[Efficacy and side-effects of long term L-dopa therapy in Parkinsonian syndromes (evaluated by CT scan as well) (author's transl)].

The evolution of symptoms and therapeutic response in a group of have been followed parkinsonian patients 81 for a period of 4 years. Hoehn and Yahr, and Webster's scales have been used. In addition CT scan, EEG, ECG, blood and urine analyses have been carried out. Treatment was carried out with L-Dopa and decarboxylase inhibitor (benserazide for 50% of cases and carbidopa for the other 50%). Anticholinergics (esp. metixene) were often required. The initial response was satisfactory for all patients except one who had hydrocephalus (with normal pressure). Four years later, the initial improvem

Morphological analysis of progressive hydrocephalus and shunt-dependent arrested hydrocephalus. An experimental study.

This experimental study was performed to determine if surgically treated feline hydrocephalus could produce any morphological and physiological changes in the periventricular tissue. The result was analyzed with clinical outcome, comparing two differently prepared models in which the biomechanical characteristics of the container property of the brain were altered. Craniectomies were performed in adult mongrel cats and the dura mater was left untouched in group A, while the dura was incised in crucial fashion in group B. Thirty-four animals underwent ventriculopleural shunt surgery 6-8 weeks a

Glymphatic MRI techniques in sleep and neurodegenerative diseases.

PURPOSE OF REVIEW: The purpose of this review article is to summarize the current in-vivo imaging techniques for the evaluation of the glymphatic function and discuss the factors influencing the glymphatic function and research directions in the future. RECENT FINDINGS: The glymphatic system allows the clearance of metabolic waste from the central nervous system (CNS). The glymphatic pathway has been investigated using intrathecal or intravenous injection of a gadolinium-based contrast agent (GBCA) on MRI, so-called glymphatic MRI. The glymphatic MRI indirectly visualizes the dynamic CSF flow

Human Cerebrospinal fluid promotes long-term neuronal viability and network function in human neocortical organotypic brain slice cultures.

Pathophysiological investigation of CNS-related diseases, such as epilepsy or neurodegenerative disorders, largely relies on histological studies on human post mortem tissue, tissue obtained by biopsy or resective surgery and on studies using disease models including animal models, heterologous expression systems or cell culture based approaches. However, in general it remains elusive to what extent results obtained in model systems can be directly translated to the human brain, calling for strategies allowing validation or even primary investigation in live human CNS tissue. In the work repor

B-cell-activating factor belonging to the tumor necrosis factor family (BAFF) and a proliferation-inducing ligand (APRIL) levels in cerebrospinal fluid of patients with meningoencephalitis.

The B-cell-activating factor belonging to the tumor necrosis factor family (BAFF) and a proliferation-inducing ligand (APRIL) are important factors for the survival of transitional and mature B cells. High levels of BAFF and APRIL are present in adults with several autoimmune diseases. However, there are few reports about BAFF and APRIL levels in the cerebrospinal fluid (CSF) of patients with meningoencephalitis. We evaluated BAFF and APRIL levels in CSF samples from patients with viral meningitis (VM) (28 patients), autoimmune encephalitis (AE) associated with antineuronal antibodies (15 pati

Adrenal haemangioblastoma presenting as phaeochromocytoma: a rare manifestation of extraneural hemangioblastoma.

PURPOSE: Adrenal haemangioblastoma presenting clinically as pheochromocytoma is a rare manifestation of extraneural haemangioblastoma. We present an unusual case of von Hippel-Lindau (VHL) disease that had adrenal and cerebellar haemangioblastoma with multiple renal cysts, and a review of the literature. METHODS: Unlike the usual manifestations of secondary polycythemia or increased intracranial pressure and hydrocephalus due to cerebellar lesion, this 36-year-old male presented with hypertension. Investigations revealed right suprarenal mass with raised urinary catecholamines and serum vanil

Some observations on the spectrum of dementia.

A study was designed to generate epidemiological and clinical data on dementia, in a teaching hospital in India. It was conducted on 124 (94 male and 30 female) elderly patients (aged more than 60 years) presenting with clinical syndrome of dementia (DSM-3). Their age range was 64-78 (mean 65.7 4.1) years. Detailed clinical, biochemical, radiological and electrophysiological evaluation was done to establish etiology. Patients with psychiatric ailments, cranial trauma and tumors were excluded. The study period was 4.2 years. Multi-infarct dementia (MID) was observed to be commonest cause of dem

Clinical and demographic features of patients with dementia attended in a tertiary outpatient clinic.

We describe clinical and socio-demographic features of patients with dementia attended in a tertiary outpatient clinic during a three years period (56.9% of the total attendance). Most of them were men, white, from the local community, urban district. Nobody had a job at the moment, two thirds of them got social welfare benefit. They lived with their family, the caregiver being the spouse or a daughter. The education level was very low, a quarter of them being illiterate. They were referred mostly from the public health care service, by neurologists or psychiatrists due to cognitive disorders.

[Metotopic craniosynostoses].

