Quantitative assessment of fracture healing using patient-specific CT finite element analysis: A novel approach to external fixation management.

BACKGROUND: Posttreatment decisions regarding fracture care primarily rely on physicians' subjective judgment based on clinical and imaging findings. This study aimed to evaluate whether patient-specific computed tomography (CT) finite element analysis could allow for the quantitative assessment of fracture site strength as an objective indicator of treatment decisions. METHODS: Our patient was a 37-year-old male who underwent external fixation for an open comminuted tibial fracture of the right lower leg. Monthly CT scans were taken starting 2 months postinjury. Patient-specific CT finite el

Fusion strategy after odontoid fracture and three-column atlantoaxial displacement in a patient with prior C2-pelvis fusion: a case report.

BACKGROUND: Surgical intervention for scoliosis often involves extensive spinal fusions, at times spanning from the cervical spine to the pelvis. Proximal junctional failure is a known complication of C2-pelvis fusions, frequently presenting as fractures of the odontoid process. Managing these fractures is complex due to anatomical constraints and the need for structural stability while minimizing risks. This report presents a nuanced solution for a C2 fracture involving all three columns and C1 displacement in a patient with previous anterior C3-C6 and posterior C2-pelvis fusions. CASE DESCR

Biomechanical Challenges of Olecranon Fractures in Patients With Parkinson's Disease: A Case Analysis of Neuro-Orthopedic Collaborative Intervention Strategies.

In the intricate landscape of Parkinson's disease (PD), patients face a notably elevated risk of fixation failure due to the motor dysfunction characteristic of this condition. This narrative presents a compelling case of recurrent olecranon fractures in a patient with PD. It highlights the clinical implications of a neuro-orthopedic collaborative strategy that is meticulously anchored in quantitative assessment. A 71-year-old male with PD, Unified Parkinson's Disease Rating Scale III (UPDRS III) score 45 and modified Ashworth scale grade 2, sustained a comminuted fracture of the right olecran

Guided tissue regeneration with heterologous materials in primary subtalar arthrodesis: a case report.

BACKGROUND: Calcaneal fractures are relatively rare and difficult to treat. Treatment options vary based on the type of fracture and the surgeon's experiences. In recent years, surgical procedures have increasingly been used due to the better long-term results. We present a case where guided tissue regeneration was performed in a calcaneal fracture that needed primary subtalar arthrodesis. We used the principles of guided tissue regeneration from oral surgery to perform primary subtalar arthrodesis and minimize the risk of non-union. We used a heterologous collagen membrane, which acts as a me

Use of Ilizarov Fixator for Grade III B Open Olecranon Fracture: a Case Report and Surgical Technique.

INTRODUCTION: External fixator application can be difficult for olecranon fractures in presence of large degloving injuries. We describe use of simple Ilizarov ring fixator construct for grade IIIB open olecranon fracture management. CASE REPORT: A 45-year-old female with Grade III B open comminuted olecranon fracture (30*15cm degloving area) and ulnar nerve palsy was treated with a novel ring fixator construct. Two cut-end olive wires were passed from the proximal olecranon across the fracture site in intramedullary fashion exiting dorsally at mid-ulnar level through healthy skin and were at

Hybrid cadaveric/surrogate model of thoracolumbar spine injury due to simulated fall from height.

A fall from high height can cause thoracolumbar spine fracture with retropulsion of endplate fragments into the canal leading to neurological deficit. Our objectives were to develop a hybrid cadaveric/surrogate model for producing thoracolumbar spine injury during simulated fall from height, evaluate the feasibility and performance of the model, and compare injuries with those observed clinically. Our model consisted of a 3-vertebra human lumbar specimen (L3-L4-L5) stabilized with muscle force replication and mounted within an impact dummy. The model was subjected to a fall from height of 2.2

[Locked volar plating for complex distal radius fractures: maintaining radial length].

