DIAMONDS-a diabetes self-management intervention for people with severe mental illness: protocol for an individually randomised controlled multicentre trial.

INTRODUCTION: Type 2 diabetes mellitus (T2DM) is two to three times more common in people with severe mental illness (SMI) than in the general population. Supporting self-management in diabetes is fundamental to improving clinical outcomes. The DIAMONDS trial aims to evaluate the clinical and cost effectiveness of a novel, codesigned, supported diabetes self-management programme for people with T2DM and SMI. METHODS AND ANALYSIS: This multicentre, two-armed, parallel, individually randomised controlled trial will be conducted in National Health Service mental health trusts across England. We

INCEPT: The Intensive Care Platform Trial-Design and protocol.

BACKGROUND: Adult intensive care unit (ICU) patients receive many interventions, but few are supported by high-certainty evidence. Randomised clinical trials (RCTs) are essential for trustworthy comparisons of intervention effects, but conventional RCTs are costly, cumbersome, inflexible, and often turn out inconclusive. Adaptive platform trials may mitigate these issues and have higher probabilities of obtaining conclusive results faster and at lower costs per participant. METHODS: The Intensive Care Platform Trial (INCEPT) is an investigator-initiated, pragmatic, randomised, embedded, multi

Enhanced prediction of ventilator-associated pneumonia in patients with traumatic brain injury using advanced machine learning techniques.

Ventilator-associated pneumonia significantly increases morbidity, mortality, and healthcare costs among patients with traumatic brain injury. Accurately predicting risk can facilitate earlier interventions and improve patient outcomes. This study leveraged the MIMIC III database, identifying traumatic brain injury cases through standardized clinical criteria. A rigorous data preprocessing workflow included missing value imputation, correlation checks, and expert-driven feature selection, reducing an initial set of features to a subset of critical predictors encompassing demographics, comorbid

Selective Bladder Denervation via Radiofrequency Ablation Versus Intravesical Onabotulinum Toxin A for Refractory Overactive Bladder: The Short-Term Results of A Preliminary Randomized Controlled Trial.

INTRODUCTION: Overactive bladder (OAB) is a prevalent and burdensome condition associated with significant impairments in health-related quality of life and substantial healthcare costs. While third-line therapies such as intravesical onabotulinum toxin A (BTA) and sacral neuromodulation are established treatment options for refractory cases, selective bladder denervation (SBD) via radiofrequency ablation (RFA) has emerged as a novel minimally invasive alternative. This study aimed to prospectively compare the efficacy and safety of SBD versus BTA in women with refractory OAB. METHODS: In thi

Effect of a Healing Program Using Marine Resources on Reducing Pain and Improving Physical Function in Patients with Non-Specific Chronic Low Back Pain: A Randomized Controlled Trial Study.

Background and Objectives: Chronic low back pain is a widespread condition, particularly in older populations, contributing to physical, mental, and social burdens. Traditional treatments, such as medications and surgery, carry long-term risks, including dependency, side-effects, and complications from invasive procedures. Additionally, healthcare accessibility is limited due to high costs, long waiting times, and geographic disparities in healthcare services, particularly in rural areas. For these reasons, non-pharmacological approaches that address both physical and psychological aspects are

Rosa roxburghii tratt residue: A novel feed resource for cattle indicated by the non-deleterious performance and blood metabolites.

Rosa roxburghii tratt residue (RRTR) is a regional and uncommon byproduct in Guizhou. Little information is available on whether RRTR can be used as feed for ruminants. In this study, the feasibility of using RRTR as a new feed resource for cattle was investigated by chemical composition analysis, in vitro gas production (Trial 1) and animal feeding experiments (Trial 2). In trial 1, compared to the commonly used fruit residues, RRTR had a proximate nutrient level; the in vitro gas production curve and dynamics fell within the normal range. In trial 2, 16 cattle were allocated to the control a

The IMpact of PerioperAtive KeTamine on Enhanced Recovery after Abdominal Surgery (IMPAKT ERAS): protocol for a pragmatic, randomized, double-blinded, placebo-controlled trial.

