Treatment patterns, intensive treatment, and hospitalization-related burden of juvenile generalized myasthenia gravis in Japan: a retrospective cohort study using a hospital-based claims database.

OBJECTIVE: Juvenile generalized myasthenia gravis (MG) management aims to control symptoms while minimizing corticosteroid-related toxicity. However, large-scale evidence on real-world pediatric treatment patterns remains limited. METHODS: We conducted a retrospective cohort study using a large Japanese hospital-based claims database (Medical Data Vision). We identified patients aged 0-17 years with juvenile generalized MG between April 2008 and November 2024. We summarized the use of cholinesterase inhibitors, oral corticosteroids, immunosuppressants, and intensive treatments (plasma exchang

The thymus-dependent immune system in the pathogenesis of type 1 (insulin-dependent) diabetes mellitus. Animal model and human studies.

The aim of the present study was to investigate the possible role of the thymus-dependent immune system in the disease mechanisms underlying … hymus-intact mice. This survey includes other experiments using various forms of immunosuppression (thymectomy, irradiation, treatment with antilymphocyte serum), which together supply evidence that the thymus-dependent immune system is involved in the pathogenesis of diabetes in the two mouse models mentioned as well as in the spontaneously diabetic BB rat, …

Too early to be different? A multi-institutional study with 30-year follow-up for prognostic factors of completely resected early stage thymoma.

BACKGROUND/PURPOSE: This multi-institutional study was aimed to analyze prognostic factors of completely resected early stage thymoma. METHODS: Patients with surgically treated early stage thymoma between 1988 and 2019 were enrolled. Statistical associations were evaluated using the χ2 test, Fisher's exact test, and Student's t-test. Disease-free survival (DFS) and overall survival (OS) curves were established by the Kaplan-Meier method, and compared using the log-rank test. RESULTS: A total of 229 patients with Masaoka stage I and 131 with Masaoka stage II thymoma were included. The DFS of

Comparison of Neoadjuvant Chemotherapy Followed by Surgery to Upfront Surgery for Thymic Malignancy.

BACKGROUND: The oncologic benefit of neoadjuvant chemotherapy in thymic malignancies remains unclear. Postoperative oncologic outcomes of curative resection after neoadjuvant chemotherapy were compared with those of upfront surgery. METHODS: Based on records from a multicenter database, 1,486 patients with surgically resected thymic malignancies between 2000 and 2013 were included in the final study cohort. Of these, 110 patients (7.4%) underwent surgical resection after neoadjuvant chemotherapy, and 1,376 patients (92.6%) underwent upfront surgery. A propensity score-matched analysis was per

Recurrent thymoma with stiff-person syndrome and pure red blood cell aplasia.

Stiff-person syndrome (formerly known as stiff-man syndrome) is a very rare autoimmune and neurogenic disorder, thought to present as a paraneoplastic variant in association with thymoma. Pure red blood cell aplasia is also a paraneoplastic disorder associated with thymoma. Although separate cases of stiff-person syndrome and pure red blood cell aplasia have been reported, we describe here what is to our knowledge the first case of recurrent thymoma with both stiff-person syndrome and pure red blood cell aplasia. We describe the successful treatment of the neurogenic symptoms of stiff-person s

Resection of an invasive thymoma extending into the superior vena cava and right atrium.

Although invasive thymoma commonly infiltrates mediastinal structures, intracardiac extension from intracaval growth is rare. We present a case of invasive thymoma with intravascular invasion of the superior vena cava extending into the right atrium.

OBJECTIVE: Simultaneous cardiothoracic and general surgical interventions are still a matter of controversy. We investigated the question whether combined open heart surgery or thoracic surgery and interventions of the parathyreoid respectively thyroid gland may be helpful for the patients. PATIENTS AND METHODS: The records of six patients (age range: 29-78 years) undergoing simultaneous cardiothoracic and para-/thyroid interventions were examined. Cardiothoracic surgery consisted in three patients of coronary artery bypass grafting, in one of aortic valve replacement, in one of thymus resect

Muscle autoantibodies in subgroups of myasthenia gravis patients.

