The Weighting of Cues to Upright Following Stroke With and Without a History of Pushing.

OBJECTIVE: Perceived upright depends on three main factors: vision, graviception, and the internal representation of the long axis of the body. We assessed the relative contributions of these factors in individuals with sub-acute and chronic stroke and controls using a novel tool; the Oriented Character Recognition Test (OCHART). We also considered whether individuals who displayed active pushing or had a history of pushing behaviours had different weightings than those with no signs of pushing. METHOD: Three participants experienced a stroke 6 months prior: eight with a history of pushing. I

Superselective Thalamotomy in the Most Lateral Part of the Ventralis Intermedius Nucleus for Controlling Essential and Parkinsonian Tremor.

OBJECTIVE: The minimum and essential thalamic areas for reducing tremor were investigated in cases treated by superselective thalamotomy in the most lateral part of the ventralis intermedius nucleus (mlp-VIM). METHODS: Stereotactic superselective VIM thalamotomy with depth microrecording was performed in 21 patients with essential tremor (ET) and 15 patients with tremor-dominant Parkinson disease (PD). A very small and narrow (axial plane) therapeutic lesion was formed as a square on the sagittal plane and inverse V on the axial plane in the mlp-VIM, which covered the kinesthetic response are

Dizziness, Unsteadiness, Visual Disturbances, and Sensorimotor Control in Traumatic Neck Pain.

Synopsis There is considerable evidence to support the importance of cervical afferent dysfunction in the development of dizziness, unsteadiness, visual disturbances, altered balance, and altered eye and head movement control following neck trauma, especially in those with persistent symptoms. However, there are other possible causes for these symptoms, and secondary adaptive changes should also be considered in differential diagnosis. Understanding the nature of these symptoms and differential diagnosis of their potential origin is important for rehabilitation. In addition to symptoms, the ev

The use of Argus® II retinal prosthesis by blind subjects to achieve localisation and prehension of objects in 3-dimensional space.

BACKGROUND: The Argus® II retinal prosthesis system has entered mainstream treatment for patients blind from Retinitis Pigmentosa (RP). We set out to evaluate the use of this system by blind subjects to achieve object localisation and prehension in 3-dimensional space. METHODS: This is a single-centre, prospective, internally-controlled case series involving 5 blind RP subjects who received the Argus® II implant. The subjects were instructed to visually locate, reach and grasp (i.e. prehension) a small white cuboid object placed at random locations on a black worktop. A flashing LED beacon wa

[How acrophobia impairs visual exploration and gait].

The life-time prevalence of visual height intolerance is 28 % in the general population. More than 50 % of those affected suffer from an impairment of daily behavior and quality of life when confronted with heights. Simultaneous measurements of spontaneous eye and head movements of these subjects while looking from a balcony revealed that visual exploration of the surroundings was restricted compared to that of control subjects. Spontaneous head movements were severely diminished and saccadic eye movements were reduced. Gaze in space was preferably directed towards structures on the horizon. T

Reflex responses to combined hip and knee motion in human chronic spinal cord injury.

The relative contributions of hip and knee proprioceptors to the origination of extensor spasms were examined in 11 subjects with chronic spinal cord injury (SCI). Ramp and hold extension and combined hip and knee oscillation movements were imposed to the right leg while the ankle was held in a static position by a custom-designed robot. Isometric joint torques of the hip, knee, and ankle and surface electromyograms (EMGs) from seven leg muscles were recorded following controlled hip and knee extension. A stereotypical torque response consisting of hip flexion, knee extension, and ankle planta

Lateral ankle ligament anesthesia significantly alters single limb postural control.

Lateral ankle anesthesia has been used as a model to explore effects of ligament deafferentation related to ankle sprain on single limb postural control with conflicting results. Time-to-boundary (TTB) is a postural control measurement technique found to be sensitive in detecting subtle deficits in postural control in those with chronic ankle instability. The objective of this study was to determine the effects of lateral ankle ligament anesthesia on TTB measures of single limb postural control in healthy adults. Twenty-two healthy adults with no history of lower extremity injury within the pa

Plasticity of reflexes from the foot during locomotion after denervating ankle extensors in intact cats.

