Severe humeral erosion in a bedridden patient: a comprehensive intervention to prevent bone fracture.

This article reports a 1-yr prospective study related to a bedridden patient with rotator cuff tear arthropathy featuring severe erosion of the proximal part of the humerus. To prevent spontaneous humeral fracture, a comprehensive intervention was planned. This included drug therapy for osteoporosis and pain, nurse team training on patient handling techniques, and patient and caregiver education. After 1 yr, clinical and radiologic findings are stable.

Imaging of degenerative and posttraumatic disease in the shoulder joint with ultrasound.

This article reviews the examination technique of shoulder ultrasound, normal and abnormal ultrasound findings in acute (posttraumatic) and chronic (degenerative) lesions. Moreover, it reviews the effectiveness of ultrasound in relation to magnetic resonance (MR) imaging. Most authors report that full-thickness tears of the supraspinatus can reliably be diagnosed by ultrasound. However, the simple diagnosis of a full-thickness rotator cuff tear is no longer sufficient for surgical management. The precise localization and size of rotator cuff tears as well as the extent of muscle degeneration i

Advances in Shoulder Pain Imaging: A Narrative Review of Current Practice and Emerging Trends.

Shoulder pain is among the most frequent musculoskeletal complaints and remains a significant therapeutic challenge in clinical practice. A wide spectrum of conditions may contribute to this symptom, including rotator cuff tendinosis or tears, calcific tendinopathy, labral or capsuloligamentous injuries and degenerative changes of the glenohumeral joint. Accurate diagnosis requires an integrated approach that combines clinical history, physical examination, and imaging. However, variability in examination technique and interpretation often limits the reliability of clinical assessment alone. D

Humeral avulsion of the inferior glenohumeral ligament in college female volleyball players caused by repetitive microtrauma.

BACKGROUND: Humeral avulsion of the inferior glenohumeral ligament is a rare injury resulting from hyperabduction and external rotation, and it is most commonly seen with sports-related injuries, including those from volleyball. The anterior band of the inferior glenohumeral ligament is most commonly injured (93%), whereas the posterior band is infrequently injured. The axillary pouch humeral avulsion of the inferior glenohumeral ligament as a result of repetitive microtrauma has not been yet described in the English literature. HYPOTHESIS: Humeral avulsions of the inferior glenohumeral ligam

Isokinetic strength of the shoulder after repair of a torn rotator cuff.

The strength of the shoulder was measured isokinetically in forty patients who had undergone surgical repair of a torn rotator cuff. Thirty-three patients were tested six months postoperatively and twenty-four, after a year. There was an increase in strength between the two intervals (p less than 0.05) for abduction measured at 180 degrees per second, external rotation measured at 60 and 180 degrees per second, and flexion measured at 60 degrees per second. On the average, strength reached 80 per cent of normal in abduction and 90 per cent of normal in external rotation at one year postoperati

Role of transplanted bone marrow cells in development of rotator cuff muscle fatty degeneration in mice.

BACKGROUND: Rotator cuff muscle fatty degeneration after a chronic tendon tear is an irreversible pathologic change associated with poor clinical outcomes of tendon repair, and its exact pathogenesis remains unknown. We sought to investigate the role of transplanted bone marrow cells in the development of fatty degeneration, specifically in adipocyte accumulation, using a mouse model. METHODS: Fourteen mice were divided into 2 bone marrow chimeric animal groups: bone marrow transplantation (BMT) group and reverse BMT group. For the BMT group, C57BL/6J wild-type mice underwent whole body irrad

Predicting rotator cuff tears using data mining and Bayesian likelihood ratios.

OBJECTIVES: Rotator cuff tear is a common cause of shoulder diseases. Correct diagnosis of rotator cuff tears can save patients from further invasive, costly and painful tests. This study used predictive data mining and Bayesian theory to improve the accuracy of diagnosing rotator cuff tears by clinical examination alone. METHODS: In this retrospective study, 169 patients who had a preliminary diagnosis of rotator cuff tear on the basis of clinical evaluation followed by confirmatory MRI between 2007 and 2011 were identified. MRI was used as a reference standard to classify rotator cuff tears

Healing improvement after rotator cuff repair using gelatin-grafted poly(L-lactide) electrospun fibrous membranes.

BACKGROUND: Rotator cuff tears (RCTs) are a common cause of shoulder pain and disability in middle and older age. Despite improvements in the understanding of this disease process and advances in surgical treatment, rotator cuff (RC) repair failure rates remain high. Insufficient healing capacity is likely the main factor for failure of reconstruction. MATERIALS AND METHODS: We fabricated implantable biodegradable gelatin-grafted poly(L-lactide) (PLLA) fibrous membranes using electrospinning technology and evaluated them using in vitro cell proliferation assays. Then, we established chronic r

Cross-sectional area of the supraspinatus muscle after rotator cuff repair: an anatomic measure of outcome.

BACKGROUND: The change in the cross-sectional area of a repaired muscle, measured with use of magnetic resonance imaging (MRI), could be an indicator of recovery of muscle function. The aims of this study were to evaluate the change in the area of the supraspinatus muscle between the immediately postoperative and one-year postoperative MRIs and to identify factors associated with the change. METHODS: Eighty-eight patients with a full-thickness rotator cuff tear were included. MRI was performed three days and one year after surgery. Patients were classified into two groups according to whether
