The need & the issues related to new-generation typhoid conjugate vaccines in India.

The low- and middle-income countries bear the highest burden of typhoid fever in the world. India, along with other South Asian countries, has a significant incidence of typhoid fever among young children though there is a paucity of published data on community burden. In spite of the availability of Vi-polysaccharide (Vi-PS) and conjugated Vi-PS vaccines, these are not adequately utilized in India and in the neighbouring countries. To address many shortcomings of the unconjugated Vi-PS vaccines, typhoid conjugate vaccines (TCVs) are developed by conjugating Vi-PS with different carrier protei

Tenacious Endemic Typhoid Fever in Samoa.

BACKGROUND: Typhoid fever has been endemic on the island nation of Samoa (2016 population, 195 979) since the 1960s and has persisted through 2019, despite economic development and improvements in water supply and sanitation. METHODS: Salmonella enterica serovar Typhi isolates from the 2 hospitals with blood culture capability and matched patient demographic and clinical data from January 2008 through December 2019 were analyzed. Denominators to calculate incidence by island, region, and district came from 2011 and 2016 censuses and from 2017-2019 projections from Samoa's Bureau of Statistics

Elimination of typhoid: Possibility or pipe dream?

The elimination of typhoid would have a substantial impact on the health of millions of people living in South and Southeast Asia and sub-Saharan Africa. Like many diseases before it, there are significant challenges to elimination, including access to safe drinking water and adequate sanitation practices, the availability of a vaccine, the identification and management of chronic carriers and the emergence of anti-microbial resistance. While provision of clean water and adequate sanitation must remain the ultimate goal, in the short term, required to cornerstone elimination in endemic areas,

Efficacy and immunogenicity of a Vi-tetanus toxoid conjugate vaccine in the prevention of typhoid fever using a controlled human infection model of Salmonella Typhi: a randomised controlled, phase 2b trial.

BACKGROUND: Salmonella enterica serovar Typhi (S Typhi) is responsible for an estimated 20 million infections and 200 000 deaths each year in resource poor regions of the world. Capsular Vi-polysaccharide-protein conjugate vaccines (Vi-conjugate vaccines) are immunogenic and can be used from infancy but there are no efficacy data for the leading candidate vaccine being considered for widespread use. To address this knowledge gap, we assessed the efficacy of a Vi-tetanus toxoid conjugate vaccine using an established human infection model of S Typhi. METHODS: In this single-centre, randomised c

Reversible encephalopathy secondary to paratyphoid infection and concomitant acute Hepatitis A.

Reversible encephalopathy has been described in association with typhoid fever as well as nontyphoidal salmonella infections. A diagnostic dilemma as to the cause of encephalopathy may arise when there is coexistent acute viral hepatitis and suspicion of fulminant liver failure. The authors report a patient who presented with acute icteric hepatitis A infection and a concomitant febrile illness due to Salmonella paratyphi associated with progressive encephalopathy and coma. The young man developed high-grade fever and coma. Concomitant infection with S. paratyphi was diagnosed and the patient'

Experimental and natural infection of early weaned pigs with Salmonella choleraesuis.

A model for experimental and natural infection of early weaned pigs with Salmonella choleraesuis, the aetiologic agent of swine paratyphoid, has been developed. An oral dose of 10(8) colony forming units (cfu) of S choleraesuis caused 100 per cent infection of 10 pigs inoculated, as indicated by recovery of the challenge organism from ileocolic lymph nodes collected at necropsy seven days post challenge. Seven of the pigs were observed shedding S choleraesuis at least once post S choleraesuis challenge. The cumulative incidence of shedding was 30 per cent and was sufficient to infect four of 1

Who introduced typhoid vaccination: Almroth Write or Richard Pfeiffer?

The British pathologist Almroth Wright generally is credited with the initiation of typhoid vaccination in 1896. His claims of priority were challenged as early as 1907 in favor of Richard Pfeiffer, a German bacteriologist and a student of Robert Koch. A review of the original literature of the 1890s and the early 1900s revealed that several groups were working on typhoid vaccine at the same time and that the credit for the initiation of typhoid vaccine studies should be shared by these two great researchers.
