Flow resistance analysis of extracranial-to-intracranial (EC-IC) vein bypass.

Although brain bypass surgery has often been selected to treat internal carotid arteries (ICA) which are restricted by aneurysm or artery stenosis, its effectiveness has not been quantitatively evaluated. The purpose of this study is to propose an innovative approach for the evaluation of brain extracranial-to-intracranial (EC-IC) vein bypass surgery, based on the analysis of flow resistance in vein bypasses and within their contralateral carotid arteries through the use of computational fluid dynamics (CFD). Seven patients who underwent vein bypass surgery were examined with the use of high-r

Acute to subacute surgical revascularization for progressing stroke in atherosclerotic vertebrobasilar occlusion.

BACKGROUND: Acute vertebrobasilar artery (VBA) occlusion is catastrophic. For embolic occlusion, thrombolysis is reasonable. However, if the occlusion is atherosclerotic, the best therapeutic approach remains unclear. The aim of this study was to characterize the clinical course, case selection, techniques and complications associated with acute to subacute surgical revascularization in atherosclerotic vertebrobasilar occlusion under appropriate patient selection based on diffusion-weighted imaging (DWI) combined with careful evaluation of progressive neurological symptoms. METHODS: We retros

Hyperventilation technetium-99m-HMPAO brain SPECT in moyamoya disease to assess risk of natural childbirth.

We report a pregnant 19-yr-old patient with moyamoya disease who had undergone bilateral superficial temporal artery to middle cerebral artery anastomosis and encephalomyosynangiosis at 8 yr with an uneventful postoperative course and who desired natural delivery after becoming pregnant at 18 yr. We determined her cerebral vascular reserve since natural delivery can result in decreased cerebral blood flow during labor. Technetium-99m-HMPAO brain SPECT, with hyperventilation challenge, was performed to assess cerebral vascular reserve since the stress of hyperventilation was thought likely to r

Carotid surgery in the octogenarian.

The records of 52 patients aged 80 years or older who underwent 56 carotid artery reconstructions were analyzed retrospectively. Four patients had amaurosis fugax, 27 patients had experienced one or more transient ischemic attacks, eight had a completely or partially reversible stroke, and 10 had vertebrobasilar insufficiency. Three patients were asymptomatic. Arteriograms documented stenosis > 80% on the operated side in 48 cases, whereas the contralateral carotid artery was occluded or had > 80% stenosis in 10 cases each. Two or more cerebral arteries were involved in 37 patients. CT scans w

Post-endarterectomy headache and the role of the oculosympathetic system.

A study was carried out on headache after carotid endarterectomy. A specific type of headache, similar in its characteristics to "cluster headache", occurred on the operative side in 30% of 54 patients, whereas no such headache occurred after extra-intracranial bypass or peripheral vascular surgery. This postoperative headache was not spontaneously reported by 56% of patients unless they were specifically asked about it. Pharmacological pupillary testing performed in 37 patients revealed that a decreased oculosympathetic activity (with or without adrenoceptor supersensitivity) was constantly a

Relationship Between Degree of Stenosis and Time-to-Peak Delay in High Grade Asymptomatic Carotid Artery Disease.

BACKGROUND: Cerebral hemodynamic impairment in patients with asymptomatic high-grade (>70%) internal carotid artery (ICA) stenosis is associated with risk of stroke and cognitive decline, but correlations between the degree of stenosis and hemodynamic impairment are derived from small case series. Using baseline data from 242 participants in the CREST-H study (Carotid Revascularization and Medical Management for Asymptomatic Carotid Stenosis-Hemodynamics), we hypothesized that the degree of stenosis in the ICA would correlate with time-to-peak (TTP) delay in ipsilateral cerebral blood flow usi

Safety Outcomes of Mechanical Thrombectomy Versus Combined Thrombectomy and Intravenous Thrombolysis in Tandem Lesions.

BACKGROUND: We aimed to describe the safety and efficacy of mechanical thrombectomy (MT) with or without intravenous thrombolysis (IVT) for patients with tandem lesions and whether using intraprocedural antiplatelet therapy influences MT's safety with IVT treatment. METHODS: This is a subanalysis of a pooled, multicenter cohort of patients with acute anterior circulation tandem lesions treated with MT from 16 stroke centers between January 2015 and December 2020. Primary outcomes included symptomatic intracranial hemorrhage (sICH) and parenchymal hematoma type 2. Additional outcomes included

Total arterial off-pump coronary revascularization with only internal thoracic artery and composite radial artery grafts.

OBJECTIVE: Total arterial off-pump coronary artery bypass (OPCAB) grafting with only internal thoracic artery (ITA) and composite radial artery (RA) grafts has been applied extensively to avoid cerebral complications and late vein graft failure. We evaluated the initial experience with this method by clinical and angiographic study. METHODS: Between April 2000 and May 2002, 257 patients underwent OPCAB grafting with this technique. The range of ages at operation was 42 to 86 years (mean, 66.1 +/- 8.6 years). On average, 3.28 +/- 0.86 grafts per patient were completed. More than 4 distal anast

Effects of end-tidal carbon dioxide levels in patients undergoing direct revascularization for Moyamoya disease and risk factors associated with postoperative complications.

