Design and Development of a Compact, Portable Nondispersive Infrared (NDIR)-Based Capnography Device for Real-Time End-Tidal Carbon Dioxide (CO₂) Monitoring.

Measuring carbon dioxide (CO₂) in exhaled breath provides critical insight into both ventilation and metabolism. Changes in CO₂ levels can indicate hypoventilation, airway obstruction, circulatory compromise, or metabolic imbalance, allowing the early detection of respiratory distress and other life-threatening conditions. Capnography provides continuous, non-invasive monitoring of respiratory status and metabolic function through the measurement of end-tidal CO₂ (ETCO₂). This work focuses on developing a low-cost, portable, battery-powered capnograph that enables accurate, continuous CO₂ moni

Consensus of Korean Asthma Study Group on Definition of Clinical Remission in Severe Asthma: A Modified Delphi Study.

BACKGROUND: Asthma remission has recently emerged as an aspirational treatment goal, yet its definition remains inconsistent across studies and expert groups. The absence of a standardized framework hampers its application in clinical practice and research, particularly in Korea where biologics use is rapidly increasing. This study aimed to establish a consensus definition of clinical remission in severe asthma among Korean experts. METHODS: A two-round modified Delphi survey, followed by a focused third round, was conducted among 28 board-certified pulmonologists from the Korean Academy of T

Macrophage Migratory Inhibitory Factor May Contribute to the Production of Nitric Oxide in Obstructive Sleep Apnea.

In obstructive sleep apnea (OSA), repeated airway obstruction alters mucosal inflammation, which increases exhaled nitric oxide (NO) production in the nasal cavity. However, the underlying mechanism remains unclear. Accordingly, we aimed to examine the mechanism underlying NO production in patients with OSA. We included eight patients with moderate-to-severe OSA who underwent continuous positive airway pressure (CPAP) therapy. Nasal cavity NO levels were measured before and after CPAP; additionally, the levels of inflammatory cytokines in the supernatants of nasal mucosal samples and mRNA expr

Breath- and blood-based molecular assessment for gastroesophageal cancer.

Gastroesophageal cancer remains a significant health care problem characterized by diagnosis at advanced stages, rendering a significant proportion of patients ineligible for curative treatment. Conversely, early diagnosis and treatment of gastroesophageal cancer demonstrates markedly improved outcomes, with 5-year overall survival rates ranging from 83% to 96%. To date, gastroesophageal cancer surveillance primarily focuses on patients with Barrett's esophagus, relying on invasive upper endoscopy. However, despite its high specificity, upper endoscopy demonstrates a suboptimal cancer yield an

Mediation Effects of Biobehavioral Factors in a Trial of Pharmacotherapy and Intensive Cessation Counseling for People with HIV Who Smoke Cigarettes in Nairobi, Kenya.

There is growing recognition of the important health risks of tobacco use in people with HIV (PWH). Multiple randomized controlled trials have tested cessation treatments in this population, but little is known about factors that mediate successful quitting. We conducted a randomized, placebo-controlled 2 × 2 factorial design trial of a behavioral intervention (Positively Smoke Free [PSF] one-on-one counseling) vs. brief advice to quit ± bupropion vs. placebo in PWH who smoked cigarettes in Nairobi, Kenya. Abstinence from cigarettes was assessed by self-report and exhaled carbon monoxide (ECO)

Filtration Performances of Heat and Moisture Exchangers and Filters Against Viral Aerosols: Adaptation of a Wind Tunnel.

BACKGROUND: The exhaled breath of infected, mechanically ventilated patients poses an infection risk to health care workers. Proper expiratory gas filtration with heat and moisture exchanger filters (HMEF) or filters could reduce that phenomenon. Current laboratory means of assessing the filtration efficiency are limited to the use of monodisperse aerosols at a single humidity level and flow. This study aims to examine the filtration efficiency of various devices under simulated clinical conditions, namely against a broad range of particle sizes containing viruses at different levels of gas hu

START CARE: a protocol for a randomised controlled trial of step-wise budesonide-formoterol reliever-based treatment in children.

BACKGROUND: Asthma is the most common chronic childhood respiratory condition globally. Inhaled corticosteroid (ICS)-formoterol reliever-based regimens reduce the risk of asthma exacerbations compared with conventional short-acting β2-agonist (SABA) reliever-based regimens in adults and adolescents. The current limited evidence for anti-inflammatory reliever therapy in children means it is unknown whether these findings are also applicable to children. High-quality randomised controlled trials (RCTs) are needed. OBJECTIVE: The study aim is to determine the efficacy and safety of budesonide-fo

Mepolizumab in Severe Pediatric Asthma: Certainties and Doubts through a Single-Center Experience and Review of the Literature.

BACKGROUND: Although, in most children with asthma, good symptom control is achieved with a low to moderate dose of inhaled corticosteroids, a small group of patients still experiences frequent symptoms, and even severe exacerbations, impairment of lung function, and reduced quality of life. Some of these subjects with severe asthma require biologic drugs as add-on therapy. In the past decade, numerous monoclonal antibodies have been approved for children or adolescents with severe asthma, in addition to their increasing use in adult asthma. However, the available evidence on how to select the

Using Near-Infrared Spectroscopy Wearable Devices to Identify Central Versus Peripheral Limitations During Exercise.

The gold standard to assess the aerobic capacity in physically active subjects and athletes is the maximal oxygen consumption test (VO2-max), which involves analysis of exhaled-gases and cardiorespiratory variables obtained via the breath-by-breath method in an ergospirometer during an incremental exercise. However, this method cannot elucidate metabolic changes at the muscular level. Near-infrared spectroscopy (NIRS) has emerged as a valuable technology to evaluate local oxygen levels (Tissular Saturation Index, TSI) by quantifying the concentrations of oxygenated (O2-Hb) and deoxygenated (H-

Capnodynamic determination of end-expiratory lung volume in a porcine model of hypoxic pulmonary vasoconstriction.

PURPOSE: The capnodynamic method, End Expiratory Lung Volume CO2 (EELV-CO2), utilizes exhaled carbon dioxide analysis to estimate End-Expiratory Lung Volume (EELV) and has been validated in both normal lungs and lung injury models. Its performance under systemic hypoxia and variations in CO2 elimination is not examined. This study aims to validate EELV-CO2 against inert gas wash in/wash out (EELV- SF6, sulfur hexafluoride) in a porcine model of stable hemodynamic conditions followed by hypoxic pulmonary vasoconstriction and inhaled nitric oxide (iNO). METHODS: Ten mechanically ventilated pigl

Efficacy of ETB-F01, Heat-Killed Akkermansia muciniphila Strain EB-AMDK19, in Patients with Respiratory Symptoms: A Multicenter Clinical Trial.

Respiratory symptoms are prevalent in the general population, and they are associated with a decline in lung function and increased mortality. The gut-lung connection suggests intestinal dysbiosis may impact lung diseases, with Akkermansia muciniphila showing promise in regulating extraintestinal diseases. However, its application in patients with respiratory symptoms lacks clinical trial evidence. In this randomized, double-blind trial, ETB-F01, containing heat-killed A. muciniphila strain EB-AMDK19, was compared with a placebo in patients experiencing respiratory symptoms for 4 to 12 weeks.
