AIMS: Ventilatory disturbance is a fatal side-effect of opioid analgesics. Separation of analgesia from ventilatory depression is important for therapeutic use of opioids. It has been suggested that opioid-induced ventilatory depression results from a decrease in adenosine 3',5'-cyclic monophosphate content in the respiratory-related neurons. Therefore, we examined the effects of caffeine, a methylxanthine non-selective phosphodiesterase (PDE) inhibitor with adenosine antagonistic activity, and rolipram, a racetam selective PDE4 inhibitor, on ventilatory depression induced by morphine.

Gastroduodenal bleedings were surgically treated in 244 patients with bleeding duodenal ulcers at the regional specialized center of treatment of gastroduodenal bleedings during 5 years. Postoperative lethality was 9.4%. A classification of surgical operations for bleeding ulcers is proposed depending on the time of performing. Substantiation is given to expediency of specialized medical care of such patients at the specialized center.

The duration of ventricular fibrillation (VF) that precedes a high energy shock has been recognized as a critical determinant of defibrillation outcome. Factors such as metabolic acidosis or alkalosis do not affect outcome. Results of the examination of 142 patients with ulcer disease were studied. The incidence of DGR was found to be 48% in gastric ulcer and 44% in duodenal ulcer. The unfavorable influence on the gastric mucosa was greater under conditions of the acidic intragastric medium. Special attention to causes of DGR in the surgical treatment of the ulcer disease and the use of areflux anastomoses can improve results of the surgical treatment of such patients.

Significant quantities of a newly discovered peptide, neuropeptide Y, were found in the rat pancreas, where they were localized to nerves in the exocrine parenchyma and around arterial and ductal structures. Acid secretion in Pavlov pouches in dogs is known to increase after antrectomy in response to histamine and gastrin. Dogs with gastric fistulae were tested with histamine and tetragastrin as a control study. The vagal nerve fibers to the antrum were divided and the dogs underwent repeat testing.
