Sliding the Medial Pin: A Safer Approach to Cross-pinning Humerus Supracondylar Fractures?

INTRODUCTION: Many methods have been described to minimize the risk of ulnar nerve injury during the insertion of a medial pin for the percutaneous pinning of pediatric supracondylar humerus fractures (SCHF). The most recent AAOS Clinical Practice Guidelines suggests that physicians might want to avoid the use of medial-entry pins due to considerations of potential injury to the ulnar nerve. However, there are circumstances whereby a cross pin configuration is required. These include cases where there is medial wall comminution or due to the obliquity of the fracture. In this study, we present

Functional Outcome of Unstable Intertrochanteric Femur Fracture Treated Surgically With Long Proximal Femoral Nail.

INTRODUCTION: As the world's population ages, hip fractures become more common. The global incidence of hip fractures will rise to a million per year by 2040. Intertrochanteric femoral fractures are common fracture in older people, typically caused by low-energy falls. These fractures are also classified based on their stability, with unstable fracture patterns posing more challenges for surgical therapy and potentially resulting in worse functional recovery. Surgical fixation is the most effective treatment for these fractures since it restores motion while reducing complications. Intertrocha

The analysis of the variables, affecting outcome in surgically treated tibia pilon fractured patients.

INTRODUCTION: Treatment of tibia pilon fractures is challenging. Outcome after tibia pilon fractures depend on multiple factors. Aim of this study is to evaluate the potential variables that are known to affect the outcome after tibia pilon fractures. PATIENTS AND METHODS: Forty patients with 42 tibia pilon fractures with a one-year follow-up, who had undergone surgical treatment between January 2007 and June 2011, were evaluated, retrospectively. Patients were divided into 3 groups regarding the choice of surgical treatment (Group A-22 patients treated with open reduction-internal fixation;

Circular external fixation for treatment of distal humerus fractures: case report.

High-energy blasts can lead to complex intra-articular distal humerus fractures with extensive soft tissue loss, and treatment is fraught with complications. We describe 2 patients with such injuries treated successfully with the use of aggressive wound management followed by distal humerus fracture stabilization with a circular external fixator. We chose this circular external fixator over the Ilizarov frame because of our experience and success in the treatment of open tibia fractures with negligible malalignment and high union rate. This circular external fixator allows for indirect reducti

Asymmetrical bilateral traumatic fracture dislocation of the hip: a report of two cases.

We report on two cases of simultaneous asymmetrical bilateral hip dislocation. Both patients were involved in car accidents. The first case is a 23-year-old man who had a bilateral hip dislocation, anterior on the right side and posterior on the left associated with bilateral femoral head fracture. The second case presented the same dislocations of the hips associated with acetabular fracture on the right side. Closed reduction of the hips was performed in both cases. In the first case the femoral head fragments was subsequently removed. In the second case internal fixation of the acetabular f

Volar fixation for dorsally displaced fractures of the distal radius: a preliminary report.

Using a volar approach to avoid the soft tissue problems associated with dorsal plating, we treated a consecutive series of 29 patients with 31 dorsally displaced, unstable distal radial fractures with a new fixed-angle internal fixation device. At a minimal follow-up time of 12 months the fractures had healed with highly satisfactory radiographic and functional results. The final volar tilt averaged 5 degrees; radial inclination, 21 degrees; radial shortening, 1 mm; and articular incongruity, 0 mm. Wrist motion at final follow-up examination averaged 59 degrees extension, 57 degrees flexion,

Union Rates of Talar Neck Fractures With Substantial Bone Defects Treated With Autograft.

BACKGROUND: This study evaluated the union rate of talar neck fractures with substantial bone defects treated acutely with autologous tibial bone graft during primary osteosynthesis. METHODS: A case series at a level 1 trauma center was performed to identify consecutive patients who underwent operative fixation of talar neck fracture with autograft (Current Procedural Terminology codes 28445 and 20902) between 2015 and 2018. "Substantial bone defect" was defined as a gap greater than 5 mm in the sagittal plane and greater than one-third of width of the talar neck in the coronal plane. Postope

Recovery Trajectories Following Complex Elbow Injuries and Radial Head Arthroplasty: A Longitudinal Study Over 8 Years.

