Conduction system abnormalities in patients with obstructive hypertrophic cardiomyopathy following septal reduction interventions.

We observed the impact of percutaneous transluminal septal myocardial ablation (PTSMA) and myectomy on the conduction system in patients with obstructive hypertrophic cardiomyopathy (HC). Septal reduction intervention is capable of eliminating the left ventricular outflow tract obstruction in patients with obstructive HC; however, conduction system abnormalities are frequent consequences of these procedures. A standard 12-lead electrocardiogram and Doppler echocardiogram were obtained in 204 patients who underwent PTSMA (n = 70) or myectomy (n = 134) before and at average of 3 months after int

Adjunctive dobutamine in patients with septic cardiomyopathy and tissue hypoperfusion: a blinded randomised controlled multicentre trial study protocol of the ADAPT-dobutamine trial.

INTRODUCTION: Sepsis-induced left ventricular (LV) dysfunction participates in cardiovascular dysfunction and associated organ failure in patients with septic shock. The tested hypothesis is that dobutamine will reduce tissue hypoperfusion and secondary organ dysfunction in increasing oxygen delivery in fluid-filled patients with septic shock and associated symptomatic septic cardiomyopathy with documented low-flow state. METHODS AND ANALYSIS: ADAPT-Dobutamine is a blinded, two parallel group, add-on, multicentre, randomised 1:1 and placebo-controlled trial. Patients will be included if hospi

Alcohol septal ablation in obstructive acromegalic hypertrophic cardiomyopathy - a first case report.

Acromegaly is a rare disease, mostly caused by a growth hormone (GH)-secreting benign pituitary tumor, with an increased production of GH and insulin-like growth factor 1 (IGF-1). Cardiovascular complications are common and are associated with cardiomyocyte apoptosis and concentric cardiac hypertrophy. Suppression of GH and IGF-1 appears to improve cardiac function only in the short term, with little or no decrease in left ventricular mass or improvement in cardiac function after prolonged treatment. Alcohol septal ablation (ASA) has emerged as a minimally invasive alternative to septal myecto

Surgical management of infants with isolated supravalvular pulmonary stenosis: case reports.

Pulmonary stenosis (PS) can be seen from the right ventricular outflow tract to the peripheral pulmonary arteries. Most frequently, the obstruction occurs at the level of the pulmonary valve; however, it occurs less frequently at the infindibular level within the trabecular component of the right ventricle or within the pulmonary arterial pathways. Lesions at any of these levels can occur as part of more congenital cardiac malformations such as tetralogy of Fallot, complete transposition of great arteries, or atrial septal defect. Isolated supravalvular pulmonary stenosis (iSPS) is less common

Improvement in gastrointestinal bleeding after septal myectomy for hypertrophic cardiomyopathy.

OBJECTIVE: Patients with obstructive hypertrophic cardiomyopathy may have occult gastrointestinal bleeding. In this study, we analyzed outcomes of septal myectomy in patients who had a history of gastrointestinal bleeding preoperatively to understand patient characteristics and impact of septal reduction on recurrent gastrointestinal bleeding. METHODS: We analyzed 73 adult patients who had a history of gastrointestinal bleeding before transaortic septal myectomy for obstructive hypertrophic cardiomyopathy and compared outcomes to 219 patients without gastrointestinal bleeding preoperatively.

Endogenous catecholamine levels and function of the systemic right ventricle following atrial switch.

BACKGROUND: As a result of successful surgery there is a large cohort of young adults with systemic right ventricles, suffering from progressive right ventricular dysfunction. Beta-blockers are successfully used in left ventricular failure. We assessed catecholamine levels and related it to cardiac function late following atrial switch, to provide the pathophysiologic basis for pharmacotherapy in patients with systemic right ventricles. METHODS: It was a prospective study of 23 consecutive adult patients with the complete transposition of the great arteries, following atrial switch. Epinephri

Clinical outcomes and indicators of normalization of left ventricular dimensions after Ross procedure in children.

Between 1993 and 2000, 50 patients (age range, 1 month to 18 years) who had left ventricular outflow tract (LVOT) disease and had undergone a Ross procedure were assessed using echocardiography. Aortic annulus size, valvular gradient, valve insufficiency, LV dimensions at end-systole and end-diastole, LV interventricular septal and posterior wall thickness, and LV mass index (LVMI) were measured. There was 1 early and 2 late deaths, and 5 reoperations. Aortic annulus size increased and degree of aortic insufficiency (AI) decreased, as did LV internal dimensions and LVMI. Peak pressure gradient

The Evolving Landscape of Hypertrophic Cardiomyopathy Treatment: Are Guidelines Keeping Pace?

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a common inherited cardiovascular disorder associated with morbidity. Rapid advances in therapeutics-particularly the emergence of cardiac myosin inhibitors (CMIs)-suggest that current management guidelines are lagging best practices. AREAS OF UNCERTAINTY: Cardiac myosin inhibitors have revolutionized the management of patients with obstructive HCM. Patients with nonobstructive HCM have had limited therapeutic options historically; however, the recently published ACACIA-HCM trial demonstrated significant benefit in these patients and may repres

Impact of septal myectomy on left atrial volume and left ventricular diastolic filling patterns: an echocardiographic study of young patients with obstructive hypertrophic cardiomyopathy.

