Conservative Management of Partial Thickness Rotator Cuff Tears: A Systematic Review.

BACKGROUND: Conservative management has emerged as an attractive option for partial thickness rotator cuff tears (PT-RCTs). A single algorithmic treatment strategy for patients with symptomatic PT-RCT has not yet been developed. This systematic review aims to ascertain whether a conservative approach to PT-RCTs yields positive results in terms of clinical outcomes and functional recovery. METHODS: This is a systematic review of the literature on patients with PT-RCTs receiving conservative treatment with physiotherapy, platelet-rich plasma (PRP) injections, collagen injections, hyaluronic aci

Superior Capsular Reconstruction Combined With Lower Trapezius Tendon Transfer Improves the Biomechanics in Posterosuperior Massive Rotator Cuff Tears.

BACKGROUND: Surgical treatments for chronic posterosuperior massive rotator cuff tear (MRCT) are still controversial. Superior capsular reconstruction (SCR), which provides a static stabilizer to decrease superior humeral head translation, and lower trapezius tendon transfer (LTTT) with centralization of the humeral head, which prevents superior humeral head migration, are potential surgical options. To date, SCR combined with LTTT has not been fully investigated. HYPOTHESIS: Restoration of static stabilizer and dynamic stabilizer together would effectively improve shoulder kinematics in post

Do patients with poor early clinical outcomes after reverse total shoulder arthroplasty ultimately improve?

BACKGROUND: Although most patients undergoing reverse total shoulder arthroplasty (rTSA) have substantial improvement in pain and function at early follow-up, improvements in pain and range of motion progress more slowly during postoperative rehabilitation in a subset of patients. The purpose of this study was to define a patient's risk of persistent shoulder dysfunction beyond the early postoperative period and identify risk factors for persistent poor performance. METHODS: We retrospectively reviewed 292 primary rTSAs with early poor performance and a preoperative diagnosis of osteoarthriti

Muscle degeneration in chronic massive rotator cuff tears of the shoulder: Addressing the real problem using a graphene matrix.

Massive rotator cuff tears (MRCTs) of the shoulder cause disability and pain among the adult population. In chronic injuries, the tendon retraction and subsequently the loss of mechanical load lead to muscle atrophy, fat accumulation, and fibrosis formation over time. The intrinsic repair mechanism of muscle and the successful repair of the torn tendon cannot reverse the muscle degeneration following MRCTs. To address these limitations, we developed an electroconductive matrix by incorporating graphene nanoplatelets (GnPs) into aligned poly(l-lactic acid) (PLLA) nanofibers. This study aimed to

Acromial and humeral head osteolysis following superior capsular reconstruction using autologous tensor fascia lata graft.

BACKGROUND: Graft tear is a critical complication following superior capsular reconstruction (SCR) as it directly links with clinical outcomes. No previous reports have described acromial and humeral head osteolysis following SCR. Acromial and humeral head osteolysis may incidentally occur after SCR using autologous tensor fascia lata graft. This study aimed to demonstrate the incidence of osteolysis following SCR using autologous tensor fascia lata graft and investigate the factors that affect osteolysis. METHODS: This retrospective cohort study included patients who underwent SCR for irrepa

Editorial Commentary: Optimizing Indications for Shoulder Superior Capsular Reconstruction: Choose the Right Patient at the Right Time!

Superior capsular reconstruction has gained popularity for the management of massive, irreparable rotator cuff tears in young patients with minimal glenohumeral arthritis. Short-term outcomes show significant improvements in pain and function. However, the failure rate has been reported to range from 3% to 36%, with higher failure rates in women and patients with subscapularis tears, a greater body mass index, lower preoperative forward flexion, a lower preoperative acromiohumeral distance, subscapularis atrophy, and advanced rotator cuff arthropathy. Inadequate restoration of the acromiohumer

Acromioplasty reduces critical shoulder angle in patients with rotator cuff tear.

