and bilateral renal cysts. Family history is significant for diabetes in paternal and maternal uncles. METHODS: This combination of early onset, multiorgan involvemen…

Polydatin alleviates adenine-induced nephropathy in mice.

BACKGROUND: Adenine-induced nephropathy (AIN) serves as a classic experimental model for studying uric acid-related renal injury, characterized by tubular obstruction, inflammatory cell infiltration, tubular epithelial cell apoptosis, and progressive interstitial fibrosis, among others. These pathological changes are primarily driven by intrarenal deposition of adenine metabolites, and the model reliably recapitulates both hyperuricemia and impaired renal function. The aim of this study was to assess the renoprotective potential of polydatin (PD) in a murine model of AIN. METHODS: Fifty male …

Exosome-Mediated Intercellular Communication in the Pathological Processes of Gouty Arthritis and Its Treatment.

Gouty arthritis (GA) is a debilitating autoinflammatory disorder precipitated by the deposition of monosodium urate (MSU) crystals, leading to intense, recurrent joint inflammation and systemic metabolic dysregulation. While hyperuricemia is a prerequisite, the transition to clinical gout involves complex intercellular signaling cascades that are not fully understood. Emerging evidence has identified exosomes,- nanoscale extracellular vesicles, -as critical mediators in this pathological process. Exosomes function as intercellular carriers, transporting a diverse cargo of bioactive molecules, …

Development and clinical validation of photochemical biosensors for monitoring hemoglobin, blood lipids and uric acid in plateau areas.

High-altitude regions are prone to plateau erythrocytosis due to unique geo-climatic conditions such as low oxygen, high altitude, and low temperatures, with significantly higher incidences of hyperlipidemia and hyperuricemia compared to lowland areas. However, the extreme environmental conditions at high altitudes and the elevated hematocrit levels in blood samples from these populations present significant challenges to the applicability of existing point-of-care testing (POCT) devices. This study describes the development, early clinical validation, and potential clinical impact of three po…

Ganglion cysts are soft tissue swellings occurring most commonly in the hand or wrist. They are typically filled with gelatinous material ri … presentation of a ganglion cyst containing uric acid crystals in the absence of gout or significant hyperuricemia. We propose that the mucopolysaccharide-rich microenvironment of the cyst may promote crystal precipitation independent of systemic metabolic disease. Exemestane-related tissue changes may also play a role, though further investigation is warranted. …

Left-Sided Perforated Appendicitis in an Elderly Man with Intestinal Malrotation: A Case Report.

BACKGROUND Left-sided acute appendicitis is rare and usually reflects congenital anomalies that misplace the cecum and appendix. Although type I intestinal malrotation (non-rotation) seldom remains undiagnosed into late adulthood, it can complicate diagnosis in older patients, whose comorbidities suggest more common causes of left lower-quadrant pain. CASE REPORT A 67-year-old man with hypertension, mixed dyslipidemia, hyperuricemia, and chronic kidney disease had 48 hours of migrating colicky abdominal pain, localizing to the left flank and iliac fossa, with fever and leukocytosis. Contrast-e…
