Effectiveness of brachytherapy in treating carcinoma of the vulva.

PURPOSE: Radical radiotherapeutic management of vulvar cancer often incorporates brachytherapy as a portion of the treatment regimen. However, few studies using this modality alone to manage vulvar cancer have been published. METHODS AND MATERIALS: Thirty four patients were treated with iridium-192 (192Ir) brachytherapy for vulvar cancer between 1975 and 1993 at Centre Alexis Vautrin. Twenty-one patients were treated at first presentation when surgery was contraindicated or declined. Of these patients, 12 had International Federation of Gynecology and Obstetrics Classification Stage III or IV

Sex and organ-specific risk and temporal trends of human papillomavirus-associated anogenital cancer among solid organ transplant recipients in the United States.

Solid organ transplant recipients (SOTRs) have an elevated risk of persistent infection of human papillomavirus (HPV) and associated anogenital (cervical, vulva, and vaginal among women, penile among men and anal among both men and women) cancers. Risk stratification and temporal trends of cancer incidence will help early detection, evaluation, management, and treatment of post-transplant cancers over time. We analyzed data of 239,613 heart, lung, liver, and kidney transplant recipients registered from 1987 to 2020 in the U.S. Scientific Registry of Transplant Recipients (SRTR) with cancer dia

Surgical Treatment and Outcomes of Patients With Extramammary Paget Disease: A Cohort Study.

BACKGROUND: Extramammary Paget disease (EMPD) is a rare intraepithelial adenocarcinoma occurring mainly in the anogenital region. Traditional management with wide local excision has shown high recurrence rates, thus Mohs micrographic surgery (MMS) has emerged as a promising treatment option. OBJECTIVE: To compare long-term outcomes after treatment with MMS or excision for primary EMPD. METHODS AND MATERIALS: Retrospective cohort review was conducted for 207 patients with EMPD treated at Mayo Clinic in Rochester, MN, between 1961 and 2012. RESULTS: Of the 25 patients treated with MMS, 19 pri

Prevalence of oncogenic human papillomavirus genotypes in patients diagnosed with anogenital malignancies in Botswana.

BACKGROUND: Human papillomavirus (HPV) associated malignancies are the leading cause of cancer death in Botswana. We sought to determine causative HPV types in patients with anogenital malignancies in Botswana to inform vaccine strategy. METHODS: We used formalin-fixed and paraffin-embedded (FFPE) tissue blocks from patients diagnosed with anal, penile and vulvar squamous cell carcinomas between the years, 2014 and 2016. Presence of HPV 16, 18, or other high-risk (HR) types was detected using Abbott m2000 real-time PCR platform. Tissues with other high-risk types were subsequently analysed us

The lymphatic superficial circumflex iliac vessels deep branch perforator flap: A new preventive approach to lower limb lymphedema after groin dissection-preliminary evidence.

OBJECTIVE: Groin dissection is the procedure with the highest risk of lower limb lymphedema. As lymph stasis causes irreversible alterations to the limb over time, therapies should be administered in early stages,or better yet, lymphatic drainage impairment should be prevented. We developed a new preventive approach to lymphedema after groin dissection, and we report our preliminary experience. PATIENTS AND METHODS: We enrolled 5 patients undergoing bilateral groin dissection for vulvar cancer. Preoperative study of lower limbs lymphatic function was obtained by lymphoscintigraphy, with lymphati

Prognostic value of bilateral positive nodes in squamous cell cancer of the vulva.

OBJECTIVES: The aim of the current study was, first, to determine whether laterality of lymph node metastases has prognostic significance, independent of the number of lymph node metastases. Second was to determine the prognostic significance of extracapsular spread irrespective of the number of lymph node metastases. METHODS: Data on 134 patients with stage III/IVA vulva cancer from 1982 till 2004 and treated with curative intent in either the Academic Medical Centre in Amsterdam or the Mercy Hospital for Women in Melbourne were reviewed. The impact of the number of lymph node metastases, ex

Carcinoma of the vulva: a retrospective review of 37 cases at a regional cancer centre in South India.

A retrospective review of 37 cases of carcinoma of the vulva presenting between 1996 and 2000 has been carried out. Thirty-three cases were managed with curative intent and four cases with advanced loco-regional disease were managed with palliative intent. The surgical treatment consisted of wide excision in one case, radical vulvectomy (RV) in six cases, radical vulvectomy and bilateral groin node dissection (RV+BGND) in 25 cases and radical vulvectomy and unilateral groin node dissection in one case. Nine of these 33 women also received adjuvant chemotherapy preoperatively in the hope of ach

[Postoperative interferon hydrogel treatment. A method for the successful therapy of chronic persistent giant condylomas in an immunologically deficient patient with Hodgkin's disease].