Myasthenia gravis (MG) is caused by autoantibodies to the acetylcholine receptor (AChR), but several other muscle autoantibodies have also been identified in patient sera. We studied muscle autoantibodies against AChR, striated muscle tissue sections (SH), titin, citric acid antigen (CA), and ryanodine receptor (RyR) in sera from 146 consecutive MG patients to evaluate whether a single test or several tests together can predict a thymoma. The MG patients were divided into five subgroups; ocular MG, early-onset MG (< 50 years), late-onset MG (> 50 years), MG with thymoma, and AChR antibody nega

Organ-specific autoimmune disease induced in mice by elimination of T cell subsets. V. Neonatal administration of cyclosporin A causes autoimmune disease.

Cyclosporin A (CsA), a potent immunosuppressive drug, caused organ-specific autoimmune disease, such as gastritis with anti-parietal cell autoantibodies or oophoritis with anti-oocyte autoantibodies, in BALB/c mice when the drug was administered daily for 1 wk to newborns. Administration to adult mice did not. CsA abrogated the production of L3T4+ T cells and Lyt-2+ T cells in the thymus. Consequently, these T cells were substantially depleted from the peripheral lymphoid organs, especially when the drug was administered from the day of birth. Autoimmune disease was prevented when CsA-treated

Thymoma originating in a giant thymolipoma: a rare intrathoracic lesion.

Thymolipoma is a rare, slow-growing, benign tumor that arises from the anterior mediastinum and corresponds to 2% to 9% of all thymic neoplasms. We present the case of a 49-year-old man who had a large heterogeneous mass with areas of soft tissue and fat tissue located on the anterior mediastinum and right hemithorax. After resection, histologic analysis confirmed the diagnosis of a giant thymolipoma containing solid components that corresponded to thymomas B1, B2, and B3. We discuss the occurrence of an atypical variant of thymolipoma containing three types of thymomas inside.

Thymic carcinoma with dissemination: a retrospective analysis of ten patients.

OBJECTIVE: Thymic carcinoma is a rare mediastinal neoplasm with frequent pleural or pericardial dissemination. We retrospectively studied ten such cases and analyzed factors that influenced the survival of the patients. METHODS: Ten thymic carcinoma patients with dissemination have been treated since 1987. The clinical and pathological data were retrospectively reviewed. RESULTS: Pretreatment tumor biopsy was performed and demonstrated squamous cell carcinomas in nine and small cell carcinoma in one. In six of ten patients pleural or pericardial dissemination was clinically evident (cT4). Th

Thymoma in children: report of two cases and review of literature.

Thymoma is an uncommon tumor of childhood. Stage of the tumor is an independent prognostic factor for survival. Surgery is the treatment of choice for stage I and stage II tumors. Chemotherapy is reserved for patients with refractory or metastatic disease. Thymomas are moderately radiosensitive. However, radiation therapy is not an attractive option for children due to its side-effects on developing organs. The authors describe 2 children with completely encapsulated thymoma who were successfully treated with surgery alone. Both patients remain free of disease 3 years after surgery. One of the

Mechanism of the induction of autoimmune disease by graft-versus-host reaction. Role of CD8+ cells in the development of hepatic and ductal lesions induced by CD4+ cells in MHC class I plus II-different host.

BACKGROUND: We previously reported that primary biliary cirrhosis (PBC)-like hepatic lesions were induced in (bm12xB6)F1 mice undergoing graft-versus-host reaction with major histocompatibility complex class II disparity. In this paper, we report on a new experimental system, enabling establishment of more progressed stages of PBC-like lesions and clarification of the role of CD8+ cells in the development of the lesions. EXPERIMENTAL DESIGN: Recipient (bm12xbm1)F1 mice were thymectomized and administered anti-Lyt-2 monoclonal antibodies intraperitoneally to deplete host CD8+ cells as complete

Analysis of Ly-1+ B-cell populations and IgH rearrangements in "normal" spleens and in lymphomas of AKR/J and AKR Fv-1b mice.

AKR mice are highly susceptible to development of spontaneous T-cell lymphoma. Thymus removal at the age of 1-3 months greatly reduces T-cell lymphoma. Lymphomas that have the characteristics of T- and/or B-cells occur sporadically in peripheral lymphoid tissues of old thymectomized AKR/J mice. These thymectomized mice were shown to carry dormant potential lymphoma cells. Transplantation of lymphoid cells from 8-12-month-old AKR/J mice, thymectomized at the age of 6 to 8 weeks, into intact or thymectomized young recipients yielded 80-100% Ly-1+ pre-B or B-cell lymphomas. In the AKR-Fv-1b conge
