First report of Acinetobacter spp. and Proteus mirabilis isolated from aborted ovine fetuses.

This report describes two cases involving the isolation of Acinetobacter spp. and Proteus mirabilis from aborted ovine fetuses in Northeastern Brazil, representing the first documented occurrence of this association worldwide. In case 1, Acinetobacter spp. was isolated from a hepatic abscess in a White Dorper fetus, accompanied by placental lesions. In case 2, A. baumannii and P. mirabilis were isolated from the brain and lungs of three Santa Inês fetuses, with pathological findings consistent with acute septicemia. In both cases, bacteria were identified through culture and biochemical testin

Predominantly cystic glioblastoma mimicking benign cystic lesions: A radiological diagnostic challenge.

Glioblastoma multiforme (GBM) is the most common and most aggressive primary malignant brain tumor in adults. Although it typically presents as a heterogeneously enhancing solid mass with central necrosis, predominantly cystic glioblastoma is an uncommon morphological variant that may mimic other cystic intracranial lesions and create diagnostic uncertainty. We report the case of a 58-year-old man who presented with headache and vomiting for 1 week's duration. Laboratory investigations, including complete blood count, erythrocyte sedimentation rate, C-reactive protein, and total white blood ce

Acrophialophora: A Comprehensive Review of Clinical Guidelines and Diagnosis.

Acrophialophora is a saprotrophic genus of fungi found in both temperate and tropical regions. The genus is comprised of 16 species, with the subspecies A. fusispora and A. levis necessitating the most clinical concern. Acrophialophora is an opportunistic pathogen with a broad range of clinical manifestations; the fungus has been implicated in cases of fungal keratitis, lung infection, and brain abscess. Acrophialophora infection is particularly of concern for immunocompromised patients, who often present with a more severe disease course involving disseminated infection and may not exhibit ty…

Purtscher-Like Retinopathy with Cardioembolic Stroke: Case Report and Literature Review.

Purtscher-like retinopathy (PUR) is a rare condition characterized by sudden vision loss with associated retinal white patches thought to be due to precapillary arteriolar occlusion. We present a case of PUR associated with a cardioembolic stroke in a patient following temporary cessation of anticoagulant therapy for a surgical procedure. Our patient presented with multiple risk factors for PUR and classic signs and symptoms including multiple peripapillary white retinal lesions near arterioles and sudden unilateral decrease in visual acuity. Optical coherence tomography showed inner retinal h

What is Behind Salmonella? Unusual Presentation in Two Pediatric Cases.

Salmonella infection is an international public health concern. Salmonella organisms are Gram-negative bacilli that belong to the family Enterobacteriaceae, and more than 2500 Salmonella serovars have been described. The most common clinical presentations of Salmonella infection are gastroenteritis, bacteremia, enteric fever, and chronic carrier state. Other rare extraintestinal infections include cellulitis, urinary tract infection, pneumonia, endocarditis, meningitis, brain abscess, and osteomyelitis. Salmonella species resistant to first-line treatment such as ampicillin, chloramphenicol, a…

Tumefactive multiple sclerosis masquerade as a central nervous system tumor: a case report.

INTRODUCTION: Tumefactive multiple sclerosis is a demyelinating disorder that appears tumor-like on MRI. To most physicians, diagnosing tumefactive MS by applying clinical, radiological, or laboratory examination like Cerebrospinal fluid (CSF) analysis, can be challenging and ultimately biopsy is necessary to confirm the diagnosis. CASE PRESENTATION: This paper reports a case of a 37-year-old woman who presented with progressive headache and a strong family history of cancer and was misdiagnosed as having a CNS glioma. After considering the MRI features, CSF analysis results and observing imp

The last ride of Henry II of France: orbital injury and a king's demise.

Jousting was a popular pastime for royalty in the Renaissance era. Injuries were common, and the eye was particularly at risk from the splinters of the wooden lance. On June 30, 1559, Henry II of France participated in a jousting tournament to celebrate two royal weddings. In the third match, Gabriel de Montgomery struck Henry on the right shoulder and the lance splintered, sending wooden shards into his face and right orbit. Despite being cared for by the prominent physicians Ambroise Paré and Andreas Vesalius, the king died 10 days later and was found to have a cerebral abscess. The wound wa

[Neonatal renal candidiasis: a case report].