PURPOSE OF THE STUDY: Maintaining radial length, likely to be the main challenge in the treatment of complex distal radius fractures, is necessary for complete grip-strength and pro-supination range recovery. In spite of frequent secondary displacements, bridging external-fixation has remained the reference method, either isolated or in association with additional percutaneous pins or volar plating. Also, there seems to be a relation between algodystrophy and the duration of traction applied on the radio-carpal joint. Fixed-angle volar plating offers the advantage of maintaining the reduction

OSTEOSYNTHESIS OF CLAVICLE FRACTURES IN CHILDREN USING TITANIUM ELASTIC NAILS.

OBJECTIVES: Titanium Elastic Nail (TEN) fixation provides superior clinical and cosmetic outcomes with fewer complications compared to plating or Kirschner wire fixation in pediatric clavicle fractures. BACKGROUND: Clavicle fractures account for about 10% of all paediatric fractures. In displaced or unstable cases, surgical fixation is increasingly advocated. Titanium elastic nails (TEN) offer a minimally invasive intramedullary fixation option, but outcomes in children remain less widely documented. DESIGN: Retrospective cohort design study. SETTING: Single tertiary care center. PATIENT S

Management of intraoperative coracoid fractures in the arthroscopic Latarjet procedure.

BACKGROUND: A marked increase in the number of Latarjet procedures performed has been observed and, with that, a rise in the number of complications, with intraoperative coracoid fracture being reported as one of the most common complications, especially in the arthroscopic approach. There is a scarcity of studies reporting on these complications; thus, the techniques and results are unclear. METHODS: Retrospective case series of patients with intraoperative coracoid graft fractures during arthroscopic Latarjet performed between 2010 and 2023. In this report, we described a new coracoid graft

Risk factors for complications and reoperation following operative management of displaced midshaft clavicle fractures.

BACKGROUND: Optimal management of a displaced midshaft clavicle fracture remains controversial. This study assessed demographic factors, fracture pattern, and surgical technique as potential predictors of surgical complications. Smoking, diabetes, obesity, polytrauma, high-energy mechanism, inpatient status, transverse or comminuted fractures, and single-plating technique were hypothesized to be associated with an increased risk of complications following clavicle fracture open reduction internal fixation (ORIF). METHODS: Consecutive patients with minimum 12-week follow-up from the trauma and

Posttraumatic enophthalmos: etiology, principles of reconstruction, and correction.

Enophthalmos is defined as a backward, and usually downward, displacement of the globe into the bony orbit. In posttraumatic enophthalmos, the mechanisms that determine globe position are: 1) the enlargement of the orbital cavity; 2) the herniation of orbital fat into the maxillary sinus; and 3) fat atrophy, loss of ligament support, and scar contracture. The aim of this article is to analyze the strategies to prevent enophthalmos and to correct late posttraumatic enophthalmos. In this study, 80 patients (52 cases of orbitozygomatic fractures and 28 late posttraumatic enophthalmos) were treate

Humeral Fractures in Metabolically Compromised Bone: Outcomes of Structural Strut Onlay Allograft with Open Reduction and Internal Fixation - A Case Series of Three Patients.

INTRODUCTION: The "terrible tetrad" of the elbow - simultaneous posterior ulno-humeral dislocation, comminuted radial head fracture with dislocation, coronoid process fracture, and proximal ulnar fracture - is an injury pattern that simultaneously fulfills criteria for both the terrible triad and a Monteggia-type fracture-dislocation. Fewer than fifteen cases have been documented worldwide. We present two cases managed by different surgeons at different institutions and propose the formal adoption of the term "terrible tetrad." CASE REPORT: Case 1: A 50-year-old male painter sustained a right

Distal Tibial Atrophic Pseudoarthrosis Treated with Vascularized Fibular Graft and Ankle Arthrodesis Nail: A Multidisciplinary Orthoplastic Case Report.

INTRODUCTION: Tibial pseudoarthrosis is an uncommon but serious complication following high-energy trauma, particularly in complex distal tibial fractures such as pilon fractures. Atrophic pseudoarthrosis, characterized by avascular bone ends and a lack of biological activity, is often associated with risk factors such as smoking, soft-tissue damage, and multiple surgical interventions. Its management is particularly challenging, requiring a tailored approach that restores both mechanical stability and the biological environment for bone healing. Orthoplastic strategies have emerged as essenti