Critical shoulder angle (CSA) is the angle between the superior and inferior bone margins of the glenoid and the most lateral border of the acromion and is potentially affected during a rotator cuff tear (RCT). Acromioplasty is generally performed to rectify the anatomy of the acromion during RCT repair surgery. However, limited information is available regarding the changes in the CSA after anterolateral acromioplasty. We hypothesized that CSA can be decreased after anterolateral acromioplasty. Data were retrospectively collected from 712 patients with RCTs and underwent arthroscopic rotator

Shoulder Hemiarthroplasty After Previous Pectoralis Major Transfer for Irreparable Subscapularis Tear: A Case Report.

CASE: A 30-year-old woman with an irreparable subscapularis tear presented with persistent painful anterior instability despite several open and arthroscopic stabilization procedures. A pectoralis major tendon transfer (PMTT) was performed. The patient subsequently developed progressive glenohumeral arthritis over the next 10 years, ultimately necessitating shoulder arthroplasty. CONCLUSIONS: PMTT provides valuable anterior soft-tissue reconstruction stabilization for subscapularis insufficiency in a multioperated shoulder. Ten years later, the transfer was found to be intact and managed like

The lower trapezius transfer: a systematic review of biomechanical data, techniques, and clinical outcomes.

BACKGROUND: Lower trapezius (LT) transfers were originally described to restore external rotation (ER) in the management of brachial plexus palsy; however, there is recent interest in the role of this transfer to restore shoulder function, specifically ER, in patients with a massive irreparable rotator cuff tear (RCT). The purpose of this systematic review is to summarize the current literature pertaining to LT transfers, including biomechanics, techniques, and clinical outcomes for patients with brachial plexus palsy and massive RCTs. METHODS: MEDLINE, EMBASE, and PubMed were searched for bi

An Unusual Presentation of a Massive Acromioclavicular Joint Ganglion Cyst Geyser Sign Secondary to Massive Rotator Cuff Tear and Cuff Arthropathy.

A Geyser sign is a rare manifestation of acromioclavicular joint (ACJ) ganglion cysts that are seen in patients with massive rotator cuff tears and arthritis of the ACJ. We present a case report of a 70-yr-old male who was presented to our hospital with a massive AC ganglion cyst measuring 7 × 8 cm with normal shoulder function tests. Imaging studies revealed a massive rotator cuff tear, advanced cuff arthropathy, degeneration of the ACJ, and a Geyser sign (seen with magnetic resonance imaging). The cyst was initially aspirated but recurred, so open debridement distal clavicle resection and ga

Glenohumeral osteoarthritis: frequency of underlying diagnoses and the role of arm dominance-a retrospective analysis in a community-based musculoskeletal practice.

Patients with shoulder pain are commonly seen in adult primary care, and are typically referred to musculoskeletal specialists when their symptoms are persistent. Rotator cuff disorders (RCD) most commonly underlie non-traumatic shoulder pain in adults, and glenohumeral osteoarthritis (GHOA) has been considered to be relatively uncommon (Nakagawa et al., J Shoulder Elb Surg 8:580-584, 1999). Our impression has been that GHOA affects significant numbers of patients, is less well studied, and may be under recognized. Among adults referred to a musculoskeletal practice for shoulder pain evaluatio

Latissimus dorsi tendon transfer for massive irreparable cuff tears: an anatomic study.

The purpose of this study was to describe landmarks for latissimus dorsi tendon transfer in massive irreparable tears of the rotator cuff. Nine pairs of embalmed cadaveric shoulders were dissected. Crucial structures were identified, and their relationship with various bony and soft-tissue reference points was documented. Identification of the dense fibrous band, at the confluence of the teres major and latissimus dorsi musculotendinous unit, facilitates localization of the radial nerve, which crosses deep to this landmark. The axillary nerve can be seen to exit the quadrangular space 27 mm me
