[Chronic cutaneous infiltration with abscess and fistula formation. A type of clinical course in atypical mycobacteriosis].

BACKGROUND: Cutaneous lesions of atypical mycobacteriosis are very rare in Western Europe. Both the slow disease progression and the rather unspecific clinical appearance of the chronic infiltration render cutaneous mycobacteriosis a diagnostic challenge. In this communication, we present an uncommon, biphasic course of a cutaneous atypical mycobacteriosis. CASE REPORT: A 3-year-old child was referred to our institution due to submandibular cutaneous infiltration together with subcutaneous abscess formation. There was no carious lesion or other odontogenic potential source of infection. First

Periodic fever, aphthous stomatitis, pharyngitis, and adenopathy syndrome: clinical characteristics and outcome.

We report 28 patients (20 male) with a syndrome characterized by abrupt onset of fever, malaise, aphthous stomatitis, tonsillitis, pharyngitis, and cervical adenopathy (PFAPA syndrome). Episodes of fever occurred at intervals of 5.1 +/- 1.3 weeks beginning at the age of 4.2 +/- 2.7 years. Fever, malaise, tonsillitis with negative throat cultures, and cervical adenopathy were reported in all 28 patients, aphthae in 19, headache in 5, abdominal pain in 5, and arthralgia in 3. Mild hepatosplenomegaly was observed in 6 patients. Mild leukocytosis, elevation of the erythrocyte sedimentation rate, a

[Hepatic and splenic micro-abscess in cat scratch disease. Report of a case].

Cat-scratch disease (CSD) is an infection caused by a gram-negative bacillus known as Bartonella Henselae. Hepatosplenic disease occurs in only 0.3-0.7% of patients. In this report we describe a 7-year-old male presented with a 4-week history of fever, after diagnosis of CSD with regional lymphoadenitis. Ultrasonography and tomography identified hepatic and splenic abscesses. Antibiotic treatment for three months was associated with resolution of lesions. In Patients affected by CSD, ultrasonography and tomography permit to identify hepatic and/or splenic lesions, indicating systemic CSD.

[The contribution of ultrasonography to various pathologies of the small intestine].

Ultrasonography has progressively become an easily available non expensive imaging modality for the study of the small intestine. It has no secondary effects and is comforting for the abdominal radiologist. Further, as ultrasonography does not require special preparation nor contrast administration in the intestine or vascular system, no delay is caused if other imaging modalities (such as CT or angiography) have to be performed. This report covers the most common pathologic conditions in the small bowel and does not consider enteroliths, foreign bodies nor mucoviscidosis.

Atypical coccidioidomycosis in an AIDS patient successfully treated with fluconazole.

Cervical lymphadenopathy due to Coccidioides immitis occurred as the sole opportunistic infection in a Spanish patient with the acquired immunodeficiency syndrome. Twelve years earlier the patient had lived in the desert regions of the southwestern USA. After an initial course of high doses of fluconazole, the patient recovered without any sequelae. This is the first case of coccidioidomycosis in Spain.

Inflammatory torticollis in children.

Acute torticollis is commonly seen in the pediatric emergency department. It often results from an inflammatory process that irritates the cervical muscles, nerves, or vertebrae. Posturing of the head occurs with unilateral spasm of the sternocleidomastoid muscle such that the child will position the head with the occiput rotated to the affected side and the chin rotated to the contralateral side. We recently treated 26 children who presented to the emergency department with acute nontraumatic torticollis. The most common causes were upper respiratory infection, sinusitis, otomastoiditis, cerv

[Frequency of perilymphadenitis in regional lymph nodes in carcinoma].

Intensity, frequency and extension of an inflammatory reaction of the lymph node capsule (perilymphadenitis) were investigated in nodes draining invasive ductal breast cancer and infiltrating adenocarcinoma of the large bowel. Occurrence of chronic perilymphadenitis was significantly higher in paracolic lymph nodes than in the axillary ones (p less than 0.05). Both paracolic and axillary lymph nodes developed perilymphadenitis more frequently when they were infiltrated by carcinoma (p less than 0.001). Frequency and extension of the inflammatory reaction of the capsule depended on the degree o…

[Diseases caused by atypical mycobacterial].

