Neurodevelopmental and growth impairment among extremely low-birth-weight infants with neonatal infection.

CONTEXT: Neonatal infections are frequent complications of extremely low-birth-weight (ELBW) infants receiving intensive care. OBJECTIVE: To determine if neonatal infections in ELBW infants are associated with increased risks of adverse neurodevelopmental and growth sequelae in early childhood. DESIGN, SETTING, AND PARTICIPANTS: Infants weighing 401 to 1000 g at birth (born in 1993-2001) were enrolled in a prospectively collected very low-birth-weight registry at academic medical centers participating in the National Institute of Child Health and Human Development Neonatal Research Network.

Broad-spectrum antibiotics for preterm, prelabour rupture of fetal membranes: the ORACLE I randomised trial. ORACLE Collaborative Group.

BACKGROUND: Preterm, prelabour rupture of the fetal membranes (pPROM) is the commonest antecedent of preterm birth, and can lead to death, neonatal disease, and long-term disability. Previous small trials of antibiotics for pPROM suggested some health benefits for the neonate, but the results were inconclusive. We did a randomised multicentre trial to try to resolve this issue. METHODS: 4826 women with pPROM were randomly assigned 250 mg erythromycin (n=1197), 325 mg co-amoxiclav (250 mg amoxicillin plus 125 mg clavulanic acid; n=1212), both (n=1192), or placebo (n=1225) four times daily for

Factors Influencing Breastfeeding Knowledge and Attitudes Among Parents of Moderate-To-Late Preterm Infants: A Cross-Sectional Study.

BACKGROUND: Prematurity constitutes a major threat to perinatal health, with moderate-to-late preterm infants accounting for over 80% of all preterm births in China. This places significant challenges on breastfeeding for the affected families. As the optimal enteral nutrition for preterm infants, human milk reduces the incidence of necrotizing enterocolitis and fulfills their growth needs. PURPOSE: The aim of this study was to explore factors associated with breastfeeding knowledge and attitudes among parents of moderate-to-late preterm infants in the context of mother-infant separation. Efficacy of Lactobacillus reuteri DSM 17938 in preterm newborns and infants with colic: a systematic review and meta-analysis of randomized controlled trials.

OBJECTIVE: Early-life gut microbiota is crucial for gastrointestinal and immune development. Dysbiosis has been implicated in infantile colic and in complications of prematurity, including feeding intolerance and necrotizing enterocolitis (NEC). Lactobacillus reuteri DSM-17938 has been proposed as a strategy to modulate gut homeostasis during early infancy. METHODS: We conducted a systematic review and meta-analysis of randomized, placebo-controlled trials evaluating single-strain L. reuteri DSM-17938 in newborns and infants (0-12 mo). Superior seizure control with first-line levetiracetam in very low birth weight neonates: a 10-year comparative retrospective study.

OBJECTIVES: Very low birth weight (VLBW) infants are at disproportionately high risk for neonatal seizures, yet optimal first-line antiseizure medication (ASM) strategies in this vulnerable population remain a matter of debate. Over the past decade, levetiracetam (LEV) has emerged as a promising alternative to phenobarbital (PB), driven by growing concerns over PB's limited efficacy and potential neurotoxic effects. METHODS: This retrospective cohort study analyzed medical records of VLBW infants treated for seizures at our Neonatal Intensive Care Unit from 2013 to 2023. We compared the effic

Short Antibiotic Treatment for Coagulase-negative Staphylococcal Sepsis in Premature Infants: A Multicenter Noninferiority Study.

BACKGROUND: Optimizing antibiotic duration in neonatal intensive care units is essential for antimicrobial stewardship and microbiome preservation. However, the safety of short antibiotic courses for uncomplicated late-onset coagulase-negative staphylococcal (CoNS) sepsis remains uncertain. AIM: To determine whether short (≤96 hours) antibiotic treatment of uncomplicated CoNS sepsis in premature infants admitted to a neonatal intensive care unit is noninferior to long treatment (>96 hours) by comparing relapse rates. METHODS: This multicenter, observational cohort study reviewed all proven n

Prepregnancy Obesity Versus Excessive Gestational Weight Gain: A Comparative Evaluation of Adverse Outcomes.

Background/Objectives: Obesity and excessive gestational weight gain (EGWG) are known risk factors for adverse outcomes in pregnancy, but the comparative risk is less well defined and increasingly relevant in an era of increasing use of weight loss medications. We aimed to determine whether EGWG was associated with adverse maternal and neonatal outcomes, compared with prepregnancy obesity. Methods: Multicenter retrospective cohort study of pregnant patients who delivered a singleton within a single university health system from 2019-2024. Patients with missing prepregnancy body mass index (BMI

Nanozymes in the Treatment of Pediatric Inflammatory Diseases: Opportunities and Challenges.

Pediatric inflammatory diseases, such as neonatal necrotizing enterocolitis (NEC), juvenile idiopathic arthritis (JIA), and asthma, pose numerous challenges in clinical treatment due to their complex pathogenesis and the still-developing stage of children's immune systems. These challenges include poor precision, strong toxic and side effects, and unclear long-term biological safety. In recent years, the development of nanozymes has provided new opportunities to address these issues. This review systematically summarizes the latest research progress of nanozymes in the diagnosis and treatment

Fluid Management in ELGANs: Striking the Perfect Balance!

Managing fluid and electrolytes in extremely low gestational age neonates (ELGANs) is often challenging because of their distinctive fluid physiology. Most of the fluid loss in the first week of life is trans-epidermal due to the immature barrier function of the skin. ELGANs also have a developmental tendency for exaggerated diuresis and natriuresis. Allowing an initial weight loss of 6-12% promotes physiological extracellular contraction. Also, restricted fluid intake in the first week of life may decrease the incidence of bronchopulmonary dysplasia, patent ductus arteriosus, and necrotizing

Impact and Citation Trends of Surveys Endorsed by the EUPSA Network Office in Pediatric Surgery: A Bibliometric Analysis.

The European Pediatric Surgeons' Association (EUPSA) Network Office regularly endorses surveys that address controversial topics in pediatric surgery. However, the scientific impact of these within the medical literature remains unclear. To address this gap, we conducted a bibliometric analysis of all published EUPSA-endorsed surveys.Surveys endorsed by the EUPSA Network Office were reviewed for topic, journal, participation, bibliometric indicators (total number of citations and citations per article and year), and citing countries. Simple linear regression was used to determine citation time
