Comparison of short bowel syndrome acquired early in life and during adolescence.

BACKGROUND: Prolonged survival in pediatric patients with short bowel syndrome (SBS) is now possible because of parenteral nutrition and small bowel transplantation. We hypothesized that there may be important differences between adult patients who developed SBS during early childhood and those who develop this as adolescents. METHODS: Sixty-seven patients between the ages of 16 and 40 years were studied. Thirty patients developing SBS younger than 12 years comprised the pediatric group (PG), 37 developing SBS at age 13 to 25 constituted the adolescent group (AG). RESULTS: Midgut volvulus (n

Higher cord blood levels of mannose-binding lectin-associated serine protease-2 in infants with necrotising enterocolitis.

Necrotising enterocolitis (NEC) causes significant morbidity and mortality in premature infants. The role of innate immunity in the pathogenesis of NEC remains unclear. Mannose-binding lectin (MBL) recognizes microorganisms and activates the complement system via MBL-associated serine protease-2 (MASP-2). The aim of this study was to investigate whether MBL and MASP-2 are associated with NEC. This observational case-control study included 32 infants with radiologically confirmed NEC and 64 controls. MBL and MASP-2 were measured in cord blood using ELISA. Multivariate logistic regression was pe

Effects of Formula Milk Feeding in Premature Infants: A Systematic Review.

The preterm baby is born at a critical period for the growth and development of the gastrointestinal and neuromotor systems. Breast milk is the food of choice for infants during the first months of life, as it provides multiple short- and long-term benefits to preterm and sick newborns. Despite this, breastfeeding is often nutritionally insufficient, requiring the addition of fortifiers. In other cases, it is important to ensure the necessary nutrients and calories, which can be provided by formula milk or pasteurized and fortified donated human milk. However, the specific guidelines for the u

[The prevalence of allergic diseases in children with short bowel syndrome].

BACKGROUND: Short bowel syndrome is the result of an extensive surgical resection that leaves the length of the small intestine at a critical value for the proper nutritional absorption. An increased risk of food allergy has been described in patients who suffer from this condition. OBJECTIVE: To describe the prevalence of allergic diseases in a group of patients with short bowel syndrome. METHODS: A descriptive, cross-sectional, and ambispective study was carried out; it included patients with short bowel syndrome who had attended the nephrology service of the National Institute of Pediatri

Probiotics and prebiotics in pediatrics.

This clinical report reviews the currently known health benefits of probiotic and prebiotic products, including those added to commercially available infant formula and other food products for use in children. Probiotics are supplements or foods that contain viable microorganisms that cause alterations of the microflora of the host. Use of probiotics has been shown to be modestly effective in randomized clinical trials (RCTs) in (1) treating acute viral gastroenteritis in healthy children; and (2) preventing antibiotic-associated diarrhea in healthy children. There is some evidence that probio

Alternative hybrid and staged interventional treatment of congenital heart defects in critically ill children with complex and non-cardiac problems.

INTRODUCTION: An individually designed strategy of comprehensive alternative hybrid and staged interventional treatment (AHASIT) can be a reasonable alternative to conventional treatment of congenital heart defects, reduce the risk of cardiac surgery or interventions performed separately, and give an additional chance for critically ill children. AIM: To present our experience and the results of AHASIT of severely ill or borderline children referred for surgery with the diagnosis of congenital heart defects. MATERIAL AND METHODS: A group of 22 patients with complex cardiac and non-cardiac pa

Unintentional i.v. injection of barium sulfate in a child.

PURPOSE: A case of barium sulfate injection into the superior vena cava during an upper gastrointestinal series (UGIS) in which the patient's central venous line (CVL) was mistaken for her gastrostomy tube is reported. SUMMARY: A 17-month-old girl was brought to the fluoroscopy suite to undergo a UGIS with barium sulfate contrast. Her medical history included premature birth and short-gut syndrome after a bowel resection for necrotizing enterocolitis and gastroschisis. She had been treated for multiple bouts of sepsis and was currently receiving antibiotic therapy at home via a CVL. She was a

Ibuprofen for the prevention of patent ductus arteriosus in preterm and/or low birth weight infants.