Infections having atypical mycobacteria are occurring more frequently for 2 main reasons. First there are the disturbances of the immunity system, whether due to the acquired immune deficiency syndrome (AIDS) or the result of therapeutic measures taken against malignant processes, other diseases or after organ transplantation. Second, travel and free-time activities increase the risk of an infection. In many cases such infections become localized on the skin, but can also develop changes in the lungs, depending on the patient's occupation. For this reason we provide a review of the new literat

Cutaneous lymphomas: correlation of histochemical and immunohistochemical characteristics and clinicopathologic features.

Twenty-four cases of cutaneous lymphomas were classified as T-cell (18 cases), B-cell (three cases), and true histiocytic (three cases), on the basis of the histochemical and immunohistochemical characteristics. Important differences in clinical and histopathologic features exist among these three types: skin lesions of T-cell lymphoma are usually chronic, pruritic, and sometimes ulcerative; those of B-cell lymphoma are nonpruritic and nonulcerative; lesions of true histiocytic lymphoma are often pruritic and ulcerative. All three patients with true histiocytic lymphoma died within six months

Surprises in operations on the inguinal area in young children.

In surgical operations in the inguinal area in infants and children many unusual pathologic states were observed that were at first thought to be simple hernia. Among the conditions observed, in addition to complicated hernias and other anomalies of the processus vaginalis, were male pseudo-hermaphroditism, ectopic spleen, ectopic adrenal with neuroblastoma, diverticulum of the bladder, inguinal adenitis and suppurative iliac adenitis. In light of the sometimes surprising contents of the hernia sac, good exposure and careful identification of all anatomic structures is mandatory.

Kikuchi-Fujimoto Disease in Pregnancy: A Rare Case With Atypical Presentation and Potential Viral Trigger.

Kikuchi-Fujimoto disease (KFD), or histiocytic necrotizing lymphadenitis, is a rare, self-limiting condition characterized by cervical lymphadenopathy, fever, and night sweats. Although KFD is most commonly reported in young Asian women, cases have emerged globally, including among non-Asian populations. The etiology remains unclear, with viral and autoimmune triggers, such as Epstein-Barr Virus (EBV) and systemic lupus erythematosus (SLE), being commonly proposed. Diagnosing KFD is challenging due to its overlapping symptoms with more severe conditions like tuberculosis, malignancies, and aut…

Kikuchi-Fujimoto-Like Renal Involvement in Autoimmune Disease.

INTRODUCTION: Kikuchi-Fujimoto Disease (KFD) is a rare, self-limiting disorder primarily involving lymph nodes and characterized by necrotiz … lymphohistiocytic inflammation with prominent karyorrhexis most commonly manifests as a necrotizing lymphadenitis or dermatitis in both KFD and SLE, our study reveals that this distinctive pattern may also be recognized as an unusual interstitial nephritis, most commonly associated with SLE and seen in biopsies with limited glomerular involvement.

Neuromyelitis optica spectrum disorder in an 11-year-old: a case report in a pediatric patient from Nepal.

INTRODUCTION AND IMPORTANCE: Neuromyelitis optica spectrum disorder (NMOSD) is an auto-immune inflammatory disorder of the central nervous system mediated by auto-antibodies against Aquaporin-4 (AQP4). It causes vision impairment, vomiting, and paralysis. CASE PRESENTATION: We report a case of an 11-year-old Nepalese girl who initially presented with persistent vomiting for 2 months, followed by progressive limb weakness and facial asymmetry. She was initially misdiagnosed as gastroduodenitis with mesenteric lymphadenitis and later with major depressive illness and dissociative motor disorder…
