This review explores a range of imaging techniques used in the pre-surgical planning of vascular access, including duplex ultrasound (DUS), digital subtraction angiography (DSA), digital subtraction venography (DSV), CO2 Venography, magnetic resonance angiography (MRA), computed tomography angiography (CTA), and Intravascular ultrasound (IVUS). For each modality, we analyze its technical background, applications, advantages and disadvantages, and comparisons with alternative imaging options. DUS is the most widely used imaging modality in pre-surgical planning due to its low cost, non-invasive

BACKGROUND: Ultrasonography is the primary diagnostic imaging modality for upper extremity deep vein thrombosis related to peripherally inserted central venous catheters (PICCs). Computed tomography (CT) venography may offer higher sensitivity while additionally providing information about the superior vena cava and central pulmonary arteries.
OBJECTIVES: We compared the diagnostic accuracy of CT venography with ultrasonography for screen-detected PICC-related venous thromboembolism (VTE).
METHODS: Consecutive adult patients with cancer were enrolled before PICC placement. On day 20 ± 5, scr

BACKGROUND: Stenting of the iliofemoral vein may be an effective treatment to improve post-thrombotic symptoms. Iliofemoral vein stents have requirements different from those of lower extremity artery stents, and there is a paucity of literature regarding the biomechanical motion of the iliofemoral vein.
METHODS: In a novel cadaveric model, stents were bilaterally inserted into the veins in the iliofemoral region. The veins were pressurized and underwent computed tomography angiography at various hip angle positions. In addition, 21 patients with iliofemoral vein disease had supine computed t

BACKGROUND: The utility of pressure waveform analyses to assess pulmonary vein (PV) occlusions has been reported in cryoballoon PV isolation (CB-PVI) using first-generation CBs. This prospective randomized study compared the procedural and clinical outcomes of pressure-guided and conventional CB-PVI.
METHODS AND RESULTS: Sixty patients with paroxysmal atrial fibrillation underwent CB-PVI with 28-mm second-generation CBs. PV occlusions were assessed either by real-time pressure waveforms without contrast utilization (pressure-guided group) or contrast injections (conventional group) and random

BACKGROUND: Superior mesenteric vein-portal vein confluence resection combined with pancreatoduodenectomy (SMPVrPD) is occasionally required for resection of pancreatic head tumors. It remains unclear whether such situations require splenic vein (SV) reconstruction for decompression of left-sided portal hypertension (LSPH).
METHODS: The data from 93 of 104 patients who underwent pancreatoduodenectomy (PD) for pancreatic head malignancies were reviewed. Surgical outcomes in three groups-standard PD (control group), PD combined with vascular resection and SV preservation (SVp group), and SMPVrP

Patients with prolonged history of central venous catheters can develop central venous occlusion. We report the creation of an axillary artery to right atrial graft using the femoropopliteal vein (FPV) for dialysis access in a patient with end-stage renal disease on hemodialysis with history of central venous occlusion. The patient had previously developed steal syndrome following a right brachial artery to right atrial polytetrafluoroethylene graft requiring revision to a left axillary artery inflow which subsequently thrombosed. This is the first report of using an autologous FPV conduit to

BACKGROUND: Estimates of effective dose (E) for cardiovascular CT are obtained from a scanner-provided dose metric, the dose-length product (DLP), and a conversion factor. These estimates may not adequately represent the risk of a specific scan to obese adults.
OBJECTIVE: Our objective was to create dose maps sensitive to patient size and anatomy in the irradiated region from a patient's own CT images and compare measured E (EDoseMap) to doses determined from standard DLP conversion (EDLP) in obese adults.
METHODS: 21 obese patients (mean body mass index, 39 kg/m(2)) underwent CT of the pulm

Pulmonary embolism (PE) associated with duplicated inferior vena cava (IVC) is rare, and there are no reports of this condition treated with catheter-directed thrombolysis. We present the case of a 54-year-old man who developed massive PE caused by thrombi in a duplicated IVC that detached during transcatheter angiography. After implantation of a retrievable filter in the common IVC, the patient underwent catheter-directed thrombolysis. He was asymptomatic at discharge, with complete resolution of thrombosis and successful retrieval of the filter. The patient had an uneventful 9-month follow-u

PURPOSE: Ultrasound (US)-guided internal jugular vein access has been the standard practice of central venous port (CVP) placement. The subclavian vein (SCV) access has also been preferred, but has potential risk of pinch-off syndrome (POS). The purpose of this study was to examine the effect of US-guided SCV access to avoid POS in patients with CVP.
METHODS: Included in this study were patients who had undergone CVP placement via the SCV. We mainly assessed the computed tomography (CT) findings from two different placement techniques of a CVP via the SCV: (i) venipuncture point described by
