Septic complications associated with the use of peritoneal drains in liver trauma.

This study reviewed all 164 cases of liver trauma seen at the Charity Hospital of New Orleans from 1980 through 1984, in 12 of whom intra-abdominal abscesses formed. Thirty four per cent of the patients had no peritoneal drainage and an abscess rate of 1.8%, 18% had only closed suction drainage and 0% abscess rate, 15% had only open sump drainage and a rate of 8.3%, 14% had only open Penrose drainage with a rate of 8.7%, and 19% had a combination of both open Penrose and sump drainage with a rate of 22.5%. Certain findings or conditions were related to the development of postoperative sepsis.

In older adults, "mild" by GCS does not mean low risk: Multicenter study of 1,157 patients with mild TBI and acute subdural hematoma.

BACKGROUND: Acute traumatic subdural hematoma (SDH) is the most common intracranial injury in mild TBI (mTBI), particularly in older adults. However, the relationship of patient and brain computed tomography (CT) characteristics with outcomes remains poorly described in a contemporary cohort. This study aims to characterize outcomes in younger and older adults with mTBI and isolated acute SDH. METHODS: This was a 10-center, retrospective cohort study of adult traumas with emergency department Glasgow Coma Scale (GCS) 13 to 15 and acute traumatic SDH on initial brain CT ('22-'23). Trauma regis

Sleep problems six-months after continuous earthquake activity in a Greek island.

Natural disasters, such as earthquakes, are traumatic events causing both acute and enduring stress to affected individuals. Psychosocial consequences include posttraumatic stress disorder (PTSD), reactions of anxiety and depression and sleep problems. The island of Cephalonia in Ionian Sea is characterized by its high seismic activity. More specifically, this activity has been considered to be the highest of the Balkan Peninsula and Europe in general. In January and February of 2014 a long sequence of earthquakes, which included two main shocks of 6.1 Richter and 6.0 R, struck the island. The

Acute Intoxication With Alcohol Reduces Trauma-Induced Proinflammatory Response and Barrier Breakdown in the Lung via the Wnt/β-Catenin Signaling Pathway.

BACKGROUND: Trauma is the third leading cause of mortality worldwide. Upon admission, up to 50% of traumatized patients are acutely intoxicated with alcohol, which might lead to aberrant immune responses. An excessive and uncontrolled inflammatory response to injury is associated with damage to trauma-distant organs. We hypothesize that, along with inflammation-induced apoptosis, the activation of the Wnt/β-catenin signaling pathway would cause breakdown of the lung barrier and the development of lung injury after trauma. It remains unclear whether ethanol intoxication (EI) prior to trauma and

Correlation Between Gut Microbiota and Plasma Metabolites in a Mouse Model for Post-Traumatic Stress Disorder.

BACKGROUND: The gut microbiota and plasma metabolites have been shown to contribute to the etiology of post-traumatic stress disorder (PTSD). The relationship between the gut microbiome and plasma metabolome in PTSD is poorly understood. This study aims to integrate the gut microbiome data and plasma metabolome data to elucidate microbial-metabolite associations specific for PTSD in a mouse model. METHODS: A PTSD mouse model was induced by single prolonged stress and electric foot shock (SPS&S). We sequenced the gut microbiota composition by 16S rRNA gene sequencing and used ultra-high perfor

[HILLY: The silent injury experienced by families of intensive care patients].

Admission to the intensive care unit represents a profound psychological ordeal for patients' family members. Far beyond the initial shock related to the severity of the illness, the hospital experience exposes families to a cumulative burden of stress, including prolonged uncertainty, confrontation with medical technology, physical exhaustion, loneliness, opaque medical language, and a sense of helplessness. Numerous studies show that during the months following an ICU stay, up to 70% of relatives experience symptoms of anxiety, 35% symptoms of depression, and nearly one third symptoms consis

Causes and outcome of coma: A nationwide cross-sectional analysis.

BACKGROUND: The Neurocritical Care Society Curing Coma Campaign has renewed interest in coma management. Large-scale cross-sectional data on the relative frequency of coma etiologies and factors influencing survival remain limited. METHODS: We analyzed the National Inpatient Sample (NIS) database and extracted all cases of coma treated with mechanical ventilation (MV) between 2010 and 2019. A hierarchical classification system assigned each patient a primary coma etiology, prioritizing (1) major structural/cellular brain injuries, (2) laboratory-confirmed diagnoses, and (3) organ failure etio

Successful Management of Open Depressed Skull Fracture with Superior Sagittal Sinus Injury and Refractory Intracranial Hypertension.

