Neuropsychological impairment and psychosis in mania.

Deficits involving executive function, working memory, speed of information processing, and new learning occur in many people with mania. Factors that predict impairment remain poorly understood, but there are indications that psychotic features may correspond with increased risk of neurocognitive dysfunction during manic episodes. The current study examined neuropsychological function in 40 inpatients with bipolar I mania, 24 of whom presented with psychotic features. Compared to a control group, the inpatients showed worse executive function, speed of information processing, new learning, an

Cortical activation associated with the experience of auditory hallucinations and perception of human speech in schizophrenia: a PET correlation study.

A [H(2)(15)O] PET correlation analysis technique was employed to correlate brain activations associated with self-reports of auditory hallucinations in hallucinating patients (n=8) and perception of transient, random human speech in non-hallucinating (n=7) patients and normal control subjects (n=8). Perception of externally generated human speech amongst the non-hallucinating and normal control participants was associated with a consistent pattern of extensive bilateral auditory cortex activation (Brodmann areas 40/41/42/22). Hallucinating participants demonstrated a network of cortical activa

[Clinical characteristics of subcortical arteriosclerotic encephalopathy (Binswanger's disease)].

The subcortical arteriosclerotic encephalopathy (Binswanger's disease) usually starts over 50 years, and it is characterised by slow progression, dementia and stroke-like episodes. The authors analyzed the data of 60 patients, treated in the Neurological Department of Medical University of Pécs between 1994-1997. There was no difference between genders. Among the risk factors only the hypertension and smoking showed correlation with incidence of the disease. The leukoaraiosis found by the CT or MRI was graded according to severity and extension and was correlated with other clinical data (dupl

Neurobehavioral manifestations in obstructive sleep apnea syndrome before and after treatment with continuous positive airway pressure.

Obstructive sleep apnea syndrome (OSAS) is characterized by recurrent apneas during sleep, resulting in repetitive hypoxemic episodes and a constant interruption of the normal sleep pattern. Vigilance impairment and neuropsychological deficits are among the main symptoms seen in this condition. One of the major questions in this field concerns the reciprocal interactions between nocturnal hypoxemia, sleep disruption, excessive daytime sleepiness and cognitive deficits. Results of this study suggest that vigilance impairment is attributable mostly to nocturnal hypoxemia. However, in cognitive d

Innominate artery compression of the trachea in infants with reflex apnea.

Compression of the trachea by an anomalous innominate artery in association with reflex apnea is a frequent cause of respiratory arrest in infants. Once considered, tracheoscopy is mandatory to rule out this disorder. Surgical correction of this condition by innominate arteriopexy has proven to be a very effective method of management. Seventy-eight patients with tracheal compression by an anomalous innominate artery managed by the authors at The Children's Memorial Hospital in Chicago between January 1977 and December 1979 are presented. In this series, 28 patients had a history of one or mor

Gray matter morphological anomalies in the cerebellar vermis in first-episode schizophrenia patients with cognitive deficits.

BACKGROUND: Cognitive deficits are a core feature of early schizophrenia. However, the pathological foundations underlying cognitive deficits are still unknown. The present study examined the association between gray matter density and cognitive deficits in first-episode schizophrenia. METHOD: Structural magnetic resonance imaging of the brain was performed in 34 first-episode schizophrenia patients and 21 healthy controls. Patients were divided into two subgroups according to working memory task performance. The three groups were well matched for age, gender, and education, and the two patie

[Implantable cardioverter-defibrillators in the prevention of sudden cardiac death].

The aim of this work is to characterize and analyse the spectrum of therapies delivered from implantable cardioverter-defibrillators (ICD), to evaluate their effectivity and to mark the most effective types of antitachycardia stimulations (ATP), cardioversion (CV) and defibrillation (CD). To compare our results with references and give precautions of trouble-shootings. Our patients had implanted ICDs according to standard criteria for ICD implantations. Before discharge from hospital we performed predischarge test of their ICD. Than we followed them periodically each three months. We have obse

Computer simulations of a dynamic sodium pump-mediated hyperpolarization and short-term motor memory in the spinal locomotor network of Xenopus frog tadpoles.

Simple four-neuron computational models comprising bilateral pairs of excitatory dIN and inhibitory cIN neurons were used to test several hypotheses concerning the role of electrogenic sodium pumps in shaping swimming CPG output in Xenopus tadpoles. The initial model had no sodium pumps and generated continuous swim-like rhythmic activity. In real tadpoles, activity-dependent "dynamic" sodium pumps are proposed to mediate the postswim ultraslow afterhyperpolarization (usAHP) apparent in most cINs that reduces subsequent swim episode durations, producing a form of short-term motor memory (STMM)

How revealing are insertable loop recorders in pediatrics?

