Evaluating an in-home multicomponent cognitive behavioural programme to manage concerns about falls and associated activity avoidance in frail community-dwelling older people: Design of a randomised control trial [NCT01358032].

BACKGROUND: Concerns about falls are frequently reported by older people. These concerns can have serious consequences such as an increased risk of falls and the subsequent avoidance of activities. Previous studies have shown the effectiveness of a multicomponent group programme to reduce concerns about falls. However, owing to health problems older people may not be able to attend a group programme. Therefore, we adapted the group approach to an individual in-home programme. METHODS/DESIGN: A two-group randomised controlled trial has been developed to evaluate the in-home multicomponent cogn

A concept for major incident triage: full-scaled simulation feasibility study.

BACKGROUND: Efficient management of major incidents involves triage, treatment and transport. In the absence of a standardised interdisciplinary major incident management approach, the Norwegian Air Ambulance Foundation developed Interdisciplinary Emergency Service Cooperation Course (TAS). The TAS-program was established in 1998 and by 2009, approximately 15 500 emergency service professionals have participated in one of more than 500 no-cost courses. The TAS-triage concept is based on the established triage Sieve and Paediatric Triage Tape models but modified with slap-wrap reflective triage

Prophylactic versus selective antibiotics for term newborn infants of mothers with risk factors for neonatal infection.

BACKGROUND: Early onset bacterial infection is an important cause of morbidity and mortality in newborn infants. Various factors that increase the risk of neonatal infection have been identified. It is unclear whether asymptomatic newborn infants born to mothers with one or more of these risk factors should receive antibiotics prophylactically rather than selectively if only clinical or microbiological evidence of sepsis emerges. OBJECTIVES: To assess the effect of prophylactic versus selective antibiotic treatment for asymptomatic term neonates born to mothers with risk factors for neonatal

IBI303, a biosimilar to adalimumab, for the treatment of patients with ankylosing spondylitis in China: a randomised, double-blind, phase 3 equivalence trial.

BACKGROUND: China approved adalimumab for the treatment of ankylosing spondylitis in 2013. However, the cost of the standard dose regimen exceeds ¥15 000 (around US$2250) per month, which is well beyond affordability for most Chinese patients. No biosimilars of adalimumab are available in China; IBI303 is a monoclonal antibody against TNFα that is currently in development. This study aimed to assess the clinical equivalence of IBI303 to adalimumab in patients with ankylosing spondylitis. METHODS: This phase 3, multicenter, double-blind, parallel, randomised controlled equivalence trial was do

Elective freezing of embryos versus fresh embryo transfer in IVF: a multicentre randomized controlled trial in the UK (E-Freeze).

STUDY QUESTION: Does a policy of elective freezing of embryos, followed by frozen embryo transfer result in a higher healthy baby rate, after first embryo transfer, when compared with the current policy of transferring fresh embryos? SUMMARY ANSWER: This study, although limited by sample size, provides no evidence to support the adoption of a routine policy of elective freeze in preference to fresh embryo transfer in order to improve IVF effectiveness in obtaining a healthy baby. WHAT IS KNOWN ALREADY: The policy of freezing all embryos followed by frozen embryo transfer is associated with a

Board game-based education for HIV prevention among rural older adults: a quasi-experimental study.

BACKGROUND AND AIM: Population aging has increased the burden of HIV infection among older adults, particularly in rural areas where access to effective health education remains limited. Traditional educational approaches often fail to engage this population. Board game-based health education has emerged as a promising strategy to enhance learning engagement and knowledge retention. METHOD: A quasi-experimental study was conducted among 100 rural older adults in Hunan Province, China. Participants were allocated to either an intervention group (n = 50) receiving a board game-based HIV educati

Platelet-Rich Fibrin with Minocycline versus Arestin in Non-surgical Periodontal Therapy: A Split-Mouth Randomised Controlled Clinical Trial.

PURPOSE: Periodontitis is a chronic inflammatory disease and while non-surgical periodontal therapy (NSPT) remains the first choice of treatment, its capability to completely resolve inflammation may be limited. Arestin® (1 mg minocycline microsphere) is an established adjunct, whereas platelet-rich fibrin (PRF) has recently emerged as a promising local drug-delivery scaffold. This split-mouth randomised controlled clinical trial compared the clinical efficacy of adjunctive minocycline delivered via conventional Arestin® versus incorporation within a PRF scaffold for the treatment of 4-6 mm pe

Aid cuts, health financing fragility, and maladaptive resilience in Sierra Leone: a rapid mixed-methods study.

Sierra Leone's health system remains highly dependent on external aid, with donor funding accounting for approximately 47% of total health expenditure between 2020 and 2024, compared with a government contribution of around 25-26%. Recent reductions in development assistance for health, including a sharp in-year contraction of USAID-supported health programming during 2024-25 and an approximately 15% reduction in Global Fund allocations, have exposed major structural vulnerabilities in the health financing system. We conducted a rapid mixed-methods study to examine the effects of recent and an

Therapeutic potential of Jatimukuladi Varti Anjana in alkali burn-induced corneal injury and neovascularization in albino rats.

INTRODUCTION: Corneal transparency is maintained by its avascular nature, and disruption following injury can lead to corneal neovascularization (CNV), resulting in visual impairment. Although current treatment such as corticosteroids, NSAIDs, anti-VEGF agents, and surgical interventions are available, their use is often limited by adverse effects, recurrence, and cost. Jatimukuladdiv arti Anjana (JMV), a classical ayurvedic ophthalmic formulation with reported anti-inflammatory and tissue-healing properties, was evaluated for its therapeutic efficacy in an alkali burn-induced CNV model in alb

Machine and deep learning based on magnetic resonance imaging to segment glioblastoma and predict the spread of recurrence: a multicenter retrospective protocol.

BACKGROUND: Glioblastoma (GB) remains one of the most aggressive brain tumors, with limited survival and high recurrence rates. In most cases, GB recurrence occurs locally, either on the residual tumor after surgery or within 2 cm of the resection cavity-but in rarer cases, tumor cells can spread beyond this margin, leading to distant recurrence. By spread, we refer to the spatial dissemination of tumor cells beyond the typical local site, which can involve distant brain regions or even the leptomeninges, significantly impacting treatment planning and surgical decision-making. This study propo

MoveMentor-examining the effectiveness of a machine learning and app-based digital assistant to increase physical activity in adults: protocol for a randomised controlled trial.

BACKGROUND: Physical inactivity is prevalent, leading to a high burden of disease and large healthcare costs. Thus, there is a need for affordable, effective and scalable interventions. However, interventions that are affordable and scalable are beset with modest effects and engagement. Interventions that integrate machine learning with real-time data to offer unprecedented levels of personalisation and customisation might offer solutions. The aim of this study is to conduct a randomised controlled trial to evaluate the effectiveness of a machine learning and app-based digital assistant to inc
