Prevention of Group B Streptococcal Early-Onset Disease in Newborns: ACOG Committee Opinion, Number 797.

Group B streptococcus (GBS) is the leading cause of newborn infection. The primary risk factor for neonatal GBS early-onset disease (EOD) is maternal colonization of the genitourinary and gastrointestinal tracts. Approximately 50% of women who are colonized with GBS will transmit the bacteria to their newborns. Vertical transmission usually occurs during labor or after rupture of membranes. In the absence of intrapartum antibiotic prophylaxis, 1-2% of those newborns will develop GBS EOD. Other risk factors include gestational age of less than 37 weeks, very low birth weight, prolonged rupture

Multicenter Evaluation of NeuMoDx Group B Streptococcus Assay on the NeuMoDx 288 Molecular System.

Group B Streptococcus (GBS) is the leading cause of neonatal sepsis and meningitis in developed countries. Recommendations for antepartum GBS detection include enriched culture with several options for identifying GBS, some of which are time-consuming. To reduce the time for identification and determination of the maternal GBS colonization status, rapid nucleic acid amplification technologies have been developed and commercialized. For rapid detection of GBS, a three-site clinical study was conducted to evaluate the NeuMoDx GBS assay, a real-time PCR test performed for vaginal/rectal swab spec

Early-onset neonatal infections in Australia and New Zealand, 2002-2012.

BACKGROUND: The epidemiology of early-onset neonatal sepsis (EONS) varies over time, and requires regular surveillance. OBJECTIVE: To analyse data on EONS in Australia and New Zealand. METHODS: Retrospective analysis of data collected longitudinally from multiple neonatal units from 2002 to 2012. RESULTS: Of 386 423 live births, 454 infants had EONS. The incidence rate of EONS was 1.20 per 1000 live births in 2002 and 0.83 in 2012, decreasing by 4% per year (95% CI 1% to 7%, p=0.007). Group B streptococcus (GBS) (37%) and Escherichia coli (25%) were the most prevalent organisms. The early-o

Group B streptococcal colonization and transmission dynamics in pregnant women and their newborns in Nigeria: implications for prevention strategies.

OBJECTIVES: Because few studies have been conducted on group B Streptococcus (GBS) in Nigeria, we sought to estimate GBS colonization and transmission frequencies for 500 women and their newborns and identify risk factors for both outcomes. METHODS: GBS strains were characterized for antibiotic susceptibilities, capsule (cps) genotype, pilus island profile and multilocus sequence type (ST). RESULTS: In all, 171 (34.2%) mothers and 95 (19.0%) of their newborns were colonized with GBS; the vertical transmission rate was 48.5%. One newborn developed early-onset disease, yielding an incidence of

Intrapartum antibiotic exposure for group B Streptococcus treatment did not increase penicillin allergy in children.

BACKGROUND: Group B Streptococcus (GBS) is the leading infectious cause of neonatal morbidity and mortality in the United States. Intrapartum administration of antibiotics to mothers with positivity to GBS is performed for prevention, with penicillin being the drug of choice. Previous studies have noted an increase in atopic diseases other than drug allergy associated with intrapartum antibiotic exposure. OBJECTIVE: To determine whether intrapartum exposure to penicillin for GBS increases the likelihood of penicillin allergy in children. METHODS: Retrospective chart review was performed for

Epidemiology of Group B streptococcus isolated from pregnant women in Beijing, China.

Group B Streptococcus (GBS) was the main causative organism of invasive infections in newborns due to vertical transmission from the colonized mothers. The study was undertaken to determine colonization rate, serotype distribution, genotypic characterization, antibiotic susceptibility profiles and molecular characteristics of erythromycin-resistant strains of GBS in pregnant women in Beijing, China. Vaginal-rectal swabs were collected from a total of 2850 pregnant women at 35-37 weeks of gestation, in which 7.1% were GBS positive. Serotypes III, Ia and V predominated. All isolates were penicil

Duration of intrapartum antibiotics for group B streptococcus on the diagnosis of clinical neonatal sepsis.

BACKGROUND: Infants born to mothers who are colonized with group B streptococcus (GBS) but received <4 hours of intrapartum antibiotic prophylaxis (IAP) are at-risk for presenting later with sepsis. We assessed if <4 hours of maternal IAP for GBS are associated with an increased incidence of clinical neonatal sepsis. MATERIALS AND METHODS: A retrospective cohort study of women-infant dyads undergoing IAP for GBS at ≥37-week gestation who presented in labor from January 1, 2003 through December 31, 2007 was performed. Infants diagnosed with clinical sepsis by the duration of maternal IAP recei

Genetic heterogeneity in pbp genes among clinically isolated group B Streptococci with reduced penicillin susceptibility.

