Less opioid at no extra cost: charge-code decomposition of an ERAS pathway for adolescent scoliosis surgery.

PURPOSE: Enhanced recovery after surgery (ERAS) pathways using dexamethasone and liposomal bupivacaine (Dex/LB) have been shown to reduce opioid use after posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS), but their economic impact is unknown. This study examined whether the Dex/LB pathway increases hospital cost, analyzing charge lines rather than encounter-level totals. METHODS: Retrospective sequential implementation study at a single pediatric center. Of 140 consecutive AIS patients undergoing PSF (2019-2025), a transitional cohort (n = 20) was excluded, leaving 120

Development and Evaluation of a Physiologically Based Pharmacokinetic Model for Cipepofol Across Diverse Clinical Populations.

Background/Objectives: Cipepofol is a novel intravenous anesthetic whose pharmacokinetics (PK) may vary with dosing regimens, sampling sites, and physiological differences across populations. However, clinical PK data remain fragmented across study settings and are limited for special populations and individualized perioperative use, highlighting the need for a mechanistic modeling framework. This study aimed to develop and evaluate a physiologically based pharmacokinetic (PBPK) model for cipepofol across diverse populations. Methods: Clinical data from nine studies were included, comprising 3

Effect of parental presence on intravenous sedation in children with cyanotic heart disease undergoing CT angiography: a prospective randomized trial.

PURPOSE: Congenital heart disease (CHD) affects 9.3 per 1000 live births in Asia, with 25% being cyanotic. Advanced imaging such as CT angiography is essential, but requires effective sedation in this high-risk group. While parental presence is known to reduce anxiety during mask induction in operating rooms, its effect on intravenous sedation in non-operating room anesthesia (NORA) settings, particularly for diagnostic imaging, has not been studied. This trial evaluates the effect of parental presence during intravenous procedural sedation on ketamine requirements for children with cyanotic C

Retrograde cricopharyngeus dysfunction: what the gastroenterologist should know.

PURPOSE OF REVIEW: To review current literature about retrograde cricopharyngeus dysfunction (R-CPD) etiology, pathophysiology, clinical presentation and management. RECENT FINDINGS: R-CPD is a clinical condition characterized by inability to burp that is often associated with a variety of other gastrointestinal symptoms. Limited data suggest that R-CPD may affect up to 22% of the general population, with 13% experiencing severely troublesome symptoms. Symptoms usually begin in childhood, though diagnosis typically occurs many years later after prolonged symptom burden. Gastroesophageal reflu

The SAP130/Mincle axis was involved in sevoflurane-induced neuronal death and microglial activation in juvenile mice.

INTRODUCTION: Sevoflurane is widely used in pediatric anesthesia and has raised concerns for years regarding its neurotoxic effects on the developing brain. Studies have shown that sevoflurane can lead to neuronal cell death and neuroinflammation, which further contribute to sevoflurane-induced neurotoxicity manifested as delirium or cognitive deficits. However, the molecular mechanism remains poorly understood. A factor of interest is Sin3A-associated protein 130 (SAP130), which can be released by dead or damaged cells and trigger sterile inflammation, exacerbating tissue damage by activating…

Efficacy of Transversus Thoracic Plane Block for Pain Management in Cardiac Surgeries.

PURPOSE OF REVIEW: Effective pain management in cardiac surgery presents as a continuous challenge related to the intensity of postoperative pain and reliance on opioid therapy. The dependance of opioid-based therapies is concerning, as these therapies carry risk future addiction and potential severe side effects. The transversus thoracic plane block (TTPB) has emerged as a promising regional anesthesia technique that blocks the anterior branches of the intercostal nerves in the chest wall, potentially providing improved analgesia for cardiac surgery patients. The present investigation evaluat

Efficacy and safety of perioperative analgesia in PENG block for children undergoing hip surgery with developmental dysplasia: study protocol for a randomized, double-blinded, non-inferiority trial.

BACKGROUND: Surgical intervention is critical in the treatment of hip developmental dysplasia in children. Perioperative analgesia, usually based on high opioid dosages, is frequently used in these patients. In some circumstances, regional anesthetic procedures such as caudal block and lumbar plexus block have also been used. Previous research has shown that pericapsular nerve group (PENG) block provides excellent anesthetic efficacy during open hip surgery in adults. PENG block is thought to provide a similar analgesia effect comparable to caudal block. METHODS: This study is a single-center

It's Hip to Go Home: An Evaluation of Outpatient Alveolar Bone Grafting in Patients With Cleft Palate.

INTRODUCTION: Secondary alveolar bone grafting (ABG) is a common procedure performed at cleft care centers used to fill the alveolar cleft. The advent of techniques such as minimally invasive trephine drill harvest and placement of continuous-infusion pain pumps at the donor site has made outpatient ABG an increasingly feasible and cost-effective procedure. However, enhanced recovery after surgery protocols to maximize pain control and recovery times for this patient population have not been well established. METHODS: A retrospective single-institution review was conducted of pediatric patien
