'Antibiotics are like gold': a qualitative study of patient perspectives on the use of antibiotics without a prescription.

BACKGROUND: Using antibiotics without a prescription (non-prescription use) is common in the USA and contributes to antibiotic misuse, potentially harming individuals and threatening public health. At the individual level, safety issues related to non-prescription use include adverse drug reactions and disruption of healthy microbiomes. At the public health level, non-prescription use increases the risk of antimicrobial resistance. OBJECTIVES: This qualitative study explored the reasons and motivations underlying non-prescription use among adult outpatients with varying healthcare coverage an

Knowledge, attitude, and practices regarding antibiotic use and antimicrobial resistance among urban slum dwellers in Uganda.

BACKGROUND: Antimicrobial resistance (AMR) remains a public health threat especially in low-and-middle-income countries (LMICs). Urban slum … health education programs. Health promotion messaging should emphasize AMR concepts and dangers of drug misuse. Antimicrobial stewardship initiatives should trickle down to the local citizen.

Silent scars: understanding interpersonal sensitivity, paranoid ideation, and hostility from adverse childhood experiences in Jamaica.

BACKGROUND: Adverse childhood experiences (ACEs) are known to predict adverse outcomes related to physical and mental health, including anxi … mes. Physical abuse had an inverse relationship with paranoid ideation, as did household alcohol or drug misuse with interpersonal sensitivity and hostility. The relationships between ACEs and the interpersonal psychopathological vulnerabilities were not moderated by the sociodemographic variables. CONCLUSIONS: The number of ACEs (ACE-IQ score) was positivel …

Opioid toxicity deaths in Indigenous people who experienced incarceration in Ontario, Canada 2015-2020: a whole population retrospective cohort study.

BACKGROUND: While Indigenous people are overrepresented in Canada's prisons and in the toxic drug supply crisis, we lack data on the harms related to opioids for Indigenous people with experiences of incarceration. We aimed to examine opioid toxicity deaths in Indigenous peoples who experienced incarceration and to compare opioid toxicity mortality rates with rates for people with no incarceration. METHODS: This retrospective cohort study linked correctional data for all people who were incarcerated in provincial correctional facilities and coronial data for all people who died from opioid to

Gut microbiota-derived ursodeoxycholic acid from neonatal dairy calves improves intestinal homeostasis and colitis to attenuate extended-spectrum β-lactamase-producing enteroaggregative Escherichia coli infection.

BACKGROUND: Antimicrobials are often used to prevent and treat diarrhea induced by enteroaggregative Escherichia coli (EAEC) in young ruminants. However, drug overuse or misuse accelerates the spread of multidrug-resistant extended-spectrum β-lactamase (ESBL)-producing E. coli. Thus, supplementary foods as alternatives to antibiotics are needed to prevent colibacillus diarrhea in neonatal dairy calves. Ursodeoxycholic acid (UDCA), a therapeutic bile acid, helps alleviate colitis. However, how UDCA helps alleviate ESBL-EAEC-induced clinical symptoms and colitis remains unclear. RESULTS: We inv

Community knowledge, perceptions, and practices regarding malaria and its control in Jabi Tehnan district, Amhara Region, Northwest Ethiopia.

BACKGROUND: Use of long-lasting insecticidal nets (LLINs), indoor residual spraying (IRS), community-based malaria education, prompt diagnosis and treatment are key programme components of malaria prevention and control in Ethiopia. However, the effectiveness of these interventions is often undermined by various challenges, including insecticide and drug resistance, the plasticity of malaria vectors feeding and biting behaviour, and certain household factors that lead to misuse and poor utilization of LLINs. The primary objective of this study was to document households' perceptions towards ma

Frequency, Predictors, and Medical Record Documentation of Chemical Coping Among Advanced Cancer Patients.

BACKGROUND: In this prospective study, we determined the frequency of opioid-related chemical coping among advanced cancer patients, as diagnosed by palliative medicine specialists. We also determined predictors for chemical coping and the concordance between the physician's diagnosis and documentation in the medical records. PATIENTS AND METHODS: Palliative medicine specialists evaluated and diagnosed consecutive patients seen for chemical coping. The proportion of patients identified as chemically coping was compared with the proportion documented in the medical records. Demographic data; c

Invasive aspergillosis in developing countries.

To review invasive aspergillosis (IA) in developing countries, we included those countries, which are mentioned in the document of the International Monetary Fund (IMF), called the Emerging and Developing Economies List, 2009. A PubMed/Medline literature search was performed for studies concerning IA reported during 1970 through March 2010 from these countries. IA is an important cause of morbidity and mortality of hospitalized patients of developing countries, though the exact frequency of the disease is not known due to inadequate reporting and facilities to diagnose. Only a handful of cente

National consumption of antimicrobials intended for use in animal livestock health: a retrospective data analysis of antimicrobial imports to Uganda from 2018 to 2020.