The local application of recombinant interferon alpha 2c hydrogel (1.10(6) I.U. recombinant interferon alpha 2c/g hydrogel) given as an adjuvant therapy after electrosurgery led to a complete cure, without relapse, of previously recalcitrant giant condylomata acuminata similar in appearance to Buschke-Löwenstein's tumors in a 19-year-old female patient suffering from Hodgkin's disease (stage II/III). Recombinant interferon alpha 2c hydrogel given as an adjuvant therapy to surgery may have more antiviral and antiproliferative effects than immunomodulating activities. This combined topical appli

A review of clinical trials of human papillomavirus prophylactic vaccines.

End of study analyses of the phase III trials of prophylactic human papillomavirus (HPV) virus-like particle (VLP) vaccines in young women are now largely completed. Two distinct vaccines were evaluated, Gardasil(®) (Merck & Co., Whitehouse Station, NJ USA) a quadrivalent vaccine containing VLPs of types 6, 11, 16 and 18 and Cervarix(®) (GlaxoSmithKline Biologicals, Rixensart, Belgium), a bivalent vaccine containing VLPs of types 16 and 18. Both vaccines exhibited excellent safety and immunogenicity profiles. The vaccines also demonstrated remarkably high and similar efficacy against the vacci

Absence of p53 mutations in benign and pre-malignant male genital lesions with over-expressed p53 protein.

Mutations of the tumor-suppressor gene p53 are common in epithelial tumors. Clonal mutations of p53 have been found in cervical and vulvar carcinomas negative for human papillomavirus (HPV), though at least in cervical cancer HPV infection and p53 mutations are not mutually exclusive. We have previously shown that about 40% of male genital warts and bowenoid papulosis lesions exhibit immunohistochemically detectable aberrant p53 protein, irrespective of the presence of HPV DNA. We studied p53 mutations in exons 4-8 with SSCP and sequencing in 13 male patients with 1 to 3 therapy-resistant geni

Endovascular Treatment of Peripheral Vascular Blowout Syndrome in End-Stage Malignancies.

BACKGROUND: Vascular blowout syndrome (VBOS) is a life-threatening condition secondary to direct tumor encasement or invasion in advanced stage malignancies. Endovascular management can be used as an alternative to surgical treatment in this fragile patient population, providing a minimally invasive measure both acutely and prophylactically. METHODS: Three patients with peripheral VBOS secondary to advanced stage malignancies underwent successful endovascular treatment. Technical success was obtained in all patients with nonsignificant perioperative complications. RESULTS: Endovascular manag

Machine Learning-Based Thermal Imaging for Vulvar Intraepithelial Neoplasia Detection.

Vulvar intraepithelial neoplasia (VIN) is increasing in prevalence, yet screening options remain limited and existing diagnostic methods show low accuracy. This study evaluates infrared thermal imaging as an alternative screening approach for VIN detection. We analyzed thermal images from 51 patients with histopathologically confirmed VIN, captured using a FLIR A400 thermal camera. Temperature distributions of healthy vulvar tissue were first characterized to establish baseline values. Thermal features of VIN lesions were then extracted and optimized using principal component analysis (PCA). T

Educational needs of patients with pre-cancerous vulval conditions: An audit and cross-sectional survey.

BACKGROUND: A systematic approach to surveillance of vulval dermatoses and pre-cancers is recommended for early detection of vulval cancer and nurses are crucial in care delivery. Typically, patients with gynaecological cancers have significant unmet information needs; however the extent in patients with vulval dermatoses and pre-cancers is unknown. AIM: To evaluate knowledge gaps and educational requirements of patients attending vulvoscopy clinics at a Western Australian tertiary centre. METHODS: A retrospective audit examined data of 307 patients attending vulvoscopy clinics. A cross-sect

Synchronous Epidermodysplasia Verruciformis and Intraepithelial Lesion of the Vulva Is Caused by Coinfection With Alpha-Human Papillomavirus and Beta-Human Papillomavirus Genotypes and Facilitated by Mutations in Cell-Mediated Immunity Genes.

CONTEXT.—: There have been exceedingly few reports of epidermodysplasia verruciformis (EV) or EV-like lesions in the vulva. We describe the first observation of vulvar lesions displaying synchronous EV-like histology and conventional high-grade squamous intraepithelial lesion (HSIL), a finding hitherto unreported in medical literature. OBJECTIVES.—: To describe this novel vulvar lesion with hybrid features of HSIL and EV, attempt to confirm the hypothesis of coinfection with α and β human papillomavirus (α-HPV and β-HPV) genotypes, and describe relevant underlying genetic mutations. DESIGN.—

Molecular subclassification of vulvar squamous cell carcinoma: reproducibility and prognostic significance of a novel surgical technique.

OBJECTIVES: Vulvar squamous cell carcinoma is subclassified into three prognostically relevant groups: (i) human papillomavirus (HPV) associated, (ii) HPV independent p53 abnormal (mutant pattern), and (iii) HPV independent p53 wild type. Immunohistochemistry for p16 and p53 serve as surrogates for HPV viral integration and TP53 mutational status. We assessed the reproducibility of the subclassification based on p16 and p53 immunohistochemistry and evaluated the prognostic significance of vulvar squamous cell carcinoma molecular subgroups in a patient cohort treated by vulvar field resection s