A history of transient ischemic attack, severity of disease, urinary output, hematocrit, hypocapnia, and hypotension during direct revascularization (superficial temporal artery to middle cerebral artery [STA-MCA]) in patients with Moyamoya disease (MMD) may lead to a poor prognosis, however, to our knowledge evidence for end-tidal carbon dioxide (ETCO2) targets is lacking. Within the ranges of standardized treatment, the article was primarily designed to study the risk factors associated with the neurologic outcomes during STA-MCA for MMD especially including ETCO2 ranges and the duration in

Clinical Prediction of Surgical Revascularization Outcome in Moyamoya Disease Via Transcranial Color Sonography.

OBJECTIVE: Transcranial color-duplex sonography (TCCS) is a promising method in evaluating the hemodynamics in patients with moyamoya disease (MMD). This study aimed to explore the feasibility of preoperative TCCS in predicting the outcome of revascularization surgery in MMD patients. METHODS: We retrospectively analysed 64 cases of MMD patients receiving revascularization surgery from January 2012 to January 2014. We utilized TCCS to perform comprehensive hemodynamic examination on the hemodynamics of bilateral intracranial and extracranial cerebrovascular flow and assessed the surgical outc

Heart Failure in Ischemic Stroke: Relevance for Acute Care and Outcome.

Background and Purpose- Heart failure (HF) in patients with acute ischemic stroke constitutes the source of various detrimental pathophysiologic mechanisms including prothrombotic and proinflammatory states, worsening of cerebral tissue oxygenation, and hemodynamic impairment. In addition, HF might affect the safety and efficacy of the acute recanalization stroke therapies. Methods- Patients treated with intravenous recombinant tissue-type plasminogen activator or mechanical recanalization at a universitary stroke center were included into a prospective registry. Patients received cardiologica

The Optimum Operative Time of Revascularization for Patients with Moyamoya Disease Following Acute Onset.

OBJECTIVE: Whether surgery should be performed in patients with acute onset of moyamoya disease (MMD) is controversial. This study aimed to determine optimum operative time for patients with MMD. METHODS: We retrospectively analyzed 57 patients with MMD admitted between January 2016 and June 2017. All patients consented to combined revascularization. Considering the time interval between acute onset of MMD and surgery, we divided all patients into an early group and later group (>90 days between MMD onset and surgery). We compared postoperative complications, neurologic improvement, and favor

Microsurgical interintimal dissection in carotid endarterectomy.

BACKGROUND: Safe and complete surgical excision of carotid plaque in carotid endarterectomy (CEA) is essential for preventing postoperative embolic stroke and restenosis. We considered the dissection plane for the plaque according to pathological findings of carotid atherosclerosis. We report our experiences with inner-intimal dissection in CEA. METHODS: To obtain complete resection of the plaque with a smooth distal edge and bloodless surface by minimal exposure of the media, the thickened intima was sliced under high-magnification microscopy. The excised specimens were examined pathological

Long-term social outcome in children with moyamoya disease who have reached adulthood.

OBJECT: Although the reported surgical outcome is favorable, there is little information regarding the long-term quality of life in pediatric patients with moyamoya disease (MMD) when they grow to adulthood. The authors conducted a survey to provide details of social adaptation and satisfaction in adults who underwent revascularization surgery for MMD during childhood. METHODS: One hundred twenty-three patients with MMD who had undergone surgery during childhood and were older than 18 years of age were recruited for this study. The authors mailed self-administered questionnaires regarding eac

The role of indirect extracranial-intracranial bypass in the treatment of symptomatic intracranial atheroocclusive disease.

OBJECT: The optimal treatment of medically refractory intracranial atheroocclusive disease remains unclear. The EC-IC Bypass Study Investigators found that patients with internal carotid and middle cerebral artery (ICA and MCA) occlusion received no benefit from direct superficial temporal artery to MCA bypass, and that patients with ICA occlusion and MCA stenosis may have actually fared worse after surgery, perhaps in part due to flow reversal in critical perforator-bearing segments. Although the results of recent investigations have suggested that direct bypass may be beneficial in a subgrou

Recurrent retinal ischemia beyond cervical carotid occlusions: clinical-angiographic correlations and therapeutic implications.

Seventeen patients with persistent amaurosis fugax ipsilateral to angiographically documented internal carotid artery (ICA) occlusions in the neck have been treated by the authors over the past 5 years. Complete cerebral arteriography in each case demonstrated that the symptomatic ophthalmic artery was perfused exclusively by the ipsilateral external carotid artery (ECA), which invariably had an obstructive and/or ulcerative lesion and its origin, and/or an adjacent residual "stump" of the occluded ICA. In nine patients, retinal artery branch emboli were visible on funduscopy. One patient had