The recent emergence of group B streptococcal isolates exhibiting increased penicillin MICs at the Funabashi Municipal Medical Center and other hospitals in Japan prompted a comparative analysis of the penicillin-binding proteins (PBPs) from those strains with the PBPs from penicillin-susceptible strains comprising four neonatal invasive strains isolated from 1976 to 1988 and two recent isolates. The PBP sequences of the penicillin-susceptible strains were highly conserved, irrespective of their isolation date. Of six strains with reduced susceptibility to penicillin (penicillin MICs, 0.25 to

Deaths of early-onset, invasive sepsis in full-term infants in Miyazaki: nine cases from a regional population-based analysis from 1998-2006.

In a population-based study we investigated the early-onset, invasive sepsis caused by group B streptococcus (GBS) and non-GBS in the era of intrapartum antimicrobial prophylaxis. From 1998 to 2006, we had 387 perinatal deaths in 98,495 deliveries, and 9 full-term infants met the criteria of early-onset, invasive sepsis, in which microorganisms were proven from blood sampling. Of these cases, 4 involved GBS, 2 ampicillin-resistant Escherichia coli, 2 methicillin-resistant Staphylococcus aureus, and 1 was unidentified. All 4 cases of GBS related death underwent improper procedures contrary to t

[Abdominal aortic-iliac thrombosis as a complication of newborn's bacterial sepsis].

A case of Group B Streptococcus sepsis is reported in a male newborn with a renal vascular hypertension. An abdominal Doppler examination revealed aortic thrombosis extending from renal to the common iliac arteries. Prolonged broad spectrum antibiotic therapy and fibrinolytic treatment combined with surgical thrombectomy were used, at last obtaining improvement of general patient condition. The infant's hypertension resolved and reperfusion of the right kidney and urine output resumed. Ultrasound examination at 4th month showed presence of collateral circulation to arteries providing right low

[Group B streptococcus colonization during pregnancy and prevention of early onset of disease].

Group B Streptococcus (GBS) is the most frequent cause of early onset of neonatal sepsis. Case-fatality rate is 6-20% for newborns. Neurological sequel occurs in 30% of survivors. In 1996, the Centers for Disease Control and Prevention (CDC), the American College of Obstetricians and Gynecologists, and the American Academy of Pediatrics recommended that obstetrics providers should adopt either a culture-based or a risk-based approach for the prevention of this disease. The aim of this prospective study was to determine the colonization rate of GBS in our population of pregnant women between Ju

Serotypes and antimicrobial susceptibility of group B Streptococcus from pregnant women in Mexico.

TITLE: Serotypes and antimicrobial susceptibility of group B Streptococcus isolated from pregnant women in México BACKGROUND: Group B streptococci (SGB) are a leading cause of neonatal sepsis and meningitis. Women colonized might also develop pregnancy-associated infections including urinary tract infection, bacteremia and postpartum endometritis. There are nine different capsular types, and with the possibility of a GBS vaccine in the future; it is essential to understand which of these types are circulating. We determined the serotype distribution of isolated rectovaginal and urine GBS obta

Change of antimicrobial susceptibility of group B streptococci over 15 years in Japan.

We compared antimicrobial susceptibility patterns of 206 group B streptococcal (GBS) strains isolated from pregnant women and six from neonates/infants with invasive infection during the two periods 1985-1986 and 1999-2000. All strains in both periods were susceptible to the penicillins, cephalosporins and carbapenem tested. Seven (3%) isolates were resistant to erythromycin and three (1%) were resistant to clindamycin. There were no significant differences between the two study periods in the incidence of GBS resistant to the 14 antibiotics tested. These results showed that penicillins are st

Inducible nitric oxide synthase and the effect of aminoguanidine in experimental neonatal meningitis.

This study explored the role of inducible nitric oxide (NO) synthase (iNOS) in an infant rat model of group B streptococcal meningitis. Brain iNOS activity increased during meningitis (P < .001), and iNOS was detected by immunocytochemistry in the walls of meningeal vessels and cells of the cerebrospinal fluid (CSF) inflammation. Animals treated with iNOS inhibitor aminoguanidine (AG; 130 mg/kg every 8 h) had reduced NO production (P < .05), higher CSF bacterial titers (P < .05), and increased incidence of seizures (P < .01) compared with untreated infected animals. AG also increased areas of

Lung hyaluronan decreases during group B streptococcal pneumonia in neonatal piglets.

Neonatal Group B streptococcus (GBS) sepsis and pneumonia result in lung injury and remain a major cause of morbidity and mortality in the newborn. Increased lung hyaluronan (HA) content is an important component of the lung's early response to damage in diseases such as adult respiratory distress syndrome (ARDS), infant respiratory distress syndrome (IRDS), and bleomycin-induced fibrosis. It is known, however, that GBS virulence factors include specific secretory enzymes such as hyaluronidase, an enzyme which breaks down HA. We therefore hypothesized that in lobar GBS pneumonia, lung HA would
