Possibility of clinical application of tremor in the upper limb.

A procedure to evaluate the function of the glenohumeral joint (i.e., shoulder joint) for persons with shoulder disorders was proposed by analyzing tremor. In order to compare subjects with shoulder disorders to healthy subjects without disorders, characteristics of tremor for the healthy persons under various angles of glenohumeral joint and under the dominant and no-dominant sides of the upper limb were obtained by peak frequency and power spectrum as basic data. As for the persons with rotator cuff injury, the ratio of total powers for the affected side and the healthy side was obtained as

"Missing Mobility": Perspectives of North American Firefighters on the Risks and Causes of Work-Related Shoulder Disorders: A Qualitative Study.

BACKGROUND: Firefighters are routinely exposed to significant work-related musculoskeletal disorders (WRMSDs) which can sometimes be career-ending due to the workplace stressors and the physical demands of the job. Shoulder disorders are the third most frequent WRMSDs that cause pain, disability, and morbidity in the general working population. However, little is known about the task-specific causes and risk factors for work-related shoulder disorders (WSDs) among firefighters (FFs). OBJECTIVES: To explore the perspectives of FFs who experienced WSDs with respect to the following: (a) risk fa

Maxillary Sarcomatoid Carcinoma with Rhabdo-Myosarcomatous Dedifferentiation: a Clinic-Pathological Emphasis.

BACKGROUND: Osteoporosis, whether generalized or localized, presents significant challenges in arthroscopic procedures, particularly in surgeries such as ligament reconstructions and rotator cuff repairs, which depend on fixation through interference screws and/or anchors. These challenges are especially pronounced in women, the elderly, and individuals with chronic injuries. The literature discusses various techniques to mitigate these issues. OBJECTIVE: Despite the recognition of these challenges, there is no clear consensus on the most effective methods to address them. Current literature

Shoulder injuries in professional rugby: a retrospective analysis.

BACKGROUND: In the literature, little is known about the level and pattern of rugby injuries. Of the shoulder injuries reported, 51% of these are caused during a tackle, and 65% of all match injuries affected the shoulder. OBJECTIVE: The study aims to describe a sport-specific unique intra-articular shoulder pathology of professional rugby players, who presented with persistent pain and dysfunction despite physiotherapeutic treatment and rest. METHOD: This study is a retrospective analysis set at a university sports medicine clinic. Eighty-seven professional rugby players, referred by their

Comparative responsiveness of shoulder patient-reported outcome measures (PROMs) to rotator cuff repair surgery and healing.

BACKGROUND: Patient-reported outcome measures (PROMs) are routinely used to assess pain, function, and quality of life in shoulder care. Although rotator cuff repair (RCR) is a highly effective treatment for symptomatic tears, the relationship between structural healing and PROM responsiveness remains unclear. This study aimed to evaluate and compare the responsiveness of 5 common shoulder PROMs-the Penn Shoulder Score, modified American Shoulder and Elbow Surgeons score, Single Assessment Numeric Evaluation, Shoulder Activity Level (SAL), and Patient-Reported Outcome Measure Information Syste

Anchorless Arthroscopic Transosseous and Anchored Arthroscopic Transosseous Equivalent Rotator Cuff Repair Show No Differences in Structural Integrity or Patient-reported Outcomes in a Matched Cohort.

BACKGROUND: Anchored transosseous equivalent suture-bridge technique (TOE) is widely used for arthroscopic rotator cuff repair. It is unknown how patient outcomes scores, ROM, and integrity of the rotator cuff after repair using this anchored technique compare with those after repair using an anchorless transosseous technique (TO). QUESTIONS/PURPOSES: (1) What are the differences in patient-reported outcomes (American Shoulder and Elbow Surgeons [ASES] score) and shoulder ROM between TO and TOE rotator cuff repair techniques at 1 and 2 years after surgery? (2) What is the difference in repair

Clinical and functional outcomes of a novel transosseous device to treat rotator cuff tears: A minimum 2-year follow-up.

PURPOSE: Despite the improvements in the repair equipment techniques of rotator cuff, a gold standard method has not been defined yet and transosseous fixation through deltoid-splitting (mini-open) approach is still regarded as a good option. The primary purpose of this study is to present satisfactory clinical and functional outcomes with a novel transosseous device in full-thickness rotator cuff tear through deltoid-splitting. MATERIALS AND METHODS: This retrospective study was performed on 70 consecutive patients who underwent surgery by a single surgeon from June 2014 to June 2016 for a f

The Association of Osteoporosis and Bisphosphonate Use With Revision Shoulder Surgery After Rotator Cuff Repair.

PURPOSE: To examine any association between osteoporosis and the failure of arthroscopic rotator cuff repair (ARCR) leading to revision surgery and to investigate whether the use of bisphosphonates had any influence on the observed associations. METHODS: Patients who underwent ARCR with a diagnosis of osteoporosis were identified from the PearlDiver database and stratified according to whether there was a filled prescription for a bisphosphonate in the perioperative period. Patients with osteoporosis who underwent ARCR with bisphosphonate use were compared with age- and sex-matched patients w

Poor patient-reported outcome after shoulder replacement in young patients with cuff-tear arthropathy: a matched-pair analysis from the Danish Shoulder Arthroplasty Registry.

