Absent vestibulo-ocular reflexes and acute supratentorial lesions.

Loss of vestibulo-ocular reflexes occurred in two patients with acute supratentorial lesions who received therapeutic doses of anticonvulsant drugs. There was no clinical or angiographic evidence of focal brain-stem damage. Absence of vestibulo-ocular reflexes is attributed to a combination of acute cerebral damage and anticonvulsant drugs. The loss of these reflexes in patients with acute cerebral lesions cannot be interpreted as evidence of irreversible brain-stem injury.

Evidence of unilateral isolated utricular hypofunction.

CONCLUSIONS: The findings demonstrate that an enduring unilateral utricular dysfunction, possibly together with canal hypofunction, can occur after labyrinthine disease or injury. They also suggest that unilateral, isolated utricular dysfunction - or utricle paresis - can occur, representing a novel entity in the differential diagnosis of peripheral vestibular function. The occurrence of subjective visual vertical (SVV) asymmetry in the presence of symmetric vestibular evoked myogenic potentials (VEMPs) also confirms that the information from the utricles, rather than the saccules, dominates S

Is oval window transport a royal gate for nanoparticle delivery to vestibule in the inner ear?

Drug delivery to the inner ear by nanomedicine strategies has emerged as an effective therapeutic approach for the management of inner ear diseases including hearing and balance disorders. It is well accepted that substance enters the perilymph from the middle ear through the round window membrane (RWM), but the passage through the oval window (OW) has long been neglected. Up to now, researchers still know little about the pathway via which nanoparticles (NPs) enter the inner ear or how they reach the inner ear following local applications. Herein, we engineered fluorescence traceable chitosan

Cholesteatoma labyrinthine fistula: prevalence and impact.

INTRODUCTION: Labyrinthine fistula is one of the most common complications associated with cholesteatoma. It represents an erosive loss of the endochondral bone overlying the labyrinth. Reasons for cholesteatoma-induced labyrinthine fistula are still poorly understood. OBJECTIVE: Evaluate patients with cholesteatoma, in order to identify possible risk factors or clinical findings associated with labyrinthine fistula. Secondary objectives were to determine the prevalence of labyrinthine fistula in the study cohort, to analyze the role of computed tomography and to describe the hearing results

Vestibular, Visual Acuity, and Balance Outcomes in Children With Cochlear Implants: A Preliminary Report.

OBJECTIVES: There is a high incidence of vestibular loss in children with cochlear implants (CCI). However, the relationship between vestibular loss and various outcomes is unknown in children. The objectives of this study are to (1) determine whether age-related changes in peripheral vestibular tests occur; (2) quantify peripheral vestibular function in children with normal hearing and CCI; and (3) determine whether amount of vestibular loss predicts visual acuity and balance performance. DESIGN: Eleven CCI and 12 children with normal hearing completed the following tests of vestibular funct

Multislice computed tomography in the diagnosis of superior canal dehiscence: how much error, and how to minimize it?

HYPOTHESIS: Multi-slice computed tomography (MSCT) overestimates the size of superior semicircular canal dehiscences (SSCDs) and also can misinterpret thin bone over the superior semicircular canal as dehiscent. A threshold of the radiodensity of the bone over the superior semicircular canal may exist that could optimize prediction of an actual SSCD. BACKGROUND: The gold standard for diagnosis of SSCD is MSCT, but there is a higher prevalence of SSCD based on MSCT compared with histologic studies. Overestimation of SSCD can lead to inappropriate diagnosis and treatment. METHODS: We correlate

Endolymphatic hydrops revealed by magnetic resonance imaging in patients with atypical Meniere's disease.