Traumatic dural venous sinus injury is uncommon but potentially fatal because of massive hemorrhage at presentation or during surgery, and intracranial hypertension caused by impaired venous outflow. We report a case of a young woman with an open depressed skull fracture over the vertex caused by repeated blunt assault. Computed tomography revealed a large midline depressed fracture with bony fragments penetrating the superior sagittal sinus. On arrival, the patient was in hemorrhagic shock, with a semicoma status and right hemiparesis. Emergency surgery was performed for hemostasis and wound

CPR-induced vertebral fractures: a case report and systematic review of literature.

Cardiopulmonary resuscitation is a critical intervention for cardiac arrest, but its forceful chest compressions can lead to skeletal injuries, including rare vertebral fractures CASE PRESENTATION: This case report details a 78-year-old woman with a history of cardiac surgery and obesity who suffered a fatal cardiac arrest due to a DeBakey type II aortic dissection. Despite 30 minutes of advanced manual CPR, she succumbed to cardiogenic shock. Autopsy revealed an incidental transvertebral fracture of the 8th thoracic vertebra alongside multiple bilateral rib fractures, with no evidence of pre-

Efficacy and Safety of Repeated Radiofrequency Thermocoagulation on Trigeminal Neuralgia Patients.

BACKGROUND: Trigeminal neuralgia (TN) is a pain condition characterized by paroxysmal, electric shock-like facial pain, affecting one or more areas of the branches. Approximately 33%-50% of patients require invasive treatment. Radiofrequency thermocoagulation (RFTC) is an established method for managing drug-resistant and chronic TN. This study evaluates treatment response quality and complication rates in repeated RFTC procedures for TN patients. METHODS: In this retrospective analysis, patient records were extracted from the electronic medical records of Kuopio University Hospital, using th

The Effect of Environmental Hypothermia on Survival in Isolated Blunt Traumatic Brain Injury.

BACKGROUND: Environmental hypothermia increases mortality in patients with major trauma; however, the impact of exposure hypothermia on outcomes in isolated traumatic brain injury (TBI) is underexplored in literature. The aim of this study is to determine the relationship between environmental hypothermia and survival in patients with isolated blunt TBI. METHODS: We analyzed data from the Trauma Quality Improvement Program database. We included patients who were aged ≥15 years, had an abbreviated injury scale ≥1 for the head/neck body region, an arrival Glasgow Coma Scale of <14, an abbreviat

Successful Management of Battlefield Traumatic Cardiac Arrest Using the Abdominal Aortic and Junctional Tourniquet (AAJT): A Case Series.

The Russo-Ukrainian war's prolonged warfare, resource constraints, and extended evacuation times have forced significant adaptations in Ukraine's medical system - including technological advancements and strategic resource placement. This study examined if the Abdominal Aortic and Junctional Tourniquet - Stabilized (AAJT-S) could manage traumatic cardiac arrest (TCA) at forward surgical stabilization sites (FSSS) as an adjunct to damage control surgery. Six patients in severe hypovolemic shock presented at an FSSS during fighting in Bakhmut (July 2022) and Slovyansk (May 2023). Following TCA d

Delayed Hemothorax Due to Rupture of a Right Inferior Phrenic Artery Pseudoaneurysm 67 Days After Rib Fractures: A Case Report.

Delayed hemothorax after blunt chest trauma usually develops within seven days, and the inferior phrenic artery (IPA) is a rare culprit vessel. We report a case of massive hemothorax that occurred 67 days after rib fractures due to rupture of an IPA pseudoaneurysm. A 73-year-old man was transported in shock with a five-day history of right hypochondrial pain. Non-contrast CT revealed a right pleural effusion with attenuation consistent with blood and multiple fractures of the right 8th-12th ribs, while contrast-enhanced CT demonstrated contrast extravasation adjacent to the right 10th rib. rTMS mechanisms for posttraumatic stress disorder treatment in a mouse model.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a psychiatric disease that may follow traumatic exposure. Current treatments fail in about 30% of patients. Although repeated transcranial magnetic stimulation (rTMS) applied to the prefrontal cortex has been shown to be effective in the treatment of PTSD, the mechanisms need further investigation. OBJECTIVE: Using a PTSD animal model, we verify the beneficial effect of rTMS, and explore the changes it induces on two putative PTSD mechanisms, GABA/glutamate neurotransmission and neuroinflammation. METHODS: PTSD-like symptoms were elicited i

Hominis Placenta modulates PTSD-like behaviors in SPSS-induced PTSD mice: Regulating energy metabolism and neuronal activity.

The symptoms of post-traumatic stress disorder (PTSD) include re-experiencing trauma, avoidance behaviors, negative alterations in cognition and mood. However, the underlying molecular mechanisms are unclear. Dysfunction of hypothalamic-pituitary-adrenal axis (HPA-axis) and dysregulation of glutamatergic and GABAergic systems were shown during PTSD. Therefore, regulating hormonal change or glutamate energy metabolism are considered as a therapeutic approach to alleviate this condition. Herbal medicine may be effective in treating PTSD due to its ability to target multiple underlying mechanisms