Background and purpose - Reverse shoulder arthroplasty (RSA) has become the treatment of choice for cuff-tear arthropathy. There are, however, concerns about the longevity and the outcome of an eventual revision procedure. Thus, resurfacing hemiarthroplasty (RHA) with extended articular surface has been suggested for younger patients. We compared the patient-reported outcome of these arthroplasty designs for cuff-tear arthropathy. Patients and methods - We included patients operated on because of cuff-tear arthropathy and reported to the Danish Shoulder Arthroplasty Registry (DSR) from January

Arthroscopic Rotator Cuff Repair With Graft Augmentation of 3-Dimensional Biological Collagen for Moderate to Large Tears: A Randomized Controlled Study.

BACKGROUND: Due to the highly organized tissue and avascular nature of the rotator cuff, rotator cuff tears have limited ability to heal after the tendon is reinserted directly on the greater tubercle of the humerus. Consequently, retears are among the most common complications after rotator cuff repair. Augmentation of rotator cuff repairs with patches has been an active area of research in recent years to reduce retear rate. HYPOTHESIS: Graft augmentation with 3D collagen could prevent retears of the repaired tendon and improve tendon-bone healing in moderate to large rotator cuff tears. S

Arthroscopic treatment of rotator cuff disease.

The goal of this article is to summarize the current concepts on rotator cuff disease with an emphasis on arthroscopic treatment. Most rotator cuff tears are the result of an ongoing attritional process. Once present, a tear is likely to gradually increase in size. Partial-thickness and subscapularis tears can both be successfully treated arthroscopically if conservative management fails. Partial tears involving greater than 50% of tendon thickness should be repaired. Articular-sided partial tears involving less than 50% of the rotator cuff can reliably be treated with debridement. Are there age induced morphologic variations of the superior glenoid labrum? About 100 shoulder arthroscopies.

Different anterosuperior aspects of the glenoid labrum have already been described and are thought to be normal anatomical variations. The goals of this study were first to characterize these anterosuperior labral morphologies and then to analyze their variations in function of the patients' age. One hundred shoulder arthroscopies were recorded to study the macroscopic characteristics of the anterosuperior labrum of the glenohumeral joint and its relationships with the proximal insertion of the tendon of the long head of the biceps. Then, patients were divided into two groups in function of th

Acute prosthetic replacement for severe fractures of the proximal humerus.

Nineteen patients with 20 severe proximal humeral fractures were treated early with a Neer humeral prosthesis from 1979 to 1986. There were 18 four-part and two three-part fractures. The average age of the patients at the time of injury was 64 years. Follow-up evaluation was performed at an average of 40 months. Pain relief was satisfactory in 18 of 20 fractures. Patients were satisfied with the result in 16 of the 20 shoulders. Functional evaluation was good or fair in 15 of 20 patients. Active forward elevation averaged 72 degrees and active external rotation 16 degrees. Complications consis

Timing of corticosteroid injection within 1 year prior to rotator cuff repair was not associated with increased risk of repair failure.

BACKGROUND: Nonoperative management of atraumatic rotator cuff tears often includes corticosteroid injection (CSI). Concerns persist regarding CSI effect on tendon quality and rotator cuff repair (RCR) outcomes, though how timing of a single CSI influences RCR outcomes is unclear. This study evaluated whether CSI timing within 1 year before RCR affects repair failure, patient-reported outcomes (PROs), range of motion (ROM), and strength, compared to patients undergoing RCR without CSI. We hypothesized that CSI within 3 months of RCR would lead to higher failure rates and worse outcomes. METHO

Shoulder pain in hemiplegia.

Shoulder pain is a common complication in poststroke hemiplegia that reduces functional recovery. Many types of shoulder pathology have been suggested as causes of shoulder pain in hemiplegia, including shoulder subluxation, capsulitis, tendinitis, rotator cuff injury, bursitis, impingement syndrome, spasticity, complex regional pain syndrome, brachial plexus injury, and proximal mononeuropathies. More than one type of pathology may exist in a given patient. Shoulder pain improves in many cases with prompt diagnosis and appropriate management. Although the relationship between subluxation and p

Evolution of the throwing shoulder: why apes don't throw well and how that applies to throwing athletes.

BACKGROUND: Humans have unique characteristics making us the only primate that can throw well while most other primates throw predominately underhand with poor speed and accuracy. The purpose of this study is to illuminate the uniquely human characteristics that allow us to throw so well. When treating an injury such as a labral tear or capsule tear, this study hopes the reader can gain a better understanding of the issues that lead to the tear and those that may determine the success of treatment besides the actual repair. METHODS: In addition to a review of scientific and medical literature