BACKGROUND: A patent ductus arteriosus (PDA) often complicates the clinical course of preterm infants and increases the risk of intraventricular hemorrhage (IVH), necrotizing enterocolitis (NEC), chronic lung disease (CLD) and death. The standard treatment to close a PDA is indomethacin. Its use is associated with renal, gastrointestinal and cerebral side-effects. Ibuprofen has been shown to be effective in closing a PDA without reducing blood flow velocity to the brain, gut or kidneys. OBJECTIVES: To determine the effectiveness and safety of prophylactic ibuprofen compared to placebo/no inte

Donor Human Milk Implementation in a High-Mother's-Own-Milk NICU: Impact on Feeding Progression and Clinical Outcomes in Extremely Low Birth Weight Infants.

Background: Donor human milk (DHM) is recommended when the mother's own milk (MOM) is unavailable for extremely low birth weight (ELBW) infants. However, the impact of DHM implementation on NICUs with already high human milk utilization remains incompletely characterized. Objective: We aimed to evaluate the association between DHM implementation and feeding practices, feeding progression, parenteral nutrition (PN) exposure, and major neonatal morbidities in ELBW infants. Methods: We conducted a retrospective before-after cohort study in a single tertiary NICU including 116 ELBW infants with fe

Acute kidney injury in infants with necrotising enterocolitis: A case-control study.

UNLABELLED: Necrotising enterocolitis (NEC) is a leading cause of morbidity and mortality in preterm infants. Acute kidney injury (AKI) is increasingly recognised as a common complication in this population, yet its relationship with NEC remains under-characterised. We sought to evaluate the incidence of AKI in infants with NEC and examine its association with NEC severity (using Bell staging) in a tertiary neonatal intensive care unit (NICU). METHODS: A retrospective 1:2 case-control study was conducted in a single tertiary NICU between October 2018 and October 2023. Forty-nine preterm infan

Lactate and gastrointestinal complications during enteral nutrition in pediatric ECMO patients: a retrospective cohort study.

UNLABELLED: Early enteral nutrition in patients undergoing extracorporeal membrane oxygenation (ECMO) is particularly challenging due to metabolic instability, feeding intolerance, and the risk of intestinal ischemia. This study aimed to identify risk factors for gastrointestinal intolerance (GI) and necrotizing enterocolitis (NEC) in pediatric patients supported with veno-arterial (VA) or veno-venous (VV) ECMO. This monocentric and retrospective cohort study involved 127 pediatric patients who underwent ECMO in the pediatric intensive care unit of the Queen Fabiola Children's University Hospi

Continuous infusion of ibuprofen for treatment of patent ductus arteriosus in very low birth weight infants.

BACKGROUND: Ibuprofen (IBU) has proved as effective as indomethacin in the pharmacological closure of hemodynamically significant patent ductus arteriosus (HsPDA), with an efficacy inversely related to gestational age (57-89%). OBJECTIVE: This study aimed to establish whether continuous infusions of IBU could be more effective in very low birth weight infants with no additional adverse effects and reduce the need for surgical ligation. METHODS: A prospective, randomized, double-dummy study was conducted on 112 very low birth weight infants (mean gestational age 27.2 weeks, SD 2; birth weight

Early trophic feeding versus enteral fasting for very preterm or very low birth weight infants.

BACKGROUND: The introduction of enteral feeds for very preterm (< 32 weeks) or very low birth weight (< 1500 grams) infants is often delayed due to concern that early introduction may not be tolerated and may increase the risk of necrotising enterocolitis. However, prolonged enteral fasting may diminish the functional adaptation of the immature gastrointestinal tract and extend the need for parenteral nutrition with its attendant infectious and metabolic risks. Trophic feeding, giving infants very small volumes of milk to promote intestinal maturation, may enhance feeding tolerance and decreas