CONCLUSION: Our study showed that patients with atypical Meniere's disease had endolymphatic hydrops (EH) in both the cochlea and the vestibule. Using an imaging method, we provide evidence that atypical Meniere's disease is a true variant of classic Meniere's disease. OBJECTIVES: The aim of present study was to investigate images of the endolymphatic space in patients with atypical Meniere's disease (cochlear and vestibular Meniere's disease). METHODS: Sixty-four patients divided into two groups were enrolled in this study. The first group included 36 patients who had fluctuating hearing lo

Unusual vestibular and visual input in human dynamic balance as a motion sickness susceptibility test.

BACKGROUND: Motion sickness (MS) is commonly thought to arise from a sensory conflict. However, few quantitative methods based on this theory are available to detect MS susceptibility. HYPOTHESIS: It was asked whether the standardized unusual stimulation of a single sensory channel under quantified dynamic balance conditions in man could elicit a sensory conflict and therefore trigger motion sickness (MS) METHODS: Vestibular and visual channels were stimulated by galvanic current and rotating prismatic glasses, respectively. The moving platform used to create the requirements for dynamic bala

Pneumococcal Meningitis Induces Hearing Loss and Cochlear Ossification Modulated by Chemokine Receptors CX3CR1 and CCR2.

PURPOSE: Pneumococcal meningitis is a major cause of hearing loss and permanent neurological impairment despite widely available antimicrobial therapies to control infection. Methods to improve hearing outcomes for those who survive bacterial meningitis remains elusive. We used a mouse model of pneumococcal meningitis to evaluate the impact of mononuclear phagocytes on hearing outcomes and cochlear ossification by altering the expression of CX3CR1 and CCR2 in these infected mice. METHODS: We induced pneumococcal meningitis in approximately 500 C57Bl6 adult mice using live Streptococcus pneumo

Reliability of the vestibular evoked myogenic potential test in assessing intratympanic gentamicin therapy in Meniere's disease.

CONCLUSIONS: The results of this preliminary study demonstrate that with regard to determining the efficacy of intratympanic (IT) gentamicin treatment in patients with Meniere's disease, the reliability of testing for vestibular evoked myogenic potentials (VEMPs) is comparable to that of caloric tests. Compared with caloric tests, VEMP measurements are more comfortable and take less time. The results of VEMP and caloric testing do not correlate with the results of hearing tests. OBJECTIVE: To test the reliability of VEMP testing to monitor the results of IT gentamicin therapy in patients with

Wall-eyed bilateral internuclear ophthalmoplegia in a patient with progressive supranuclear palsy.

Wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) is a rare disorder consisting of a bilateral adduction deficit and primary gaze position exotropia. Associated with bilateral medial longitudinal fasciculus lesions, it has been mostly reported in patients with multiple sclerosis and brainstem stroke. A 72-year-old man with characteristic clinical features of progressive supranuclear palsy (PSP) later developed WEBINO. Brain MRI revealed atrophy of the midbrain tegmentum. Caloric irrigation revealed intact horizontal eye movements in both eyes. We believe this to be the first report of

Th1: mediator lymphocytes in experimental autoimmune labyrinthitis.

A previous study on experimental autoimmune labyrinthitis (EAL) consistently demonstrated transient infiltration of lymphocytes only into the inner ear of mice. To clarify the profile of lymphocytes in the initiation of EAL, the present study investigated cell surface antigens, as well as cytokines, from Day 4 to Day 35, using immunohistochemical techniques. Many CD4+ cells mainly infiltrated the endolymphatic sac as early as Day 4 and gradually spread to the rest of the inner ear. Infiltration peaked on Day 12 and persisted in most animals until Day 35, although the number of cells gradually

Inner ear disorders due to pressure change.

We reviewed the records of 136 patients who had inner ear disorders including hearing loss and vertigo caused by pressure change. We divided them into three groups, according to the aetiology: group A, change in atmospheric pressure (diving, airplane travel, etc.); group B, rapid change in ear pressure in normal atmosphere (nose blowing, heavy lifting, etc.); and group C, blast injury. A flat initial audiogram was the most common type in groups A and B. In group C, high-tone hearing loss was the most common type of audiogram. These results correspond to findings previously reported in animal e