BACKGROUND: Multimodal analgesic strategies that reduce perioperative opioid consumption are well-supported in Enhanced Recovery After Surgery (ERAS) literature. However, the optimal analgesic regimen has not been established, as the contributions of each individual agent to the overall analgesic efficacy with opioid reduction remains unknown. Perioperative ketamine infusions can decrease opioid consumption and opioid-related side effects. However, as opioid requirements are drastically minimized within ERAS models, the differential effects of ketamine within an ERAS pathway remain unknown. We

Optimizing nanoliposomal formulations: Assessing factors affecting entrapment efficiency of curcumin-loaded liposomes using machine learning.

BACKGROUND: Curcumin faces challenges in clinical applications due to its low bioavailability and poor water solubility. Liposomes have emerged as a promising delivery system for curcumin. This study aims to apply ensemble learning, a machine learning technique, to determine the most effective experimental conditions for formulating stable curcumin-loaded liposomes with a high entrapment efficiency (EE). METHODS: Two liposomal formulations composed of HSPC:DPPG:Chol:DSPE-mPEG2000 and HSPC:Chol:DSPE-mPEG2000 at 55:5:35:5 and 55:40:5 M ratios, respectively, were prepared using the remote loadin

Modified Brain Injury Guidelines for preinjury anticoagulation in traumatic brain injury: An opportunity to reduce health care resource utilization.

INTRODUCTION: The Brain Injury Guidelines (BIG) stratify patients by traumatic brain injury (TBI) severity to provide management recommendations to reduce health care resource burden but mandates that patients on anticoagulation (AC) are allocated to the most severe tertile (BIG 3). We sought to analyze TBI patients on AC therapy using a modified BIG model to determine if this population can offer further opportunity for safe reductions in health care resource utilization. METHODS: Patients 55 years or older on AC with traumatic intracranial hemorrhage (ICH) from two centers were retrospectiv

Effectiveness of a specialist palliative home care nurse-patient consultation followed by an interprofessional telephone case conference compared with usual care among patients with non-oncological palliative care needs: protocol for the multicentre KOPAL cluster-randomised controlled trial.

INTRODUCTION: Progressive chronic, non-malignant diseases (CNMD) like congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and dementia are of growing relevance in primary care. Most of these patients suffer from severe symptoms, reduced quality of life and increased numbers of hospitalisations. Outpatient palliative care can help to reduce hospitalisation rate by up to 50%. Due to the complex medical conditions and prognostic uncertainty of the course of CNMD, early interprofessional care planning among general practitioners who provide general palliative care and spec

The effect of emergency room consultations on emergency general surgery operations.

BACKGROUND: Hospitalizations in emergency general surgery (EGS) cases in the USA have increased by 28% since 2001. The costs of these cases are estimated to increase by 45% annually until 2060, reaching 41.20 billion dollars. According to the literature, the general surgery clinic team allocates an average of 5.5 hours a day for emergency room consultations. The aim of this study is to determine the effects of consultations from the emergency room in our country on the EGS approach and to create appropriate solution proposals with the data obtained from the regional hospitals. METHODS: The so

Evaluating the effectiveness of e-cigarettes compared with usual care for smoking cessation when offered to smokers at homeless centres: protocol for a multi-centre cluster-randomized controlled trial in Great Britain.

BACKGROUND AND AIMS: Smoking is extremely common among adults experiencing homelessness, but there is lack of evidence for treatment efficacy. E-cigarettes are an effective quitting aid, but they have not been widely tested in smokers with complex health and social needs. Here we build upon our cluster feasibility trial and evaluate the offer of an e-cigarette or usual care to smokers accessing a homeless centre. DESIGN, SETTING AND PARTICIPANTS: Multi-centre two-arm cluster-randomized controlled trial with mixed-method embedded process and economic evaluation in homeless centres in England,

Experimental feasibility of xenon-enhanced dual-energy radiography for imaging of lung function.

