Schedules for home visits in the early postpartum period.

BACKGROUND: Maternal complications, including psychological/mental health problems and neonatal morbidity, have commonly been observed in the postpartum period. Home visits by health professionals or lay supporters in the weeks following birth may prevent health problems from becoming chronic, with long-term effects. This is an update of a review last published in 2017. OBJECTIVES: The primary objective of this review is to assess the effects of different home-visiting schedules on maternal and newborn mortality during the early postpartum period. The review focuses on the frequency of home v

Sleep quality is associated with vasopressin methylation in pregnant and postpartum women with a history of psychosocial stress.

BACKGROUND: The relationship between disturbed sleep and stress is well-documented. Sleep disorders and stress are highly prevalent during the perinatal period, and both are known to contribute to a number of adverse maternal and foetal outcomes. Arginine vasopressin (AVP) is a hormone and a neuropeptide that is involved in stress response, social bonding and circadian regulation of the sleep-wake cycle. Whether the AVP system is involved in regulation of stress response and sleep quality in the context of the perinatal mental health is currently unknown. The objective of the present study was

Modified Single-Side-Lying Quadratus Lumborum Block Combined with Liposomal Bupivacaine versus Transversus Abdominis Plane Block for Post-Cesarean Analgesia: A Randomized Controlled Trial.

BACKGROUND: Inadequate post-cesarean analgesia remains a common clinical issue, associated with increased opioid use and risks of chronic pain and postpartum depression. Although the anterior quadratus lumborum block (QLB III) is more effective than transversus abdominis plane (TAP) block, it requires repeated positioning and may cause quadriceps numbness. To address these limitations, we designed a modified QLB to overcome these drawbacks. This study evaluated its analgesic efficacy and safety when combined with liposomal bupivacaine in cesarean delivery. METHODS: This single-center, randomi

Peripartum Management of Refractory Graves' Thyrotoxicosis.

Graves' disease (GD) is the most common cause of hyperthyroidism, characterized by autoimmune stimulation of the thyroid gland and typically managed with antithyroid drugs (ATDs), radioiodine (RAI) therapy, or surgery. While most patients respond to standard antithyroid therapy, a minority exhibit refractory thyrotoxicosis, posing significant clinical challenges. Pregnancy further complicates management due to immunological shifts, increased risk of adverse maternal and fetal outcomes, and limitations in therapeutic options. We present the case of a 39-year-old woman with severe, persistent Gr

Striae gravidarum in the Han Chinese pregnant population: identifying genetic markers and risk factors through a prospective cohort study.

BACKGROUND: Striae gravidarum (SG), commonly known as stretch marks, are a frequent connective tissue alteration observed in pregnant women. The presence of SG can negatively impact postpartum women's self-perception, potentially resulting in diminished self-esteem and psychological issues, including anxiety and depression. The study aimed to evaluate the potential risk factors and genetic associations of SG within a Chinese Han population. METHODS: A multicenter trial was conducted involving 1,017 pregnant women of Chinese Han descent who provided informed consent. Participants completed que

Effect of Informative Cesarean Delivery Operative Steps Video on Maternal Anxiety Level: A Randomized Controlled Trial.

BACKGROUND: Cesarean section is the most common obstetric procedure performed. This can lead to maternal anxiety, which is a significant contributor to postpartum depression. This can adversely affect pregnant women both mentally and emotionally, negatively impacting their well-being and family bonding. The aim of this study was to evaluate the effect of the addition of an informative cesarean section operative steps video on the maternal anxiety score compared with standard pre-cesarean section care. METHODS: This randomized controlled trial was conducted at Thammasat University Hospital, Th

Childhood adversity, social support, and telomere length among perinatal women.

Adverse perinatal health outcomes are heightened among women with psychosocial risk factors, including childhood adversity and a lack of social support. Biological aging could be one pathway by which such outcomes occur. However, data examining links between psychosocial factors and indicators of biological aging among perinatal women are limited. The current study examined the associations of childhood socioeconomic status (SES), childhood trauma, and current social support with telomere length in peripheral blood mononuclear cells (PBMCs) in a sample of 81 women assessed in early, mid, and l

Correlates of Abuse Around the Time of Pregnancy: Results from a National Survey of Canadian Women.

