[Endoscopic surgery in neoplasms of Vater's ampulla].

Twenty patients with tumors located in the ampulla of Vater underwent an endoscopic papillo-sphincterectomy. Four of them were palliative procedures and 16 were curative. The patients complaints were: obstructive jaundice, pain on the right hypochondrium, digestive hemorrhage or neoplastic syndrome. The mean age was 72.6 years ranging from 54 to 87. Ten were males and 10 females. The overall mean survival was 36.2 months (S.E.+ - 5.6) ranging between 9 and 50 months. The actuarial survival 18 months after the procedure was 63.5% (S.E.+ - 13%). The survival correlated with the macroscopic shape

Minor Papillotomy for Treatment of Idiopathic Acute Pancreatitis With Pancreas Divisum: A Randomized Clinical Trial.

IMPORTANCE: Pancreas divisum is implicated as an obstructive cause for acute pancreatitis. Observational data suggest endoscopic retrograde cholangiopancreatography (ERCP) with minor papillotomy reduces the risk of pancreatitis episodes. Even though this endoscopic procedure is widely used in practice, clinical trials are lacking. OBJECTIVE: To determine whether ERCP with minor papillotomy reduces the risk of acute pancreatitis among adults with unexplained acute recurrent pancreatitis and pancreas divisum. DESIGN, SETTING, AND PARTICIPANTS: This multicenter, sham-controlled, double-blind ra

Pancreatic stent improves the success rate of needle-knife papillotomy in patients with difficult biliary cannulation.

BACKGROUND: Needle-knife precut papillotomy (NKP) is typically performed freehand. However, it remains unclear whether pancreatic stent (PS) placement can improve the outcomes of NKP. AIM: To explore whether PS placement improves the success rate of NKP in patients with difficult biliary cannulation. METHODS: This single-center retrospective study included 190 patients who underwent NKP between January 2017 and December 2021 after failed conventional biliary cannulation. In cases with incidental pancreatic duct cannulation during conventional biliary cannulation, the decision for pre-NKP PS

The clinical significance of bile duct sludge: is it different from bile duct stones?

BACKGROUND: Some patients with suspected common bile duct (CBD) stones are found to have sludge and no stones. Although sludge in the gallbladder is a precursor of gallbladder stones, the significance of bile duct sludge (BDS) is poorly defined. This study aimed to compare BDS with bile duct stones in terms of frequency, associated risk factors, and clinical outcome after endoscopic therapy. METHODS: The study enrolled 228 patients who underwent therapeutic endoscopic retrograde cholangiopancreatography (ERCP) for suspected choledocholithiasis. The patients were divided into two groups: patie

Endoscopic stenting in the management of biliary stones.

INTRODUCTION: Endoscopic sphincterotomy and stenting are established modes of treatment in the management of bile duct stones and cholangitis. It is the first choice of treatment when the patients are elderly and have other medical conditions rendering them unfit for surgery. AIMS: We studied the immediate and long-term outcome of endoscopic stenting in patients who underwent Endoscopic Retrograde Cholangiopancreatography (ERCP) for ductal stones. PATIENTS AND METHODS: From January 1990 to December 1992, 366 patients had ERCP done for biliary stone disease. Fifty-five of these patients recei…

[Obstructive jaundice caused by residual and/or recurrent choledochal lithiasis: endoscopic and surgical treatment].

Results of surgical and endoscopic treatment in 79 patients (58 females and 21 males, age 40-89, mean 62), affected with jaundice, due to secondary choledochal lithiasis, are reported. Thirty-four patients (43%) underwent surgical operation: choledochotomy and T-tube 12, choledocho-jejunostomy 4, choledochoduodenostomy 3, papillosphincteroplasty 15. In 45 patients (57%) EST has been performed. Comparison of results has been obtained after 30 days and after 3-105 months (mean 49). Optimal results have been achieved in 94.9% of patients: 100% after surgery and 94.1% after EST. Morbidity and mort

Key independent predictors of PEP included endoscopic sphincterotomy (EST), previous pancreatitis, age, operative time, and intubation difficulty. Management of common bile duct stones: a comprehensive review.

CBD stones impose significant morbidity and cost. This review compares the efficacy and safety of endoscopic sphincterotomy (EST) and laparoscopic common bile duct exploration (LCBDE) and summarizes emerging techniques and remaining controversies. While EST remains a cornerstone for rapid stone extraction, particularly in high-risk surgical candidates, its association with post-procedural pancreatitis and long-term sphincter dysfunction underscores the need for judicious patient selection. Conversely, LCBDE emerges as a compelling alternative, offering superior complete stone clearance rates f…

CONCLUSIONS: Post-ERCP cholangitis is a relevant complication of endoscopic treatment and is associated with the presence of non-anastomotic strictures and higher treatment failure rates. Since the success rate of endoscopic intervention …

Management of chronic anal fissures: should fissurectomy be performed with botulinum toxin injection?

BACKGROUND: Despite the superior success rate of lateral internal sphincterotomy for managing chronic anal fissure (CAF), there is a trend towards sphincter-preserving treatment due to the increased risk of incontinence. Botulinum toxin (BT) and fissurectomy are two sphincter-preserving options for CAF. We aim to assess if combining fissurectomy with botox treatment is superior to botulinum toxin alone in the management of CAF. METHODS: This retrospective cohort study was conducted across two Sydney hospitals over 7 years. All patients with a CAF managed with either BT and fissurectomy (group…