BACKGROUND: Antimicrobial use in animals is one of the major drivers for the emergence and spread of resistant microorganisms. Antimicrobial resistance (AMR) can spread from animals to humans and vice versa. However, there is scanty data on antimicrobial consumption in livestock in low and middle income countries especially Uganda. Monitoring antimicrobial consumption and use (AMCU) in the veterinary sector is important to identify areas of overuse and misuse and to design targeted interventions to reduce the need for unintentional exposure. This study aimed to quantify and characterize by pha

A novel multisite model to facilitate hepatitis C virus elimination in people experiencing homelessness.

BACKGROUND & AIMS: Only a handful of countries are on target to achieve elimination of HCV by 2030. People experiencing homelessness (PEH) remain an important HCV reservoir. The END C study evaluated clinical, patient reported, and health economic outcomes of a decentralised integrated model. METHODS: This prospective study assessed a decentralised regional service based at multiple homeless sites in southeast England. Novel linkage-care strategies were used. We assessed generic and liver specific health-related quality of life (HRQoL) (SF-12v2; EQ-5D-5L, and SFLDQol) pre-/post-HCV treatment,

The continuum of hepatitis C care for criminal justice involved adults in the DAA era: a retrospective cohort study demonstrating limited treatment uptake and inconsistent linkage to community-based care.

BACKGROUND: Incarcerated populations are disproportionately burdened by hepatitis C virus (HCV) infection. The introduction of highly-effective, direct-acting antiviral (DAA) treatment has potential to substantially reduce the burden of liver disease in this population, but accurate information about access to and utilization of this treatment is currently limited. The goals of this study were to characterize receipt of HCV care and treatment services for a cohort of HCV-infected adults identified in a state prison system, and to describe the complex health needs of this population. METHODS:

Medical entrance examination pattern needs tweaking!

Androgenic-anabolic steroids (AAS) are synthetic derivatives of the male hormone testosterone. They can exert strong effects on the human body that may be beneficial for athletic performance. A review of the literature revealed that most laboratory studies did not investigate the actual doses of AAS currently abused in the field. Therefore, those studies may not reflect the actual (adverse) effects of steroids. The available scientific literature describes that short-term administration of these drugs by athletes can increase strength and bodyweight. Strength gains of about 5-20% of the initia

[Rapid determination of five antipyretic analgesics in surface water by online solid phase extraction and ultra-high performance liquid chromatography-tandem mass spectrometry].

Antipyretic analgesics are typical pharmaceutical and personal care products (PPCPs) that are widely used in our daily life because they relieve fever and pain, and have anti-inflammatory and anti-rheumatic properties. These drugs inhibit the synthesis and release of prostaglandins (PGs) in the neurons of the anterior hypothalamus and exert therapeutic effects as a consequence. However, these drugs are relatively commonly misused and abused, often owing to a lack of proper medication guidance. As a result, these drugs enter the environment via various pathways, including wastewater treatment p

A qualitative process evaluation of SBIRT implementation in pediatric trauma centers using the Science to Service Laboratory implementation strategy.

BACKGROUND: Screening, Brief Intervention, and Referral to Treatment (SBIRT) is an evidence-based practice that can identify adolescents who use alcohol and other drugs and support proper referral to treatment. Despite an American College of Surgeons mandate to deliver SBIRT in pediatric trauma care, trauma centers throughout the United States have faced numerous patient, provider, and organizational level barriers to SBIRT implementation. The Implementing Alcohol Misuse Screening, Brief Intervention, and Referral to Treatment Study (IAMSBIRT) aimed to implement SBIRT across 10 pediatric traum

Use of Naltrexone for Patients With Stimulant Use Disorder in Malaysia: Protocol for a Retrospective Cohort Study.

BACKGROUND: Naltrexone is an opioid receptor antagonist. Naltrexone is used to block the euphoric and sedative effects of drugs such as heroin, codeine, and morphine. The medication helps to bind and block opioid receptors to decrease opioid cravings. In Malaysia, naltrexone has been used for maintenance treatments for heroin and alcohol since 1996. However, since 2011, naltrexone has been used as an off-label stimulant use disorder (StUD) treatment to achieve abstinence. OBJECTIVE: This study aims to determine the abstinence among StUD and non-StUD patients treated and without naltrexone. M

Clinical features of the failed-back syndrome.

This study comprises 78 patients who were treated for chronic back pain at a multidisciplinary, multimodal pain treatment center. These patients were selected from 494 patients examined by the authors because all of their previous medical records, operative notes, and imaging studies were available for review. The records and imaging studies were reviewed independently by a neurosurgeon and an orthopedist, and a retrospective decision was made concerning the historical and physical findings correlated with imaging studies in order to provide justification for the intervention. At the time of a

Differences in opioid use and overdose among younger and older justice-impacted adults.

BACKGROUND: Justice-impacted persons aged 40 and up with substance use disorders (SUD) demonstrate increased health risks and health disparities relative to general population peers. Persons with SUD are less likely to age out of criminal behavior, appearing on criminal dockets, in jails, prisons, and under community supervision throughout the life course, with greater community-level cost burdens as they age. Justice system involvement presents health risks that compound with SUD to undermine well-being, which is amplified as people age and experience age-related health decline. Propensity fo