OBJECTIVES: Although several studies have examined risk factors associated with abuse during pregnancy or postpartum periods, many used clinic-based or small regional samples, and few were national or population-based, limiting their generalizability and clinical utility. The purpose of this study was to describe the correlates of abuse around the time of pregnancy among a nationally representative sample of women in Canada. METHODS: Using data from 6421 postpartum women (weighted n = 76,500) who completed the Canadian Maternity Experiences Survey from 10/2006 to 01/2007, we explored the asso

What are the Risks of Contact Sport During Pregnancy: a Survey-Based Investigation.

BACKGROUND: Current guidelines recommend avoiding activities with the risk of contact during pregnancy, despite a lack of empirical data to support this recommendation. As a result, individuals who participate in contact and collision sports such as football or rugby are often confronted with difficult decisions and, in the absence of clear guidance, may resort to making choices based on personal experience, limited advice, or fear. OBJECTIVE: We aimed to examine the impact of continued participation in contact sport during pregnancy on maternal and fetal health outcomes. METHODS: We conduct

The association between preterm birth and postpartum mental healthcare utilization among California birthing people.

BACKGROUND: While mental health conditions such as postpartum depression are common, little is known about how mental healthcare utilization varies after term versus preterm delivery. OBJECTIVE: This study aimed to determine whether preterm birth is associated with postpartum inpatient and emergency mental healthcare utilization. STUDY DESIGN: The study sample was obtained from a database of live-born neonates delivered in California between the years of 2011 and 2017. The sample included all people giving birth to singleton infants between the gestational age of 20 and 44 weeks. Preterm bir

Validation of a prediction model for postpartum hospital use in geographic contexts with greater rural representation.

BACKGROUND: The postpartum period is a critical window to address maternal health inequities. Black, Hispanic, Indigenous, and rural populations experience disproportionately high rates of postpartum morbidity and postpartum hospital use (PHU), defined as readmissions or emergency department (ED) visits after delivery. Delivery hospitalizations provide an opportunity for early identification of individuals at high risk of PHU, who may benefit from targeted interventions to prevent adverse outcomes. We previously developed a 30-day PHU prediction model using New York City (NYC) birth data (2016

Exposure to polybrominated diphenyl ether-47 increases the risk of post-partum depression.

INTRODUCTION: Post-partum depression (PPD) affects up to 19.1% of pregnancies and is associated with increased levels of proinflammatory cytokines, inflammation, and reductions in brain-derived neurotrophic factor (BDNF). Previous work by our team suggests that environmental toxins such as polybrominated diphenyl ethers (PBDEs) enhance placental inflammation and reduce BDNF production. Nearly, 100% of studied women in California have some level of exposure to these compounds due to extensive use of the flame retardants. High levels of exposure to PBDEs has been linked to increased risk of adve

Neural processing of infant and adult face emotion and maternal exposure to childhood maltreatment.

Face processing in mothers is linked to mother-infant social communication, which is critical for parenting and in turn for child development. Neuroimaging studies of child maltreatment-exposed (CME) mothers are sparse compared to studies of mothers with postpartum depression, which have suggested blunted amygdala reactivity to infant stimuli. We expected to see a similar pattern in CME mothers. Based on broader studies in trauma-exposed populations, we anticipated increased amygdala reactivity to negative adult face stimuli in a comparison task in CME mothers given heightened evaluation of po

Prevalence and course of anxiety disorders (and symptom levels) in men across the perinatal period: A systematic review.

BACKGROUND: Men's experiences of anxiety within the perinatal period can adversely impact themselves, their partner and infant. However, we know little about the prevalence and course of men's anxiety across the perinatal period. The current review is one of the first to systematically review the published literature. METHODS: Five databases (PubMed, PsycINFO, Cochrane, SCOPUS, and Web of Science) were searched to identify relevant papers published prior to April 2015. The literature search identified articles with data for expectant fathers (prenatal period) and/or fathers of an infant aged

Disasters and perinatal health:a systematic review.

BACKGROUND: The empirical literature on the effects of disaster on pregnancy and the postpartum period is limited. The objective of this review was to examine the existing evidence on the effect of disasters on perinatal health. METHODS: A systematic review was conducted by searching electronic databases (MEDLINE, EMBASE, Cinahl, PsycInfo), including literature on disasters and pregnancy outcomes (e.g., preterm birth, low birth weight, congenital anomalies), mental health, and child development. A total of 110 articles were identified, but many published reports were anecdotes or recommendati
