Surgical Techniques for Zenker's Diverticulum: A Comparative Analysis.

OBJECTIVE: To compare the different modalities for treatment of Zenker's diverticulum and the associated clinical outcomes. STUDY DESIGN: Case series with chart review. SETTING: Tertiary care hospital. SUBJECTS AND METHODS: Between 1995 and 2011, 164 patients underwent surgery for Zenker's diverticulum (stapler, n = 69; laser, n = 68; open, n = 27). Patient sociodemographics, medical comorbidities, pre- and postoperative subjective dysphagia and regurgitation score, complications, length of stay, time to oral intake, and recurrence were reviewed for each surgical modality. RESULTS: No stat

A clinicopathological study of serotonin of sigmoid colon mucosa in association with chronic symptoms in uncomplicated diverticulosis.

INTRODUCTION: Neurotransmitter imbalance is hypothesised as a pathogenetic mechanism in several bowel conditions. We previously reported increased 5-HT in the sigmoid mucosa of colon resected for complicated diverticular disease (DD). We aimed to identify if abnormal 5-HT expression is associated with symptoms of uncomplicated DD. METHODS: This was a prospective, comparative study and follow-up survey of symptoms. We examined the differences in 5-HT between DD patients and controls, as well as the presence of bowel symptoms at time of endoscopy and also 2 years later. Sigmoid biopsies were co

[Adenocarcinoma arising in Meckel's diverticulum: a case report and literature review].

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract. The presence of malignant lesions arising in the diverticulum is very rare, the most common malignant lesion being sarcoma, followed by carcinoid tumors, and less frequently by adenocarcinomas. We present the case of an 86-year-old man who developed acute abdomen. Surgery revealed a perforated Meckel's diverticulum. Histology identified a poorly-differentiated adenocarcinoma arising in Meckel's diverticulum. We provide a review of the literature.

Vacuum-assisted closure for the treatment of parastomal skin necrosis: a novel approach to an unusual complication. Report of a case.

Extensive peristomal skin necrosis is an unusual complication, which is difficult to manage because of frequent contamination from the stoma site. This wound leads to difficulty in applying the stoma appliance. This case report describes the successful use of the vacuum-assisted closure system in the management of a patient who developed peristomal skin necrosis after emergency Hartmann's procedure.

[Meckel's diverticulum: surgical complications].

A 20-year (1971-1990) retrospective follow-up of all patients with a Meckel diverticulum (MD) out of the Kantonsspital and St. Claraspital Basel is reported. There were 57 men and 34 women with a mean age of 46.7 years (14-89). 60.4% of the cases were occasional findings, 39.6% were symptomatic MD. During the same period of time 5000 appendectomies were performed (ratio diverticulectomy to appendectomy was 1:50). 38.5% of the resections of MD were done when an acute appendicitis was supposed. In one third of the cases the MD was responsible for the symptomatology. Histological findings were 13

The outcome of staged management of complicated diverticular disease of the sigmoid colon.

In a retrospective study, 83 consecutive patients with indication for a staged treatment of complicated diverticular disease were reviewed. There was a considerable overall morbidity rate following construction of the colostomy, colonic resection, and colostomy closure. However, only 58% underwent a resection of the diseased segment, and even fewer (46%) obtained an eventual restoration of the colon continuity. This was primarily due to an unacceptable high mortality rate when patients with general peritonitis were managed by a proximal colostomy and drainage only. Subsequently, advanced age a

Cancers of the large bowel: associated disorders in individuals.

The individual medical histories in the files of the Oxford Record Linkage Study for the years 1963 to 1967 were analyzed to ascertain any previously unsuspected associations between cancers of the large bowel and other diseases in individuals, and to quantify the relative risks of disorders already known to be associated. In males significant associations were shown between cancers of the large bowel and cancer of the prostate. In females, cancer of the colon was associated with breast cancer, and cancer of the rectum with a mixed group of genital cancers. The relative risk of colorectal canc