UNLABELLED: Neonatal renal candidiasis is an increasingly common condition affecting predominantly premature infants receiving neonatal intensive care or term infants with urogenital tract anomalies. CASE REPORT: a female infant was born by cesarian section at 30 weeks' gestation to a mother whose pregnancy had been complicated by rupture of membranes for 5 days. The infant was admitted at birth, maternofetal bacterial infection was suspected, and intravenous antibiotics were prescribed. The patient developed sepsis caused by Klebsiella pneumoniae at the age of 13 days. She was referred to ou

[Relapsing polychondritis with idiopathic hypoparathyroidism].

A 49-year-old man who had been treated with vitamin D since idiopathic hypoparathyroidism (IPH) was diagnosed at age 15 was referred to our hospital because of progressive wheezing. The same condition was also diagnosed in his sister at age 19. Chest radiography and computed tomography (CT) showed diffuse tracheal stenosis to a diameter of 6-8 mm. Laryngoscopy revealed no abnormality in the patient's epiglottis or vocal cords. No intact parathyroid hormone was detected in his sera but a brain CT scan showed calcification of the basal ganglia, which was compatible with IPH. Steroid therapy reli

The spectrum of pulmonary infections in cancer patients.

The lung is a common site of infection in patients with cancer. The spectrum of pulmonary infection depends on the underlying immunologic deficit or deficits. In neutropenic patients, gram-negative bacterial infections predominate early, whereas fungal infections (Aspergillus, Zygomycetes, Fusarium species) are common if neutropenia persists. In patients with impaired cellular immunity, viral infections (cytomegalovirus, other herpes viruses) predominate and may coexist with bacterial (Legionella, Nocardia), mycobacterial, and fungal (Aspergillus, Histoplasma, etc.) infections. Pneumocystis ca

What is the infection risk of oesophageal dilatations?

UNLABELLED: Oesophageal dilatation is the most widely used treatment option for the management of oesophageal strictures. Complications include bleeding, a slight increase in body temperature, thoracic or abdominal pain, oesophageal perforation, brain abscess and bacteraemia. We performed a prospective study to evaluate the frequency of post-dilatation bacteraemia in nine patients subjected to a total of 50 dilatations. Bacteraemia was detected in 36 cases (72%), In all but three cases, however, it was transient and not associated with fever or other clinical complications. The organisms most …

Central nervous system infections in the immunocompromised host.

Central nervous system (CNS) infections in the immunocompromised host are predictable according to the arm of the immune defense which is most defective. A limited number of organisms with a tendency to cause CNS infections will take advantage of the different types of immune defects. Signs and symptoms of CNS infection are often quite subtle in the immunocompromised host because of the diminished inflammatory response. Serologic responses may not be as reliable as in a normal host. Aggressive diagnostic approaches are often necessary, including early brain biopsy and abscess drainage. Aggress

Rationale for the use of contrast media in MR imaging.

Understanding the rationale of contrast media use in MR imaging is based on knowledge of the blood brain barrier (BBB) and the pathophysiology of infectious agents attacking the central nervous system. This article focuses on the relationship between BBB structures and functions in healthy and unhealthy conditions and contrast media in patients with acquired immunodeficiency syndrome.

Neurologic Complications of Infective Endocarditis: A Joint Model for a Septic Thromboembolism and Inflammatory Small Vessel Disease.

OBJECTIVES: Embolic events from vegetations are commonly accepted as the main mechanism involved in neurologic complications of infective endocarditis. The pathophysiology may imply other phenomena, including vasculitis. We aimed to define the cerebral lesion spectrum in an infective endocarditis rat model. DESIGN: Experimental model of Staphylococcus aureus or Enterococcus faecalis infective endocarditis. Neurologic lesions observed in the infective endocarditis model were compared with three other conditions, namely bacteremia, nonbacterial thrombotic endocarditis, and healthy controls. SE
