Randomized Single Blind Sham Controlled Trial of Adjunctive Home-Based tDCS after rTMS for Mal De Debarquement Syndrome: Safety, Efficacy, and Participant Satisfaction Assessment.

BACKGROUND: Mal de debarquement syndrome is a medically refractory disorder characterized by chronic rocking dizziness that occurs after exposure to passive motion. Repetitive transcranial magnetic stimulation (rTMS) can acutely suppress the rocking dizziness but treatment options that extend the benefit of rTMS are needed. OBJECTIVES: 1) To determine whether transcranial direct current stimulation (tDCS) added after rTMS can extend the benefit of rTMS; 2) to determine whether participants can safely perform tDCS at home. METHODS: Participants were given five days of rTMS (1 Hz right DLPFC/1

Susceptibility to seasickness.

This paper explains part of the observed variability in passenger illness ratings aboard ships by gender, age and sickness history. Within the framework of a European project, 2840 questionnaires, gathered on several ships operating all over Europe, were analysed. Gender, age and sickness history all had a highly significant effect on seasickness. Furthermore, these effects could be characterized by two fixed parameters describing a general age effect, a third parameter dependent on sickness history and a fourth parameter dependent on gender. Female illness ratings peaked at an age of 11 years

Drop attacks secondary to superior canal dehiscence syndrome.

Two patients with unprovoked drop attacks were found to have dehiscence of the superior semicircular canal on CT of the temporal bone. Both had conductive hearing loss, preservation of stapedius reflex, and abnormal vestibular evoked myogenic potentials. Neither had sound- or pressure-induced nystagmus. Repair of the dehiscence in one case stopped the drop attacks, supporting a causal relationship between the dehiscence and the drop attacks.

Comparison of the neurokinin-1 antagonist GR205171, alone and in combination with the 5-HT3 antagonist ondansetron, hyoscine and placebo in the prevention of motion-induced nausea in man.

AIMS: In man a neurokinin-1 (NK1) receptor antagonist has previously been shown to be ineffective in the prevention of motion-induced nausea. The antiemetic efficacy of NK1 receptor antagonists against chemotherapy-induced emesis is, however, enhanced when combined with a 5-HT3 receptor antagonist. Hence the efficacy of the NK1 antagonist GR205171 in combination with the 5-HT3 antagonist ondansetron (Zofrantrade mark) was assessed in motion-induced nausea. METHODS: GR205171 25 mg i.v., with and without concomitant administration of ondansetron 8 mg i.v., and hyoscine hydrobromide 0. 6 mg oral

Dramatic favorable responses of children with learning disabilities or dyslexia and attention deficit disorder to antimotion sickness medications: four case reports.

Responses of four learning disabled children who showed dramatic improvements to one or more antimotion-sickness-antihistamines and -stimulants are described qualitatively. These cases were selected from a prior quantitative study in which three antihistamines (meclizine, cyclizine, dimenhydrinate) and three stimulants (pemoline, methylphenidate, dextroamphetamine) were tested in variable combinations (using a specific clinical method) for favorable responses by 100 children characterized by diagnostic evidence of learning disabilities and cerebellar-vestibular dysfunctioning. Impact of Display Technology and Head-Tracking Latency on the Perceived Quality of a Driving Simulation in Virtual Reality.

ABSTRACT: BACKGROUND: Given the current lack of evidence comparing head-mounted display (HMD) and projection-based simulators and with the overall goal of developing a driving simulator for use in patients with eye diseases, the aim of this study was to determine the effects of display technology and latency on perceived driving simulation quality and simulator sickness. ABSTRACT: PATIENTS AND METHODS: Participants with a valid driving license, normal or corrected-to-normal vision, and no known visual disorders were asked to perform a 5-minute driving task on a driving simulator under three d

First metatarsophalangeal hemiarthroplasty for hallux rigidus.

There is a paucity of objective information in the literature about first metatarsophalangeal (MTP) hemiarthroplasty. The authors postulate that it is a reasonable treatment option for severe hallux rigidus in selected patients. Twenty-two elective first MTP hemiarthroplasties were performed on 20 patients that met the inclusion criteria. Pre- and postoperative evaluations were done using the American Orthopaedic Foot and Ankle Society (AOFAS) forefoot score, visual analogue scale (VAS) pain score, range of motion (ROM) measurements, and radiographs. Average ROM and dorsiflexion improved by 15

The effects of transdermal scopolamine on simulated ship navigation and attention/reaction time.

Transdermal scopolamine is commonly used by mariners to prevent or treat seasickness. Most studies indicate that scopolamine administered transdermally via an adhesive patch does not impair performance of skills required to navigate a vessel, but trials have not been conducted testing navigation and ship handling under realistic conditions. The aim of this study was to test the effects of transdermal scopolamine on performance using training simulators to assess complex vessel navigation and rough-weather ship handling abilities. A randomized double-blind crossover study assessed 32 Swedish ma

Coriolis-induced cutaneous blood flow increase in the forearm and calf.

Using venous occlusion plethysmography, Sunahara et al. reported that Coriolis-induced nausea was accompanied by an increase in forearm blood flow, suggesting a decrease in sympathetic activity to this vascular bed. No significant blood pressure and heart rate changes were observed. Vasodilation of the limbs theoretically impairs orthostatic tolerance, particularly if blood flow is shown to increase simultaneously in the lower limbs. This study examined the latter possibility. Seventeen subjects were exposed to the Coriolis cross-coupling effects induced by 20 RPM yaw rotation, and a simultane

[The effect of pyridoxine on the cerebral hemodynamics in vestibular disorders].

By means of hydrogen clearance on conscious rabbits with implanted platinum electrodes it was established that pyridoxine (1 and 10 mg/kg) used against the background of sea sickness decreased the dilatational reaction of the cerebral vessels occurring during the stimulation of the vestibular apparatus, reduced the blood supply to the cerebral hemispheres with insignificant changes of oxygen tension in the cortical structures, relieved acidosis and hypoxemia developing during sickness in the orthostatic position, increased oxygenation of the arterial blood without influencing significantly pat

Chronic subjective dizziness.

CONCLUSION: Chronic subjective dizziness (CSD) is frequent and affects twice as many women as men. Anxiety is a strong predisposing factor. The pathophysiologic concept of this disorder assumes that balance function and emotion share common neurologic pathways, which might explain that the balance disorder can provoke fear and vice versa, giving rise to a problem in perception of space and motion. In anxious patients this can turn into a space and motion phobia, with avoidance behaviour. OBJECTIVE: CSD is a diagnosis based on the hypothesis of an interaction between the vestibular system and

Origin of orientation-dependent asymmetries in vestibulo-ocular reflexes evoked by caloric stimulation.

A caloric stimulus evokes primarily a horizontal vestibulo-ocular reflex (VOR) when subjects are in a supine or prone orientation with the horizontal semicircular canal plane oriented vertically. In both monkeys and humans, the magnitude of VOR eye movements is greater in the supine than in the prone orientation, indicating that some factor or factors, other than the conventionally accepted convective stimulation of the horizontal canals, contributes to the generation of the VOR. We used long-duration caloric irrigations and mathematical models of canal-otolith interactions to investigate fact