BACKGROUND: Septal myectomy in young patients with obstructive hypertrophic cardiomyopathy (HCM) can be performed with low mortality, and leads to excellent improvement in symptoms. Left atrial (LA) enlargement has been associated with exercise intolerance, severity of mitral regurgitation (MR), and left ventricular (LV) outflow tract (LVOT) obstruction in adults with HCM. METHODS: Young patients (1-22 years of age) who had a septal myectomy for obstructive HCM between 2002 and 2005 were identified. Retrospective analyses of premyectomy and postmyectomy echocardiograms were performed. RESULT

Durability of the Arterial Switch Operation: Long-Term Survival and Structural Sequelae in a Multicentre Cohort.

BACKGROUND: The arterial switch operation (ASO) is the standard repair for complete transposition of the great arteries (TGA), but long-term data across large contemporary cohorts remain limited. In this study we evaluated 30-year survival, reintervention, and structural sequelae after ASO and identified factors that influenced late outcomes. METHODS: A total of 1125 patients, 652 with TGA with intact ventricular septum, 389 with TGA with ventricular septal defect, and 84 with double-outlet right ventricle with Taussig-Bing anomaly who underwent ASO between 1990 and 2015 at 10 tertiary centre

Anesthetic Management for Valve-in-Valve Transcatheter Aortic Valve Replacement (TAVR) After Primary TAVR Failure With Mixed Aortic Valve Dysfunction.

Redo valve-in-valve transcatheter aortic valve replacement (TAVR) confronts anesthesiologists with compounded hemodynamic and neurologic risk. We managed an 85-year-old male with severe mixed prosthetic aortic dysfunction whose pre-procedural transthoracic echocardiogram showed a peak velocity of 3.7 m/s⁻¹, a mean gradient of 24 mm Hg, an effective orifice area of 0.8 cm², and a posteriorly directed, eccentric regurgitant jet filling >50% of the left ventricular outflow tract. The failure was suspected to have been caused by structural valve deterioration within five years after the initial TA

Long-term assessment of clinical outcomes and disease progression in patients with corrected Tetralogy of Fallot.

OBJECTIVE: Understanding changes of right ventricular (RV) geometry and function in repaired Tetralogy of Fallot (rToF) patients can improve decision-making for pulmonary valve replacement. Therefore, we aimed to assess the magnitude and clinical correlations of RV changes in rToF patients. PATIENTS AND METHODS: Clinical and MRI data of rToF patients who underwent repeated cardiac magnetic resonance imaging (MRI) at two centers between December 2003 and September 2020 were analyzed together with anatomical factors, including RV outflow tract obstruction, pulmonary artery branch stenosis, and

Impact of pulmonary valve replacement on left ventricular rotational mechanics in repaired tetralogy of Fallot.

BACKGROUND: In repaired tetralogy of Fallot (rTOF), abnormal left ventricular (LV) rotational mechanics are associated with adverse clinical outcomes. We performed a comprehensive analysis of LV rotational mechanics in rTOF patients using cardiac magnetic resonance (CMR) prior to and following surgical pulmonary valve replacement (PVR). METHODS: In this single center retrospective study, we identified rTOF patients who (1) had both a CMR ≤ 1 year before PVR and ≤ 5 years after PVR, (2) had no other intervening procedure between CMRs, (3) had a body surface area > 1.0 m2 at CMR, and (4) had im

Comparison of valvar and right ventricular function following transcatheter and surgical pulmonary valve replacement.

OBJECTIVE: Trans-catheter (TC) pulmonary valve replacement (PVR) has become common practice for patients with right ventricular outflow tract obstruction (RVOTO) and/or pulmonic insufficiency (PI). Our aim was to compare PVR and right ventricular (RV) function of patients who received TC vs surgical PVR. DESIGN: Retrospective review of echocardiograms obtained at three time points: before, immediately after PVR, and most recent. PATIENTS: Sixty-two patients (median age 19 years, median follow-up 25 months) following TC (N = 32) or surgical (N = 30) PVR at Yale-New Haven Hospital were include

Right ventricular infundibulum sparing (RVIS) tetralogy of fallot repair: a review of over 300 patients.

OBJECTIVE: The natural history of standard (large, transmural right ventriculotomy) repair of tetralogy of Fallot (TOF) is associated with a concerning incidence of right ventricular (RV) failure and reoperation. We believe preserving the infundibulum using a TOF repair method with a mini-(<5 mm) or no ventricular incision optimizes RV function and confers long-term benefit. Over the past 13 years, we have uniformly applied this RV infundibulum sparing (RVIS) strategy. METHODS: Using a retrospective cohort study design, 304 TOF patients who underwent the RVIS strategy (July 1995-June 2008) we

Previous palliation in patients with tetralogy of Fallot does not influence the outcome of later repair.

OBJECTIVE: Primary repair is the treatment of choice in patients with tetralogy of Fallot. The timing of repair, however, remains controversial, and an initial palliative procedure might be considered a valuable option in the early management of symptomatic young infants and in those with either unfavourable anatomy, major associated lesions or chromosomal abnormalities with a poor life expectancy. METHODS: We reviewed the management of 100 consecutive patients with tetralogy of Fallot who were operated upon at our department during an 8-year period from June 1995 to March 2003. The rationale

Damus-Rastelli procedure for biventricular repair of aortic atresia and hypoplasia.

BACKGROUND: Biventricular repair (BVR) can be achieved in aortic atresia with ventricular septal defect (VSD) by creating a double outlet left ventricle, Damus-Kaye-Stansel procedure and placement of a right ventricular-pulmonary artery conduit. This study is a review of 15 years experience with this "Damus-Rastelli" technique to assess clinical outcomes in comparison with a standard univentricular approach. METHODS: A review of 16 patients with aortic atresia or complex left ventricular outflow tract obstruction who underwent BVR between 1990 and 2005; a comparison with outcomes for the Norw