Chronic obstructive pulmonary disease (COPD) is a leading cause of death worldwide. We experimentally investigated the feasibility of two-dimensional xenon-enhanced dual-energy (XeDE) radiography for imaging of lung function. We optimized image quality under quantum-noise-limited conditions using a chest phantom consisting of a rectangular chamber representing the thoracic volume and PMMA slabs simulating x-ray attenuation by soft tissue. A sealed, air-filled cavity with thin PMMA walls was positioned inside the chamber to simulate a 2 cm thick ventilation defect. The chamber was ventilated wi

Can metformin relieve tibiofemoral cartilage volume loss and knee symptoms in overweight knee osteoarthritis patients? Study protocol for a randomized, double-blind, and placebo-controlled trial.

BACKGROUND: Osteoarthritis (OA) is the most common joint disease, and is most frequently seen in the knees. However, there is no effective therapy to relieve the progression of knee OA. Metformin is a safe, well-tolerated oral medication that is extensively used as first-line therapy for type 2 diabetes. Previous observational studies and basic researches suggested that metformin may have protective effects on knee OA, which needs to be verified by clinical trials. This study, therefore, aims to examine the effects of metformin versus placebo on knee cartilage volume loss and knee symptoms in

Melatonin for prevention of postoperative delirium after lower limb fracture surgery in elderly patients (DELIRLESS): study protocol for a multicentre randomised controlled trial.

INTRODUCTION: Postoperative delirium (POD) is one of the most frequent complication after surgery in elderly patients, and is associated with increased morbidity and mortality, prolonged length of stay, cognitive and functional decline leading to loss of autonomy, and important additional healthcare costs. Perioperative inflammatory stress is a key element in POD genesis. Melatonin exhibits antioxidative and immune-modulatory proprieties that are promising concerning delirium prevention, but in perioperative context literature are scarce and conflicting. We hypothesise that perioperative melat

Efficacy of linezolid on Treponema pallidum, the syphilis agent: A preclinical study.

BACKGROUND: Penicillin G, the current standard treatment for syphilis, has important drawbacks, but virtually no preclinical or clinical studies have been performed to identify viable alternatives. We tested, both in vitro and in vivo, three marketed antibiotics with adequate pharmacological properties to treat syphilis. METHODS: We used an in vitro culturing system of T. pallidum to perform drug susceptibility testing and applied quantitative PCR targeting the tp0574 gene to measure bacterial growth. To confirm in vivo efficacy, fifteen rabbits were infected intradermally with T. pallidum at

Comparison of autologous osteoperiosteal cylinder and osteochondral graft transplantation in the treatment of large cystic osteochondral lesions of the talus (OLTs): a protocol for a non-inferiority randomised controlled trial.

INTRODUCTION: Large cystic osteochondral lesions of the talus (OLTs) have been shown to have inferior clinical outcomes after reparative techniques such as bone marrow stimulation. Autologous osteochondral transplantation has been viewed as an alternative choice for treating these lesions, but donor-site morbidity has limited its application. Excellent clinical outcomes have been shown in repairing these types of lesions with autologous osteoperiosteal grafts, and these outcomes are achieved at a low cost and without donor-site morbidity in the normal knee joint. This will be the first randomi

KARAOKE: Krill oil versus placebo in the treatment of knee osteoarthritis: protocol for a randomised controlled trial.

BACKGROUND: Knee osteoarthritis (OA) is a common and important cause of pain and disability, but interventions aimed at modifying structures visible on imaging have been disappointing. While OA affects the whole joint, synovitis and effusion have been recognised as having a role in the pathogenesis of OA. Krill oil reduces knee pain and systemic inflammation and could be used for targeting inflammatory mechanisms of OA. METHODS/DESIGN: We will recruit 260 patients with clinical knee OA, significant knee pain and effusion-synovitis present on MRI in five Australian cities (Hobart, Melbourne, S