BACKGROUND: Premature closure of the metopic suture leads to inhibited growth of the frontal bones, producing a keel-shaped forehead (trigonocephaly). Simple metopic synostosis is usually sporadic. Trigonocephalic patients account for 8-16% of the referrals to craniofacial centers, with a marked male predominance. Intracranial pressure (ICP) may be increased, whereas shunt-dependent hydrocephalus is infrequent. Infrequently, patients have intra- or extracerebral anomalies; one third have varying degrees of neuropsychological problems. The treatment is primarily surgical. MATERIAL AND METHODS:

Intestinal pseudo-obstruction in myotonic dystrophy.

We describe four myotonic dystrophy (DM) patients who developed recurrent intestinal pseudo-obstruction. Some episodes were associated with gastroenteritis, while abdominal crowding may have occurred in one case during the third trimester of pregnancy. In most instances, however, no apparent cause could be identified. Intestinal pseudo-obstruction may occur at any stage of DM. In one of our cases intestinal pseudo-obstruction preceded significant muscle weakness by 15 years. Intestinal pseudo-obstruction is usually treated effectively with conservative measures. These include restriction of or

Gait Improvement Following CSF Tap Test in NPH Patients With and Without Striatal Dopaminergic Deficit: A Preliminary Study.

BACKGROUND AND OBJECTIVES: Recent studies have highlighted the occurrence of reduced striatal signal intensity on dopamine transporter (DAT) scans in patients with idiopathic normal pressure hydrocephalus (iNPH). The aim of our study was to investigate whether the extent of symptom improvement after a CSF tap test (TT) differs between iNPH patients with reduced striatal DAT uptake and those with normal uptake. METHODS: We conducted gait analysis on 44 patients with iNPH who underwent DAT scans at Seoul National University Bundang Hospital (SNUBH) and Kyungpook National University Chilgok Hosp

[A family of hereditary communicating hydrocephalus with trigonocephaly].

We studied a family of hereditary hydrocephalus with trigonocephaly, affecting eight members (four males and four females) across two generations. A proband in this family was 51 year old man, who complained of nonprogressive gait disturbance after the age of 20 years. He was admitted to our hospital because of non-neurological disease. Accidentally urinary incontinence appeared and then CT scan was performed, which revealed marked dilatation of the lateral ventricles. He was short stature and showed trigonocephalic forehead, but his head circumference was 56 cm. Mild dementia, spastic gait an

d-serine levels in Alzheimer's disease: implications for novel biomarker development.

Alzheimer's disease (AD) is a severe neurodegenerative disorder still in search of effective methods of diagnosis. Altered levels of the NMDA receptor co-agonist, d-serine, have been associated with neurological disorders, including schizophrenia and epilepsy. However, whether d-serine levels are deregulated in AD remains elusive. Here, we first measured D-serine levels in post-mortem hippocampal and cortical samples from nondemented subjects (n=8) and AD patients (n=14). We next determined d-serine levels in experimental models of AD, including wild-type rats and mice that received intracereb

Diffusion tensor imaging in hydrocephalus--findings before and after shunt surgery.

BACKGROUND: To evaluate changes in diffusion tensor imaging (DTI)-derived parameters in patients with hydrocephalus (HC) before and several weeks after shunt surgery. METHODS: Thirteen HC patients were examined with DTI before and after shunt surgery. In a combined region of interest and whole brain voxel-based analysis, different DTI parameters were compared with an age-matched control group. RESULTS: Alteration of DTI parameters in HC patients and changes after shunt surgery are regionally different. HC patients show an increase in fractional anisotropy values based on increases in paralle

[An autopsy case of cryptococcal meningoencephalitis with AIDS: correspondence between MRI and pathological findings of basal ganglia and cerebellum].

We reported a 71-year-old male who showed subacute progression in cognitive decline and gait disturbance. Cystic lesions in the basal ganglia and a high signal in the right cerebellar hemisphere were detected respectively on MRI fluid-attenuated inversion recovery (FLAIR) image and diffusion weighted image (DWI) taken at 7 days after admission to our hospital. Dilatation of the Virchow-Robin space and fresh cerebellar infarction were suspected. Since an examination of cerebrospinal fluid (CSF) did not reveal any significant features, diagnosis of this patient proved very difficult. His cellula

Results of cranial vault reshaping.

This study was designed to assess the outcome of cranial vault reshaping for correction of deformity of the skull and the upper face. A retrospective review of all children who underwent cranial vault reshaping by a single team of surgeons between 1993 and 1996 was performed. There were 10 children in the series. The age at surgery ranged from 6 to 62 months (mean age, 25 months). Five children in the series had untreated sagittal craniosynostosis with scaphocephaly, two had pansynostosis resulting in cloverleaf skull deformity, and three had turricephaly after shunt treatment of hydrocephalus

A new method and software for quantitative analysis of continuous intracranial pressure recordings.

OBJECTIVE: A computer software utilising a new method for quantitative analysis of intracranial pressure (ICP), was developed to provide a more accurate analysis of continuously recorded ICP. Intracranial pressure curves were analysed by the software to explore the relationship between mean ICP and the presence of ICP elevations. METHODS: The Sensometrics Pressure Analyser (version 1.2) software provides a quantitative analysis of the ICP curve, presenting the ICP recordings as a matrix of numbers of ICP elevations of different levels (e.g. 20 or 30 or 40 mmHg) and durations (e.g. 0.5, 5 or 1