PURPOSE: Radial head arthroplasty (RHA) is commonly performed to manage comminuted unreconstructible radial head fractures. Although the outcomes of RHA are often satisfactory, revisions are usually considered when pain intensity is higher than expected. Therefore, it is important to investigate the recovery trajectories of patients following RHA over an extended period and the characteristics that may lead to unfavorable outcomes. METHODS: The Patient-Rated Elbow Evaluation (PREE) was used to assess recovery in 94 patients at baseline (within 2-7 days after surgery); 3 and 6 months; and 1, 2

[Secondary treatment for undetected Essex-Lopresti lesion].

AIM: Essex-Lopresti lesion (ELL) consists of a radial head fracture in combination with a rupture of the interosseous membrane and a dislocation of the distal radioulnar joint. In the case of radial head resection proximal migration of the radius occurs. The aim of this retrospective study was to analyse the treatment and outcome for chronic ELL. METHOD: Altogether 6 patients with secondary ELL were detected and clinically reviewed. Therapy consisted of implantation of a silastic radial head prosthesis in 3 cases and ulna shortening in 2 other cases. One patient received both procedures. In t

Finger Pulp Infection Due to a Retained Cement Bead: A Report of a Rare Case.

Finger pulp infections commonly occur due to bacteria entering through wounds. Foreign body-related pulp infections are uncommon and can present atypically. In this rare case, infection occurred due to a retained cement bead, which was surgically implanted for treating a compound, comminuted distal phalanx fracture with bone loss. A 22-year-old woman presented with persistent swelling, dull pain, and discharge from the pulp of the right middle finger for three months, unresponsive to empirical antibiotic therapy. Examination revealed localized swelling, tenderness with a puncture wound, and pu

Reconstruction of the Quadriceps Extensor Mechanism with a Calcaneal Tendon-Bone Allograft in a Dog with a Resorbed Tibial Tuberosity Fracture.

A non-reducible tibial tuberosity fracture is a rare complication of tibial tuberosity transposition performed during correcting of medial patella luxation (MPL) in dogs. This condition severely disrupts the quadriceps extensor mechanism, leading to significant pelvic limb lameness. An 11-year-old, 1.8 kg spayed female Yorkshire Terrier sustained a comminuted left tibial tuberosity fracture during surgical correction of an MPL. Six months after surgery, the dog was markedly lame and unable to extend the left stifle. Radiographs revealed patella alta and resorption of the fragmented tibial tube

Intra-articular Migration of Broken Patellar Tension Band Wire: A Rare Case.

INTRODUCTION: Comminuted fracture of patella is most commonly treated by tension band wiring with cerclage wiring. Broken cerclage wiring after such surgery is not uncommon but intra-articular migration of broken cerclage wire is rarely known and described complication. CASE REPORT: A 51-year-old male patient of Indian origin which was treated, 15 months back for fracture patella with tension band wiring with cerclage wiring, presented with painful restriction of knee movements for the last 4 weeks. X-ray showed cerclage wire had broken at multiple sites with one broken piece migrated intra-a

Incidence and pattern of concurrent capitellum fracture associated with radial head and neck fractures.

BACKGROUND: Radial head and neck fracture is a common fracture around the elbow. It is mostly caused by axial or valgus loading after a fall in elbow extension and forearm pronation. When a radial head and neck fracture occurs, the capitellum collides with the radial head and experiences a shear force, leading to capitellum fracture. The primary aim of this study was to evaluate the incidence of concurrent capitellum fracture and analyze its patterns among patients treated for radial head and neck fractures based on computed tomography. Secondary aim is to determine factors associated with con

[Miniinvasive technologies in treatment of intraarticular splittered fractures of the proximal shoulder].

The algorithm of diagnostics, miniinvasive ostheosynthesis and postoperative rehabilitation of patients with the splittered fractures of caput femori was worked out. Treatment results of 24 patients were analyzed. All patients were treated with the use of the original apparatus of the external osteal fixation. The method allows early functional treatment and limb function restoration in cases, when classic immersinal ostheosynthesis is contraindicated.

Mistaken Endobronchial Placement of a Nasogastric Tube During Mandibular Fracture Surgery.

A 64-year-old male had an awake right nasal fiber-optic intubation with an endotracheal tube for open reduction and internal fixation of bilateral displaced mandibular fractures. After induction of anesthesia, an 18 Fr nasogastric tube (NGT) was inserted through the left nostril and was secured. The patient required high flow rates to deliver adequate tidal volumes with the ventilator. A chest x-ray done in the postanesthesia care unit revealed a malpositioned NGT in the left lower lobe bronchus, which was immediately removed. The patient was extubated on postoperative day 2. Various tradition