INTRODUCTION: An insertable loop recorder (ILR) in patients with infrequent syncope or palpitations may be useful to decide management strategies, including clinical observation, medical therapy, pacemaker, or implantable cardioverter defibrillator (ICD). We sought to determine the diagnostic utility of the Reveal ILR (Medtronic, Inc., Minneapolis, MN, USA) in pediatric patients. METHODS: Retrospective review of clinical data, indications, findings, and therapeutic decision in 27 consecutive patients who underwent ILR implantation from 1998-2007. RESULTS: The median age was 14.8 years (2-25

Bilateral Acute Hippocampal Ischemia in Two Patients Abusing Cocaine: What is the Outcome?

Hippocampal ischemia is a rare complication of cocaine abuse that has been thought to arise from vasospasm, anoxic injury, and/or catecholaminergic excitotoxicity. We present two cases of patients abusing cocaine, who presented with an acute onset anterograde amnesia due to bilateral hippocampal ischemia, and had different outcomes. Case 1 is a 49-year-old male with a history of IV heroin abuse who presented after being found down for an unknown period of time. He awoke with no memory of events leading up to hospitalization and was unable to retain new information. Urine toxicology was positiv

Severe respiratory disease caused by human respiratory syncytial virus impairs language learning during early infancy.

Human respiratory syncytial virus infection is a leading cause of pediatric morbidity and mortality. A previous murine study showed that during severe acute respiratory infections the virus invades the central nervous system, and that infected animals evolve with long-lasting learning difficulties associated with long-term potentiation impairment in their hippocampus. We hypothesized here that human infants who presented a severe episode of respiratory syncytial virus infection before 6 months of age would develop long-term learning difficulties. We measured the acquisition of the native phone

Correlates of cognitive impairment in first episode schizophrenia: the EUFEST study.

BACKGROUND: Profile and correlates of cognitive deficits in first episode (FE) schizophrenia patients are still debated. The present study is aimed to clarify in a large sample of FE patients the extent of impairment in key cognitive domains and its relationships with demographic and clinical variables. METHOD: The European First Episode Schizophrenia Trial collected demographic, clinical and neurocognitive baseline data in 498 FE patients with minimal or no prior exposure to antipsychotics. Two-hundred-twenty healthy subjects (HS) were also evaluated. Neurocognitive assessment included the R

The behavioral complications of pallidal stimulation: a case report.

We report a case of recurrent manic episodes associated with chronic deep brain stimulation (DBS) targeting globus pallidus (GP) in the treatment of Parkinson's disease (PD). Cardinal PD symptoms and dyskinesia improved with DBS, and neuropsychological testing found improvements in visuospatial measures associated with left DBS and in verbal memory with right DBS when compared to the patient's preoperative baseline. Under conditions of right, left, and bilateral DBS, the patient experienced bouts of mania and hypomania lasting several days at a time. Positron emission tomography (PET) with (15

Transient global amnesia.

Transient global amnesia (TGA) is a transient, benign neurological syndrome, characterized by global loss of memory, preserved consciousness and self-awareness, associated with some behavioral changes (in particular, repetitive questioning). It generally resolves within 24 h. Mild brain stem symptoms can often be demonstrated during the attack, but major neurological abnormalities never occur. The only sequel is a permanent amnesic gap for the duration of the episode. The episode is often preceded by typical precipitating events, such as physical activity, emotional stress, acute pain, compris

Postpartum delirious mania in a patient with diagnosis of bipolar disorder after cesarean delivery: a case report.

BACKGROUND: Postpartum depressive episodes are common. Patients with bipolar disorder (BD) may also experience manic or mixed episodes postpartum, but cases of delirious mania (DM)-characterized by rapidly evolving delirium, mania, and may involve psychotic symptoms-are rarely reported. Given its severity, frequent underdiagnosis, and life-threatening risks, a thorough exploration of its clinical features, potential risk factors, and treatment strategies can aid clinicians in early recognition and intervention, thereby improving patient outcomes. CASE PRESENTATION: We report a rare case of po

Drug-Induced Type 1 Brugada Pattern: A Case Report.

Brugada syndrome is a rare genetic disorder, classified as an autosomal dominant inherited cardiac sodium channelopathy. It is associated with a high incidence of syncope and sudden death due to ventricular tachycardia or ventricular fibrillation in patients with structurally normal hearts. This report presents the case of a 33-year-old male who experienced recurrent syncopal episodes over the course of a year. The episodes resembled seizure activity, with associated memory loss of the events and no preceding warning symptoms. The patient was ultimately diagnosed with a drug-induced type 1 Bru

Cannabinoid facilitation of fear extinction memory recall in humans.

A first-line approach to treat anxiety disorders is exposure-based therapy, which relies on extinction processes such as repeatedly exposing the patient to stimuli (conditioned stimuli; CS) associated with the traumatic, fear-related memory. However, a significant number of patients fail to maintain their gains, partly attributed to the fact that this inhibitory learning and its maintenance is temporary and conditioned fear responses can return. Animal studies have shown that activation of the cannabinoid system during extinction learning enhances fear extinction and its retention. Specificall