Although sensory feedback is important in regulating the timing and magnitude of muscle activity during locomotion few studies have evaluated how it changes after peripheral nerve lesions. To assess this, reflexes evoked by stimulating a nerve before and after denervating other nerves can be quantified to determine changes. The aim of this study was to investigate consequences of denervating ankle extensor muscles, the lateral gastrocnemius, and soleus (LGS) on reflexes from the plantar foot surface evoked by stimulating the tibialis (Tib) nerve. Three cats (n = 3) were trained to walk on a tr

Error correction strategies for motor behavior after unilateral brain damage: short-term motor learning processes.

In order to identify the mechanisms underlying motor impairments and motor learning following stroke-related brain damage, we analyzed correction strategies used by hemiparetic individuals to produce precise elbow flexion movements of the paretic arm and compared them to those of healthy individuals. Participants made rapid elbow flexion movements to a 6 degrees wide target and were instructed to correct movement errors as quickly as possible when a spring-like load was unexpectedly introduced. Angular positions and torques before correction were used to identify error patterns. Results showed

Steps toward recovery of function after hemilabyrinthectomy in frogs.

Removal of the labyrinthine organs on one side results in a number of severe postural and dynamic reflex deficits. Over time some of these behavioral deficits normalize again. At a chronic stage the brain of frogs exhibits a number of changes in vestibular and propriospinal circuits on the operated side that were studied in vitro. The onset of changes in the vestibular nuclear complex was delayed, became evident only after head posture had recovered by more than 50%, and was independent of the presence or absence of a degeneration of vestibular nerve afferent fibers. The time course of changes

A sustained hyper-g load as a tool to simulate space sickness.

In April 1989 the three European scientist astronauts of the D1 Spacelab Mission were exposed to a 1.5 hours +3G centrifuge run in supine position, resulting in a linear acceleration along the subjects' x-axis. Afterwards, severe motion sickness symptoms were provoked by head movements (Sickness Induced by Centrifugation: SIC). The astronauts mentioned close similarities with what they experienced in space during the D1-Spacelab Mission in 1985, where head movements also provoked motion sickness symptoms (Space Adaptation Syndrome: SAS). Moreover, the astronauts agreed that the rank order of t

Perception of spatial orientation in spasmodic torticollis. Part 2: The visual vertical.

Twenty-nine patients with idiopathic spasmodic torticollis (ST) and matched normal control subjects were asked to align a target line to perceived earth vertical [visual vertical (VV)]. Settings were made against a whole-field random-dot background that was either stationary or rotating around the line of sight, and subjects performed the task upright and lying horizontally on their sides. Normal subjects were tested both head upright and after assuming a voluntary head tilt. Patients with ST set the VV close to true upright with a minimal deviation toward tilt of the head in contrast to norma

Corticofugal projections to the vestibular nuclei in squirrel monkeys: further evidence of multiple cortical vestibular fields.

Single- and multiple-unit recordings were made from nerve cells located in the different nuclei of the brainstem vestibular nuclear complex (VNC) of anaesthetized squirrel monkeys (Saimiri sciureus) by conventional stereotaxic techniques. After neurons responding to semicircular canal stimulation in a yaw, roll, or pitch direction or to otholith stimulation were identified, small amounts of retrograde tracer substances were deposited at the recording sites. Up to three different tracers were administered to different parts of the VNC in the same animal (Fast Blue, HRP-WGA, and Rhodamine-dextra

Multiple representations of the body and input-output relationships in the agranular and granular cortex of the chronic awake guinea pig.

The organization of somatosensory input and the input-output relationships in regions of the agranular frontal cortex (AGr) and granular parietal cortex (Gr) were examined in the chronic awake guinea pig, using the combined technique of single-unit recording and intracortical microstimulation (ICMS). AGr, which was cytoarchitectonically subdivided into medial (AGrm) and lateral (AGrl) parts, also can be characterized on a functional basis. AGrl contains the head, forelimb, and most hindlimb representations; only a small number of hindlimb neurons are confined in AGrm. Different distributions o

Dual-Task Training Improves Shoulder Function and is Associated with Changes in Sensorimotor Network Connectivity After Arthroscopic Rotator Cuff Repair: A Randomized Controlled Trial.

BACKGROUND: Recovery after arthroscopic rotator cuff repair (ARCR) is influenced by pain, proprioception, sensorimotor control, and psychological factors in addition to tissue healing. Cognitive-motor dual-task training may improve attentional allocation and motor control, but its role in postoperative ARCR rehabilitation remains unclear. METHODS: In this randomized, parallel-group, controlled trial (Department of Sports Medicine, The First Affiliated Hospital of Fujian Medical University, China; January 2025-January 2026), 72 patients after ARCR were randomly assigned to a control group or a…
